Health Services Management

This study examines the long-run relationship among the per capita private, public, and total health care expenditure and per capita gross domestic product and population growth of Turkey. We find some evidence of multivariate cointegrating relationships among the health care expenditure and gross domestic product, and population growth. We further find a bivariate cointegrating relationship between private health care expenditure and per capita gross domestic product. Accordingly, a 10% increase in gross domestic product would translate into a 21.9% increase in total health care expenditure while controlling population growth. The income elasticity of health expenditure is found to be greater than 1, implying that health care is a luxury good in Turkey. Finally we note that there exists one-way causality running from per capita gross domestic product to various definitions of health care expenses.

This study includes hospital decentralization reform in Turkey. The component of this reform aims at improving effectiveness, accessibility and quality of hospital services. The autonomization of hospitals will be achieved by giving the public hospitals an autonomous status and turn them into health enterprises (HE). HE are hospitals, autonomous both administratively and financially, which serve with a management based on principles of efficiency that are able to afford their costs with their own revenues and also able to compete. Legal arrangements related to HE were put into effect in 1995 and Turkish Higher Specialization Hospital was given the status of a HE for the first time. In this study it has been aimed to make an analysis concerned with the effect of the hospital s status change on its performance. For this reason, some top bureaucrats and hospital administrators have been interviewed; data related to years 1990-1999 have been collected; related literature has been reviewed. At the end of ten year trial, it has been understood that the hospital has not achieved the suggested organizational and financial structure. It has still had share in the government annual budget for some expenditures; hospital has had many application problems. However, the number of contract staff and the amount of hospital income have increased. Apart from having insufficient laws, it has been thought that these problems may have occurred because hospital has been well prepared physically and socially and also due to the in-service directions not well-formed.

Dış kaynak kullanımı devlet hastanelerinde temel bir strateji haline gelmekte ve son zamanlarda tanınırlığı artmaktadır. Türkiye'de bu trendi oluşturan itici güç, özel sektördeki baskıların ve rekabetin kamu sektörünü daha verimli kılacağı gerçeğine dayanarak Sağlık Bakanlığı tarafından başlatılmıştır. Bu çalışmanın amacı, sistematik analiz yoluyla Türkiye'deki hastanelerde dış kaynak kullanımı konusunda elde edilen farklı araştırma bulgularını karşılaştırmak, bütünleştirmek ve gelecekteki araştırmalara yönelik öneride bulunmaktır. Analizde 2005 ve 2011 yılları arasında yayınlanmış toplam 15 çalışma (8 tez, 7 makale) kapsama alınmıştır. Bu çalışmaların sonuçlarına göre, dış kaynak kullanımının maliyetleri azalttığı, verimliliği ve hizmetlerin kalitesini artırdığı bulunmuştur. Sadece bir çalışmada dış kaynak kullanımının hastane maliyetlerine etkisi hastane kayıtlarına dayalı olarak incelenmiştir. Başka bir çalışmada ise hastane çalışanlarının memnuniyet düzeyi düşük bulunmuştur. Sonuç olarak, Türkiye'deki kamu ve özel hastanelerde dış kaynaklardan yararlanılarak verilen hizmetlerin etkilerini belirleyen objektif kalite, verimlilik ve maliyet indikatörlerine dayalı ampirik araştırmalara gereksinim duyulduğu söylenebilir.

Reklam, sağlık sektöründe hedef kitlelere ulaşmak için kullanılan, yüksek ikna gücüne sahip bir iletişim aracıdır. Bu yolla, reklamı yapılan sağlık kurumuna veya hizmetine ilişkin farkındalık oluşturmak, reklam mesajını anlatabilmek, önerilen vaadi kabul ile hizmetten yararlanma davranışına yöneltmek amaçlanmaktadır. Öte yandan, sağlık hizmetlerinin özellikleri ve toplumun sağlık hizmetine ulaşımında eşit koşullara sahip olması gerekliliği, bu sektörde hazırlanan reklamların önemini ortaya koymaktadır. Bu çalışmada, sağlık sektöründe verilen reklam cezaları incelenerek, reklamın sağlık sektöründeki önemi ve özelliklerinin ortaya konulması amaçlanmaktadır. Bu amaçla, 2010 yılında Reklam Kurulu tarafından sağlık sektörüne ilişkin verilen 223 reklam cezası retrospektif olarak incelenmiştir. Buna göre, sektörde en fazla cezaya konu olan reklam araçları, ceza türleri, cezaların nedenleri, reklamın aykırı olduğu yasal düzenlemeler, reklam konuları ve reklamı yapılan sağlık kurumlarının özellikleri analiz edilmektedir.

