This study was performed to determine the various cardiac manifestations in DM1, their frequency and the relevance of cardiac electrophysiological study in this disease. Ten patients with DM1, five men and five women, mean age 44.3+/-7.8 years underwent neurological and cardiac assessments. The most frequent electrocardiographic findings were conduction abnormalities, essentially by intraventricular conduction defects (eight out of ten cases) such as bundle branch or fascicular blocks. Echocardiography showed alterations in systolic left ventricular function in two cases. Invasive electrophysiology testing showed sub-hisien block in three patients, requiring cardiac pacemaker implantation. Autosomal dominant forms were initially described by Silver in 1966. We report two cases, from the same Tunisian family, presenting FSP with severe amyotrophy of the hands. A brother and his sister, aged respectively 37 and 36 years old, presented practically the same clinical picture. Their parents were cousins. The female patient was hospitalized. Both patients developed gait disorders around the age of three years. Muscular atrophy of the hands arose much later, around the age of 20 years. The neurological examination disclosed a spastic gait with distal amyotrophy, severe in the hands and moderate in the feet. Sensitivity was preserved and there was no fasciculation. Purification and properties of a maltoheptaose- and maltohexaose-forming amylase produced by Bacillus subtilis US116

A new bacterial strain, identified as Bacillus subtilis US116, was isolated from Tunisian soil an... more A new bacterial strain, identified as Bacillus subtilis US116, was isolated from Tunisian soil and selected for its potential production of an atypical amylase with an industrial interest. The identification was founded on physiological tests and molecular techniques related to the 16S rRNA, 23S rRNA genes and intergenic sequences showing the highest similarity of 98% with regions in the complete genome of Bacillus subtilis 168 (accession no. Z99104). This strain produces an atypical amylase that was purified to homogeneity by a combination of acetone precipitation, size exclusion and ion exchange chromatography. The molecular mass of the enzyme is about 60 kDa as determined by SDS-PAGE. Optimal conditions for the activity of the purified enzyme are pH 6 and 65 • C. The half-life duration is about 3 h at 70 • C and 5 h at 65 • C. This enzyme belongs to the endo-type amylases according to the hydrolytic mode study using Ceralpha and Betamyl methods. It is classified as a maltoheptaose-and maltohexaose-forming amylase since it generates about 30% maltohexaose (DP6) and 20% maltoheptaose (DP7) from starch. Moreover, the minimum length of maltosaccharide cleaved by this enzyme was maltoheptaose.

L'athérosclérose en est l'étiologie dominante, responsable des vrais anévrismes dégénératifs, ce qui les oppose aux faux anévrismes. L'anévrisme poplité est le plus fréquent, associé à une haute incidence de complications artérielles thromboemboliques augmentant le risque d'amputation. L'échographie doppler est l'examen le plus performant dans le diagnostic et le suivi évolutif. La bilatéralité des anévrismes fémoral et poplité, ainsi que leur association à d'autres localisations de la maladie anévrismale en sont une particularité. Une chirurgie réglée avant la survenue de complications est l'attitude raisonnable. La revascularisation, utilisant plus volontiers du matériel autogène (artériel ou veineux) au niveau poplité, du matériel prothétique à l'étage fémoral est la stratégie habituelle.

Coarctation of the descending thoracic aorta is uncommon, with controversial etiology. Usually, s... more Coarctation of the descending thoracic aorta is uncommon, with controversial etiology. Usually, severe hypertension is the main symptom; lower extremity claudication is less often found. Surgical management remains the standard for long coarctation and provides good results. We report three cases of coarctation of the descending aorta operated at our department of cardiovascular surgery of Hospital La Rabta between January 2012 and December 2013. The median age was 19 years and the median follow-up was 16 months. Hypertension was the most common clinical manifestation. The diagnosis was made by computed tomography angiography. Two cases were treated by an aorto-aortic bypass and one by subclavian-descending aorta bypass. Recovery was excellent, with a decrease in antihypertensive medications (four to two) and restoration of all distal pulses. Middle aortic coarctation is a rare entity. Etiologies include congenital, acquired, inflammatory and infectious causes. The condition is considered a life-threatening emergency as a result of the complications associated with severe hypertension. Depending on technical considerations, open surgical bypass remains the standard repair for mid-aortic syndrome.

