Infantile chronic recurrent parotitis (ICRP) is an insidious disease whose etiopathogenesis remains an enigma. Alterations in the physical appearance of parotid saliva from ICRP patients have been frequently reported. However, sialochemical studies in regard to ICRP are very rare. The aim of this study was to determine whether saliva of ICRP patients presents major physicochemical and biochemical alterations compared with saliva from paired healthy controls. Parotid, whole, and submandibular/sublingual saliva was collected at an asymptomatic stage from 33 ICRP patients (5-16 y old, both sexes) and from 33 sex-and age-matched healthy controls. Saliva was analyzed for protein concentration, mode of protein diffusion on cellulose membranes, unidimensional sodium dodecylsulfate (SDS)-polyacrylamide gel electrophoresis protein profiles and zymographic profiles of metalloproteinase 2 (MMP-2) and metalloproteinase 9 (MMP-9). Parotid saliva of ICRP patients showed an increased protein concentration, altered mode of protein diffusion, a higher frequency of polypeptide bands of 43, 37,, and 10 kD, higher asymmetry in the polypeptide profiles of both contralateral parotid saliva, and an increase in the frequency of MMP-2 and MMP-9. Parotid saliva of patients with ICRP is molecularly altered with respect to normal saliva. Some of the molecular differences could be related to the etiopathogenesis of the disease.

The study was aimed to investigate whether the occurrence of matrix metalloproteinases (MMP) 2 and 9 in parotid saliva of juvenile recurrent parotitis (JRP) patients is associated with the degree of glandular involvement. Study design. Thirty-three JRP patients were included. Involvement of parotid gland was assessed by sialography. Parotid saliva was assayed for MMP-2 and MMP-9 by zymography. Medical charts were examined for number of recurrences, disease laterality, and time of follow-up. Logistic regression analysis between occurrence of either MMP, the clinical parameters, and sialographic staging was conducted. Results. None of the clinical parameters under analysis were found to be associated with degree of sialographic involvement. Statistical associations were found between presence of MMP-9 and MMP-2 in parotid saliva and sialographic stage (P ϭ .017; odds ratio [OR] 6.5, 95% confidence interval [CI] 1.4-30.4; and P ϭ .009; OR 6.1; 95% CI 1.6-23.7; respectively). Conclusions. Occurrence frequency of MMP-2 and MMP-9 in parotid saliva from affected glands of JRP patients was associated with degree of gland damage. Chronic Recurrent Parotitis or Juvenile Recurrent Parotitis (JRP) is an uncommon clinical condition that causes episodes of recurrent inflammation of the parotid gland, usually unilaterally. It affects children between 3 and 6 years of age and resolves spontaneously at puberty. Fever and general discomfort occasionally accompany it. In the periods between attacks, the patient is asymptomatic. Its prevalence is higher among men and, even its unknown etiology, it is related to autoimmune phenomena. The true extent of this disease lies in the destruction of parenchyma that is caused by recurrent episodes. This reduces the functionality of the gland 50% to 80%. Treatment options go from conservative treatments with anti-inflammatories, analgesic and antibiotics to invasive surgical treatments.

The results of a clinical, sonographic and sialographic study in 10 children (aged 4-10 years) with recurrent sialectatic parotitis are presented. Alteration in the sonographic pattern of the parotid glands, consisting of multiple hypoechogenic areas in the parenchyma, were found in four cases; milder nonhomogeneity was seen in four cases, and an almost normal pattern in two cases. Sonographic follow-up in two children showed a parallel reduction in non-homogeneity with symptomatic improvement. A probable therapeutic action following sialography using fat-soluble contrast medium was observed in four of the 10 children. The familial nature of the disease, which has not previously been described, is documented.

It is a rare condition and characterized by multiple episodes of parotid swelling and/or pain associated with fever or malaise over a period of years. Juvenile Recurrent Parotitis (JRP) is defined as recurrent inflammatory parotitis in children of unknown etiology. Salivary gland diseases in children are rare, apart from viral-induced diseases. Clinical diagnostics and especially the surgical treatment are influenced by stringent indications and a less invasive strategy. Due to the rarity of tumors of the salivary glands in children, it is recommended to treat them in a specialized center with greater surgical experience. In most cases, the symptoms resolve spontaneously after puberty but all children should be screened to exclude Sjogren's syndrome, lymphoma and immunodeficiency including human immunodeficiency virus. Parotid abscess results as a complication of primary parotitis and are very rare in paediatrics population, we present a 6-year-old girl who had 15 episodes of recurrent parotid swelling over past 3 months.

Objective. The aim was to evaluate the utility of dynamic magnetic resonance (MR) sialographic images as a diagnostic tool for patients with xerostomia as chief complaint. Methods: Various parameters of dynamic MR sialographic images from 40 healthy volunteers were compared with those from 28 patients having dry mouth as chief complaint. Dynamic MR sialographic images were acquired using a 1.5-T full-body MR system and 2-dimensional fast asymmetric spin-echo (2D-FASE) sequences. Acquisition of the optimal section using 2D-FASE sequencing with single-section acquisition of thick sections was repeated every 30 s before and after a few drops of 5% citric acid was placed on the tongue. Student t test was used to examine differences between the healthy volunteer and patient groups regarding the maximum area of salivary gland ducts before and after citric acid stimulation. Results. The maximum area of the detectable ducts in the patient group was significantly smaller (P Ͻ 0.001), whereas the change in detectable duct area was significantly lower (P Ͻ 0.001).

The present investigation has characterised the influence of gustatory stimulation and duration of stimulation on the secretion pattern of salivary mucins MG1 and MG2 and non-mucin glycoproteins in submandibular/sublingual secretion (SMSL). Resting SMSL was collected for three 2 min intervals and stimulated SMSL was collected for ten 1 min intervals from six healthy subjects. Flow rates and total protein were significantly different under the two conditions. The secretion patterns of these proteins under resting and stimulated conditions was examined on periodic acid-Schiff reagent (PAS)-stained polyacrylamide gels using a Kodak Digital-Science Image Station. Image analyses revealed that the level of MG1 increased and the level of MG2 remained nearly the same after stimulation. Six other major glycoproteins (designated Band 1-6) were identified on the basis of their electrophoretic mobilities and immuno-reactivity on Western blots. After stimulation the intensity of Band 1 (lactoferrin and peroxidase) and Band 2 (amylase) decreased whereas the intensity of Band 3 (carbonic anhydrase), Band 4 (proline-rich glycoprotein) and Bands 5 and 6 (basic glycosylated proline-rich proteins) increased. These patterns probably reflect secretion from preformed vesicles since de novo synthesis would be unexpected within the time frame of these experiments. The variable patterns observed suggest that mucins and non-mucin glycoproteins in SMSL derive from different subsets of secretory vesicles, some of which may originate in mucous and others in serous acini, as well as in ductal cells. Quantification of mucins was performed by image analysis technology using purified MG1 and MG2 standards. Finally, the present investigation has shown that the secretory patterns of mucins and non-mucin glycoproteins from submandibular/sublingual glands are complex and represent an important aspect of salivary gland physiology. ß