Organ transplantation has become one of the fastest growing branches of medicine in recent years. However, there is an imbalance between organ demand and supply throughout the world. Turkey is among the countries that have difficulty in providing the organ necessary for organ transplantation. There are various legal, religious, educational, administrative and psychological problems in front of organ procurement. In addition, the quality of life, life expectancy and treatment costs of the patients reveals the necessity of policy development and implementation in organ transplantation and donation. In this study, sample country policies related to the subject are examined and organ transplantation policies are evaluated in Turkey and recommendations are given. 

Objective: The increase in the financial imposition of laboratories together with developing technology and evidence-based medicine applications increased the importance of quality and efficiency studies in laboratories. The aim of the study is to examine the processes of pathology laboratory, determine the causes of waste and select lean management techniques to eliminate the causes of waste. Material and Method: A five-phase plan was used for the application of lean management in the pathology laboratory. After a comprehensive literature review, these phases were determined as follows; the support of the senior management, observation, training of employees, drawing of value stream maps and creation of value stream plan. Gastric biopsy process was selected as product/service family when value stream maps were generated. In addition, fishbone diagram and Pareto analysis were used to determine the causes of waste. Results: In the current state, 73.6% of the time spent on a gastric biopsy sample in the pathology laboratory was wasted. The most common causes of waste were identified as problems with cleaning, equipment supply problems, lack of clinical information, equipment malfunction and errors. A value stream plan was developed with lean techniques to eliminate the causes of these wastes. In the pathology laboratory, a 4.6% improvement will be achieved with the achievement of the future stated target. Conclusion: A model for lean management techniques in a developing country that will enable the pathology laboratories to work more efficiently and with improved quality has been developed in this study.

Refugee's health has become an increasingly important policy concern in Turkey. The aim of this record-based study was to present the size and economic dimension of services provided to refugees in public hospitals throughout the country and explain the legal status related to health insurance. The legal regulations and data on utilization of hospitals were obtained from the Ministry of Health. The results showed that all refugees used preventive and emergency services free of charge and also registered refugees had the right to use public primary and secondary health services for free. According to utilization data, number of births was 103347, deaths 3460, operations 260212, emergency and outpatients 8.849518, and inpatients 252470 in 2014 to 2015. Total health spending for the public hospitals was approximately $ 338392896 in the same years. It can be suggested that the role and capacity of preventive health services and primary health services provided by family physicians should be increased in order to reduce the work load in public hospitals caused by the Syrian patients.

Health sector worldwide faces an increasing wastes problem; unfortunately, the problem has attracted much attention in the United States of America as opposed to other countries. A significant amount of resources (such as financial, human) have been lost due to this problem. The aims of the study are to determine causes of the hospital waste, evaluate its consequences, present measures to reduce them also raise awareness and attention to this issues. In this way, it can be ensured that the waste of health care services comes to the agenda in other countries, including Turkey.

savings were 12,118,232BRL/year for the public health care system or 1,348BRL/year per 1,000 admissions. CONCLUSIONS: RCI has shown similar effi cacy when compared to RCC with fewer costs. The cost difference was mild in magnitude but when extrapolated to a large-scale perspective these results reinforce the need of evidencebased decision making and rational resource allocation.

ÖZET Sosyal pazarlama, ticari pazarlama unsurlarını kullanarak, sosyal değişime neden olacak, programların planlanması ve uygulanmasıdır. Bu pazarlama tekniğinde amaç, organizasyonun faaliyetlerini yürütürken hiçbir finansal fayda beklemeden açık ve adil bir şekilde davranış değişimini pazarlamaktır. Sosyal pazarlama sağlık sektöründe sağlığı koruma ve geliştirme faaliyetleri ile kamu sağlık kurumlarında kullanılmaktadır. Bu çalışmada, sağlık sektöründe sosyal pazarlamanın önemi ve kullanımının ortaya konulması amaçlanmaktadır. Bu amaçla, öncelikle sosyal pazarlamanın tanımı, amacı, ticari pazarlama ile farklılıkları ve benzerlikleri teorik olarak incelenmiş, daha sonra sağlık sektöründe sosyal pazarlamanın kullanımı konusunda örneklere yer verilmiştir.

