MICR:Mobile-Assisted Immunization of Child in Remote Healthcare

Immunization saves millions of lives against vaccine-preventable diseases. Yet, 24 million children born every year do not receive proper immunization during their first year. UNICEF and WHO have emphasized the need to strengthen the immunization surveillance and monitoring in developing countries to reduce childhood deaths. In this regard, we present a software application called Jeev to track the vaccination coverage of children in rural communities. Jeev synergistically combines the power of smartphones and the ubiquity of cellular infrastructure, QR codes, and national identification cards. We present the design of Jeev and highlight its unique features along with a preliminary evaluation of its performance. We plan to pilot test Jeev in a rural population to study its effectiveness and identify socio-cultural issues that may arise in a large-scale deployment.

In Bangladesh, full vaccination rates among children living in rural hard-to-reach areas and urban streets are low. We conducted a quasi-experimental pre-post study of a 12-month mobile phone intervention to improve vaccination among 0-11 months old children in rural hard-to-reach and urban street dweller areas. Software named "mTika" was employed within the existing public health system to electronically register each child's birth and remind mothers about upcoming vaccination dates with text messages. Android smart phones with mTika were provided to all health assistants/vaccinators and supervisors in intervention areas, while mothers used plain cell phones already owned by themselves or their families. As part of a randomised controlled trial in rural Madhya Pradesh, India to assess the impact of Kilkari, a maternal messaging programme, we explored determinants of parental immunisation knowledge and immunisation practice (completeness and timeliness) for children 0–12 months of age from four districts in Madhya Pradesh.MethodsData were drawn from a cross-sectional survey of women (n=4423) with access to a mobile phone and their spouses (n=3781). Improved routine immunization (RI) coverage is recommended as the priority public health strategy to decrease vaccine-preventable diseases and eradicate polio in Pakistan and worldwide. The objective of this study was to ascertain whether customized, automated, one-way text messaging (short message service, SMS) reminders delivered to caregivers via mobile phones when a child is due for an RI visit can improve vaccination uptake and timelines in Pakistan. This was a randomized controlled trial, conducted in an urban squatter settlement area of Karachi, Pakistan. Infants less than 2 weeks of age with at least one family member who had a valid mobile phone connection and was comfortable receiving and reading SMS text messages were included. The conventional manual method of recording vaccination information is less organized and difficult to be retrieved back. IVS is an integrated information technology system that tries to address the weaknesses in the conventional method by providing storage for child's vaccination data and the use of MyKidVAX grant parents as a reminder for the next vaccination date. A software editor called Dreamweaver is used in creating the IVS form. IVS showed more benefit with the use of this latest mobile application and positively progressed. During the Covid-19 pandemic, vaccination was the prime requirement. As it is the best strategy to fight against it. Since then various methodologies have been implemented, one of them is Mobile Vaccination. Recipients can book a slot based on wards on the web app. They are also able to track their vaccination status. Two days prior to the vaccination drive, these registrations are fed into an algorithm and appropriate locations and number of vans are selected dynamically. The spot distribution is stored as vaccination drives which are then reflected to the public on the day of the drive. On the day of the vaccination drive, van will visit the mentioned spots, vaccinate the recipients and update vaccination status as well. This is how Mobile Vaccination WebApp works.

Immunization during pregnancy and infancy significantly reduces morbidity and mortality of mothers, unborn fetuses, and young infants. Several studies show the merits of getting complete, quality, and accurate data on time to enhance policy and decision-making for society or country development. Despite the efforts by nations to ensure the success of maternal immunization through electronic immunization registries, limited resources such as poor internet access, shortage of electricity, and digital illiteracy in developing countries hinder the goal of full immunization of mothers and infants. Since 2015, immunization programs in Tanzania use internet-based information systems to collect immunization data from health facilities and submit them to the responsible authority for further decision-making such as the allocation of vaccines to health facilities. The internet-based media is not fully achieved in developing countries due to its cost and resource setting, thus, the responsible authority does not receive instant data to update its vaccine inventory and management activities which often results in partial immunization due to the unavailability of vaccines in some facilities. This challenge can be solved by having an affordable system that instantly incorporates and transmits vaccination details such as the utilization of vaccines and demands from each health facility to responsible authority with less resources. The present study proposes a USSD platform to enhance the receipt of real-time data by immunization authorities from both health facilities with poor and good internet connectivity at a lesser cost. A greater number of health facilities in Tanzania prefer to use both online and offline platforms for collecting and recording immunization data. As electronic immunization registry has been introduced in areas with limited resources, it is recommended the use online and offline platforms for data collection so that they can submit immunization data in real-time without the delays caused by poor resource setting.
