Background: Femoral neck fractures are a common and severe consequence of road traffic accidents (RTAs), leading to significant morbidity and disability. Understanding their prevalence, demographic distribution, and fracture patterns is essential for optimizing trauma management and prevention strategies. Objective: To determine the prevalence, demographic distribution, and fracture patterns of femoral neck fractures among road traffic accident (RTA) patients. Methods: A descriptive cross-sectional study was conducted at Avicenna Hospital, Lahore, from May to October 2024. A total of 130 RTA patients with radiologically confirmed femoral neck fractures were included using purposive sampling. Data on demographics, mode of injury, and fracture classification were collected. Plain radiography and computed tomography (CT) were utilized for diagnosis. Statistical analysis was performed using SPSS version 25. Results: Out of 130 patients, 91 (70%) were male, and 39 (30%) were female. The highest prevalence was in the 20-40-year age group (67%). Intertrochanteric fractures were the most common (69.2%), followed by partial fractures (66.9%) and femoral stress fractures (65.4%). Motorcyclists and cyclists accounted for the majority of cases (44%). Conclusion: Femoral neck fractures were most prevalent in young male RTA victims, particularly motorcyclists and cyclists with intertrochanteric fractures being the most prevalent type of pattern. Early diagnosis and targeted prevention strategies are essential to reduce associated morbidity.

Methods: The retrospective study was conducted at Aga Khan University Hospital, Karachi, and comprised records of patients having low-velocity pertrochanteric fractures who were fixed with spiral blade Dynamic Helical Hip Systemfrom July to December 2014 and were followed up for a minimum of 3 months. Demographic variables and clinical outcomes were noted from the medical records whereas operative details were recorded from the operative note. Radiological variables and outcomes were assessed by viewing appropriate pre-operative, post-operative and follow-up radiographs. Results: Of the 32 patients in the study, 14(44%) were men and 18(56%) were women, with an overall mean age of 77.81±7.04 years and mean body mass index of 25.99±4.13 kg/m2. Of the total, 1(3.13%) patient had implant cut-out, 1(3.13%) had myocardial infarctionand 2(6.2) expired. Conclusions: The introduction of spiral blade dynamic hip screw manifested favourable results and good clinical and radiological outcomes with low cut-out rates.

A dynamic hip screw (DHS) remains the implant of choice for stabilization of trochanteric fractures because of its favourable results and low rate of non-union or hardware failure, but complication rates of the DHS are higher in unstable and osteoporotic trochanteric fractures. The proponents of the dynamic helical hip system (DHHS) report that it has the potential to decrease the cutout rates in such fractures as helical blade allows compaction in osteoporotic femoral head which in itself improves anchorage. The purpose of the present study was to evaluate the radiological and functional outcome of DHHS in unstable and osteoporotic trochanteric fractures. This was a prospective observational study. The mean age of the 51 patients (24 men and 27 women) was 72.8 years. Fractures were type AO31A2.2 in 28 patients and AO31A2.3 in 23 patients. According to DEXA scans, 41 patients had osteoporosis and 10 patients had osteopenia. Osteoporosis was grade 3 in 36 patients and grade 2 in 15 patients according to Singh's index. The mean follow-up was 1.84 years. The average sliding of the lag screw was 3.6 mm (range 2-10 mm). The mean operative time was 54.74 (range 48-65) min. The average tip-apex distance was 20.24 mm (range 12-28 mm). All but one fractures united. The average time to union was 13.14 (range 11-24) weeks. There were four mechanical complications namely late helical blade migration (n = 1), late medialization of shaft (n = 2) and varus collapse with cut through (n = 1). No patient was noted to have a plate pull-out. The average Harris hip score was 92.87 (range 76-97). The use of a DHHS for stabilization of unstable(AO31A2), osteoporotic trochanteric fractures in the elderly patients was associated with reliable rates of union and functional outcome and a decreased incidence of screw cutout and side plate pull-out.

Background The use of cephalo-medullary nails (CMN) is a widely accepted management option for the treatment of unstable per-trochanteric hip fractures. A growing body of literature has reported good functional and radiological outcomes in patients managed with a dynamic hip screw supplemented with a trochanteric stabilisation plate (DHS w/ TSP). However, a robust meta-analysis does not exist in the current literature comparing the two fixation methods. Background: Proximal Femoral fractures are common in elderly. Their incidence had increased due to the increased life expectancy and osteoporosis. Objective: The aim of this study was to evaluate the use of locked dynamic hip screw plate (LDHS) as a treatment of intertrochanteric femoral fracture in elderly patients. Patients and Methods: This study was a non-randomized clinical trial including eighteen patients with intertrochanteric femoral fracture fixed with locked dynamic hip screw plate. They came to Orthopedic Department in Zagazig University Hospital and Orthopedic Department in Tripoli Central Hospital over 3 months from July 2020 until September 2020. Patients with sub-trochanteric extension were excluded. There was 10 cases group A1 and eight cases group A2 according to AO Orthopedic Trauma Association (OTA). Results: Clinical and radiological outcome assessments showed that 77.8% were excellent clinically, 11.1% were good and 11.1% were fair. Regard radiological assessment, 83.3% were excellent, 11.1% were good and 5.6% were fair. As regards complications, 4 cases had groin pain and 1 case had lag screw cutout and overall complications were in 22.2%. Conclusion: Locking DHS plate for trochanteric femur fractures is a good option in elderly patients.

OBJECTIVE To evaluate safety and feasibility of two-stage total hip arthroplasty and to compare it with single-stage procedure. METHODS The retrospective study was conducted at The Aga Khan University Hospital and comprised all cases of total hip replacements between January 2001 and December 2014 that were retrieved from the database using International Classification of Diseases (9th Revision) coding. A standardised questionnaire was completed, including patient demographics, primary diagnosis, peri and postoperative morbidity and mortality. Differences among patients\' data were analysed using chi square test for dichotomous variables and student t-test for continuous variables. RESULTS Of the 48 cases, 34(71%) had single-stage bilateral total hip replacement and 14(29%) had two-stage procedure.