Bu politikaların yürütülmesinde belirleyici olan ise, Atatürk'ün sa lık ve sa lık hizmetlerine bakı açısıdır. Sınırlı sayıdaki çalı malar dı ında bu bakı açısı yeterince de erlendirilmemi tir. Sa lık sektöründe çok önemli de i imlerin ya andı ı bir dönemde bu de erlendirmenin yapılması yararlı olacaktır. De erlendirme yapılırken dönemin, 1. Orgut iklimi, orgute kimligini kazandiran, calisanlarin davranislarini etkileyen ve onlar tarafindan algilanan, orgute egemen olan tum ozellikler serisi olarak tanimlanabilir. Bu calisma, Ankara Yuksek Ihtisas Hastanesi’nin orgut iklimini degerlendirmek ve orgut iklimi boyutlarinin calisanlarin sosyo-demografik ve calisma durumu ozelliklerine gore farkliligini ortaya koymak amaciyla yapilmistir. Arastirmada orneklem secilmemis, evrenin tamamina (279) ulasilmasi hedeflenmistir. Hastanede 2015 yilinda gorev yapan 157 idari personel arastirma kapsamina alinmistir. Veri toplama araci olarak “Orgut Iklimi Olcegi” kullanilmistir. Olcegin guvenirlilik degerleri alt boyutlarina gore hesaplanmis ve yuksek duzeyde guvenilir bulunmustur. Hastanede en yuksek puan ortalamasinin orgut iklimi alt boyutlarindan isin iddiali olmasi, en dusuk puan ortalamasinin rol belirsizligi boyutunda oldugu saptanmistir. Bu çalışmada, sağlık sistemlerinde cevap verebilirlik üzerine yapılan uluslararası çalışmaların sistematik derleme yöntemi ile analiz edilmesi amaçlanmıştır. Çalışmada, sistematik incelemeler ve meta analizleri için geliştirilen "PRISMA" yönergesi kullanılmış, belirlenen dahil edilme kriterleri doğrultusunda ulaşılan 27 çalışma araştırma kapsamına alınmıştır. İnceleme sonucunda sağlık sistemlerinde cevap verebilirlik üzerine yapılmış çalışmaların en fazla 2017 yılında gerçekleştirildiği (n=5) ve en fazla çalışma yapılan ülkenin İran (%25.9) olduğu belirlenmiştir. İncelenen çalışmaların büyük kısmında ayakta ve yatan hastalar (%29.6) birlikte değerlendirilmiştir. Cevap verebilirlik alt boyutlarından en yüksek puanı alan alt boyutlar, saygılı tedavi (%25.9), gizlilik (%22.2) ve sosyal destek (%18.5); en düşük puanı alan boyutlar, seçim (%37.04) ve otonomi (% 25.9)'dir. Ayrıca cevap verebilirliğinin cinsiyet, yaş, eğitim seviyesi, yerleşim yeri ve hizmet türü gibi değişkenlerden etkilendiği saptanmıştır. Cevap verebilirliğin ölçümü, hem genel sağlık sistemi hem de belirli sağlık hizmetlerinin performans değerlendirmesinde kullanılmaktadır. Cevap verebilirlik çalışmaları üzerinde yapılan bu araştırmada elde edilen bulgular, bütüncül ve derinlemesine bir perspektif sunarak, ulusal düzeyde sağlık sisteminin değerlendirilmesinde yapılacak çalışmalara rehber olabilir, çalışmaların gündeme gelmesini sağlayabilir.