La migration d'endoprothèse aortique est l'un des événements les plus sévères qui peuvent compliq... more La migration d'endoprothèse aortique est l'un des événements les plus sévères qui peuvent compliquer les restaurations endovasculaires de l'aorte abdominale. Nous rapportons le cas d'un patient de 75 ans qui a bénéficié d'une cure endovasculaire d'un anévrisme de l'aorte abdominale sous-rénale par une endoprothèse couverte aorto-mono-iliaque associée à un pontage fémoro-fémoral croisé. Le contrôle angiographique réalisé en fin de procédure montrait une exclusion totale de l'anévrisme. Les suites opératoires étaient simples. Les contrôles scanographiques à 1, 6 et 12 mois étaient sans anomalies. Le contrôle à 18 mois montrait une migration proximale de l'endoprothèse à l'origine d'une endofuite de type 1 proximale associée à une disjonction prothétique avec une endofuite de type 3. L'indication d'une réintervention endovasculaire par une nouvelle endoprothèse aorto-mono-iliaque était portée. Dans l'attente de la réintervention, le patient est décédé par rupture de son anévrisme. La migration d'une endoprothèse aortique est une complication rare mais grave. Sa prévention passe par une sélection anatomique précise et un déploiement adéquat de l'endoprothèse.

Popliteal pseudoaneurysm and arteriovenous fistula after acupuncture

Few cases have previously been reported after acupuncture therapy. Such events are typically observed when the procedure is performed by non-medical acupuncturist. They may present with acute ischemia, recent claudication, distal emboli, or less commonly rupture. Duplex ultrasound should be considered as the 1st method of investigation. Computed tomography scanning is particularly accurate in making the diagnosis. Treatment strategies consist of surgery or endovascular management. The most commonly performed surgical technique for popliteal pseudoaneurysm repair is resection with bypass grafting, whereas popliteal arteriovenous fistula are usually treated surgically with ligation and primary repair. Endovascular procedure using a stent-graft is thought to be a reasonable option for treating popliteal false aneurysm or even arteriovenous fistula. We will describe two cases of an arteriovenous fistula and pseudoaneurysm of the popliteal artery that developed after acupuncture needling in the region of the popliteal artery.

The cerebrospinal fluid contained 1 mononuclear cell/mm3 and 10 mg protein/100 ml.Abnormalities of the cranio-vertebral junction, Arnold-Chiari malformation, syringomyelia and familial juvenile amyotrophic lateral sclerosis (ALS) were excluded and the diagnosis of Silver&#39;s syndrome was evoked.

Mutation analysis of the paraplegin gene (SPG7) in patients with hereditary spastic paraplegia

Mutations in the SPG7 gene, which encodes paraplegin, are responsible for an autosomal recessive ... more Mutations in the SPG7 gene, which encodes paraplegin, are responsible for an autosomal recessive hereditary spastic paraplegia (HSP). To screen the SPG7 gene in a large population of HSP families compatible with autosomal recessive transmission. The authors analyzed 136 probands with pure or complex HSP for mutations in the SPG7 using denaturation high-performance liquid chromatography and direct sequencing. The authors identified 47 variants including 6 mutations, 27 polymorphisms, and 14 changes with unknown effects. In one family from Morocco, compound c.850_851delTTinsC and c.1742_1744delTGG heterozygous mutations were shown to be causative. This family had complex HSP with cerebellar impairment. Progression of the disease was rapid, resulting in a severe disease after 8 years of duration. Also detected were 20 families with one heterozygous mutation that was not found in a large control population. The mutations produced highly defective proteins in four of these families, suggesting that they were probably causative. Direct sequencing of all exons and reverse transcription PCR experiments demonstrated the absence of a second mutation. However, the p.Ala510Val missense substitution previously described as a polymorphism was shown to be significantly associated with HSP, suggesting that it had a functional effect. SPG7 mutations account for less than 5% of hereditary spastic paraplegia (HSP) families compatible with autosomal recessive inheritance. Cerebellar signs or cerebellar atrophy on brain imaging were the most frequent additional features in patients with SPG7 HSP. Rare nucleotide variants in SPG7 are frequent, complicating routine diagnosis.