Introduction: COVID-19 is a contagious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). The nature of dentistry leads to close contact with patients and exposure to the saliva, blood and other bodily fluids during treatment processes and it is a field where high frequency devices that can make it easier for virus contamination are used. This study aims to determine the knowledge and approaches of COVID-19 infection control of intern dentists who have begun faceto-face education and their COVID-19 related fear and anxiety levels. Methods: The study comprised 4th and 5th year students who began face-to-face education at the Ankara University Faculty of Dentistry 2020/2021 spring semester. A questionnaire was used as the data collection tool of this study. The data were collected using a knowledge questionnaire and a COVID-19 fear and anxiety scale. Results: The average COVID-19 knowledge score of the students was 63.65±9.64, their coronavirus fear average score was 17.63±5.57, and their anxiety average score was found to be 2.37±3.32. A positive relationship was found between the anxiety scores and the COVID-19 fear scores. The results of this study show that the COVID-19 knowledge level and fear of dentistry students is moderate and that their COVID-19 anxiety level is low. Conclusion: It was found that the knowledge and fear of coronavirus levels of intern dentistry students were moderate, and that their coronavirus anxiety level was low.

The purpose of this study was to establish a comprehensive causal model for leadership behaviour, organizational commitment and job satisfaction and to examine it with empirical data in the hospital employees. Structural equation model (SEM) was used to test the validity of constructs of leadership behaviour, organizational commitment and job satisfaction as well as to examine the causal relationship among them. The model was evaluated using data obtained from randomly selected 343 employees from Hacettepe University Adult Hospital in Ankara. The results showed that the measurement model for job satisfaction was a valid reliable model. In testing structural relationships, the results showed that leadership practices directly affect (r = 0.76) job satisfaction and indirectly affect (r = 0.095) it through organizational commitment. The results also showed that organizational commitment directly affects (r = 0.223) job satisfaction.

Artan maliyetler günümüzde tüm sağlık hizmeti sistemlerinin en temel sorunu haline gelmistir.
Öyle ki verimlilik, etkililik ve kalite gibi öncelikli hedeflerin önünde duran maliyet sorunu
hemen hemen her modern yönetim tekniğinin üstesinden gelmeye çalıstığı bir sorun
durumundadır. Bu modern yönetim tekniklerinden biri olan Dıs Kaynaklardan Yararlanma,
Türk Kamu Sektörü’nde de kendisine genis uygulama bulmus ve önemli finansal büyüklüklere
ulasmıstır. Bu denli yaygın ve önemli miktarda fonla ilintili uygulamalara ait yasal altyapının
yöneticilerce iyi bilinmesi kaçınılmaz bir gerekliliktir. Bu çalısmada öncelikle kavramsal
olarak Dıs Kaynaklardan Yararlanma irdelenmis, ardından Türk Kamu Sektörü’nde Dıs
Kaynaklardan Yararlanma uygulamalarının hukuki altyapısına ve bu yapı içerisinde
gerçeklestirilen kamu alım süreçlerine değinilmistir. Son bes yıllık zaman dilimine ait kamu
alım istatistikleriyle çalısma desteklenmistir.

Bu çalısmada örgüt kültürü, bürokratik, yenilikçi ve destekleyici olmak üzere üç boyutta ele
alınmıstır. Buna ek olarak, hastanedeki örgütsel bağlılık seviyesinin tespit edilmesi ve bu
çerçevede de örgüt kültürünün bu üç boyutunun örgütsel bağlılığa olan etkisini incelemek
amaçlanmıstır. Arastırmanın evrenini Ankara ilindeki bir eğitim arastırma hastanesinde görev
yapan 93 sağlık çalısanı (doktor, hemsire, idari personel) olusturmaktadır. Çalısmada
kullanılan veri seti Wallach tarafından gelistirilen Örgüt Kültürü Đndeksi ve Porter’ın Örgütsel
Bağlılık Ölçeği olmak üzere iki bölümden olusmaktadır. Bu çalısmada kullanılan ölçeklerin
Cronbach&Alpha değerleri kabul edilebilir aralıkta bulunmustur. Örgüt kültürünün bağlılığa
olan etkisi destekleyici, bürokratik, yenilikçi olarak üç alt boyutta incelenmistir. Arastırma
sonuçlarına göre örgüt kültürü alt boyutları sağlık çalısanlarının örgütsel bağlılıklarına iliskin
toplam varyansın %34’ünü açıklamaktadır. Regresyon modelinde, regresyon katsayısının
anlamlılığına iliskin t-testi sonuçları incelendiğinde tek anlamlı iliski örgüt kültüründe
görülmüstür. Sağlık çalısanlarının destekleyici örgüt kültürü algılamaları, örgütsel
bağlılıklarını istatistiksel olarak artırmaktadır.