Here, we performed a genome-wide linkage analysis on a consanguineous family presenting an autosomal recessive form of HSP associated with mild mental retardation, brainstem dysraphia, and clinically asymptomatic cerebellar atrophy. We have mapped the disease locus SPG32 to chromosome 14q12-q21 within a 30-cM interval, which excludes the atlastin gene.

A generic framework for semantic video indexing based on visual concepts/contexts detection

Providing a semantic access to video data requires the development of concept detectors. However,... more Providing a semantic access to video data requires the development of concept detectors. However, semantic concepts detection is a hard task due to the large intra-class and the small inter-class variability of content. Moreover, semantic concepts co-occur together in various contexts and their occurrence may vary from one to another. Thus, it is interesting to exploit this knowledge in order to achieve satisfactory performances. In this paper we present a generic semantic video indexing scheme, called SVI_REGIMVid. It is based on three levels of analysis. The first level (level1) focuses on low-level processing such as video shot boundary/ key-frame detection, annotation tools, key-points detection and visual features extraction tools. The second level (level2) aims to build the semantic models for supervised learning of concepts/contexts. The third level (level3) enriches the semantic interpretation of concepts/ contexts by exploiting fuzzy knowledge. Despite the technological advancements in diagnostic and neurosurgical procedures, brain abscess continues to remain a potentially fatal central nervous system infection. In a descriptive study, we evaluated 30 cases of brain abscess were treated at Loghman Hakim hospital from march 2003 to December 2007. Method: Out of 30 patients 20 (66.7%) were males and 10 (33.3%) females. Mean age was 32 years (range 8 years to 70 years). There was a peak at 20 to 29 years. Predisposing factors were identified in 20 (66/7%) patients. The common predisposing factors were otic infection (26.7%), I.V drug using (16.7), neurosurgery (13.3%) and penetrating head trauma (6.7%). The most frequent presenting signs and symptoms were headache (73.3%), nausea and vomiting (70%), fever (50%), focal neurologic deficits (50%) and mental status changes (46.7%). Triad of fever, headache and focal deficit were seen in 20% of patients. Solitary abscess was found in 70% of the cases while in 30% of the cases multiple abscess were found. Frontal lobe and parietal lobe were the commonest sites involved. Cultures were found positive for microorganism in 66.7% of the cases. 45% of which were aerobic and 55% anaerobic bacteria. Staphylococci (30%), peptostreptococcus (25%), bacteroids (20%) and streptococcus (15%) were the commonest isolates. Third generation cephalosporins and metronidazole and vancomycin were used most often for empirical therapy. Burr hole aspiration was done in only 50% of the cases. Over all mortality was 23.3% in this series. Mortality rate was 33.3% in patients with medical treatment.

Tunisian hereditary spastic paraplegias: clinical variability supported by genetic heterogeneity

Hereditary spastic paraplegias (HSP) constitute a clinically and genetically heterogeneous group ... more Hereditary spastic paraplegias (HSP) constitute a clinically and genetically heterogeneous group of neurodegenerative disorders characterized by slowly progressive spasticity of the lower extremities. We performed the first clinical, epidemiological and genetic study of HSP in Southern Tunisia. We investigated 88 patients belonging to 38 unrelated Tunisian HSP families. We could establish the minimal prevalence of HSP in the district of Sfax at 5.75/100,000. Thirty-one percent of the families had a pure HSP, whereas 69% had a complicated form. The mode of inheritance was almost exclusively compatible with an autosomal recessive trait (97%, 37/38). Taking into account previously published results and new data generated in this work, genetic studies revealed significant or putative linkage to known HSP loci in 13 families (34.2%) to either SPG11 (7/38, 18.4%), SPG15 (4/38, 10.5%) or to SPG4 and SPG5 in one family each. The linkage results could be validated through the identification of two recurrent truncating mutations (R2034X and M245VfsX246) in the SPG11 gene, three different mutations (Q493X, F683LfsX685 and the novel S2004T/r.?) in the SPG15 gene, the recurrent R499C mutation in the SPG4 gene as well as the new R112X mutation in the SPG5 gene. SPG11 and SPG15 are the major responsible HSP genes in Tunisia.
