Early detection of core deficits in autism

Abstract

The goal of this review of the research literature is to discuss approaches to the early detection of autism in infancy. Early detection would enable diagnoses to be made before 18 months of age rather than at 24 -30 months, the age where diagnoses start to be made now. After summarizing the criteria for a deficit to be considered "core" to the disorder, the literature on research strategies used in early detection is examined. In order to guide the design of future studies, the review then turns to an overview of what is known about the processes of early social development in typically developing children that underlie the domains in which core deficits are manifested in young children with autism. The social domains covered in the review are those that show development in typically developing infants below 18 months of age: dyadic interaction and imitation; emotion discrimination; and attachment. The review concludes that all of these areas are worthy of investigation in young children, particularly those at higher risk of showing some of the core deficits of autism such as the infant siblings of children with autism.

Earlier identification and diagnosis of autism spectrum disorders (ASDs) can improve opportunities for children to benefit from intervention and lessen the burden on concerned parents. This review summarizes current knowledge about early signs of autism. Convergent data from both retrospective studies and prospective studies of high-risk infants indicate that ASD symptoms emerge in the first two years of life, affecting multiple developmental domains, mapping onto symptom dimensions consistent with current diagnostic frameworks including social-communication, and repetitive interests/behaviors but also extending to motor delays and atypical regulation of attention and emotion. Recent findings have shed new light on patterns of symptom onset and progression, and promise to inform early detection and diagnosis. Further attention to effective application of new findings and related challenges in building health system capacity to ensure timely access to specialized assessment and interventions is needed to fully realize the promise of improved outcomes resulting from this research.

Case Study of the Development of an Infant with Autism from Birth to Two Years of Age

This report describes a case study of the development of an infant with autism who was observed closely by professionals from birth and to whom a comprehensive psychological evaluation was administered at approximately 1 and 2 years of age. During the first 6 months of life, this infant displayed difficulties in oral motor coordination and muscle tone that fluctuated between hypotonia and hypertonia. He startled easily, had poor state regulation, and was hypersensitive to touch. Notably, however, during the first 6 months, this infant vocalized and responded socially to others by smiling and cooing. During the second half of the first year, he continued to demonstrate diffuse sensorimotor difficulties and diminished oral motor control. Hypersensitivity now extended to a wider range of stimuli. He had problems in sleep regulation. Motor stereotypies, including rocking, head banging, and toe walking, were observed. Difficulties in the domain of social interaction began to emerge during the second 6 months, including poor eye contact, failure to engage in imitative games, and lack of imitative vocal responses. By a little over 1 year of age, this infant met diagnostic criteria for autism based on the Autism Diagnostic Interview. There were several domains in which this toddler with autism did not show impairments. In the areas of immediate memory for actions, working memory, response inhibition, and speech perception, this 1-year old with autism displayed no evidence of significant impairment on the tests administered. This case study offers clues regarding the nature of autism at its earliest stages. Understanding early development in autism will be important for developing early screening and diagnostic tools.

Autism Spectrum Disorders in Infancy and Toddlerhood: A Review of the Evidence on Early Signs, Early Identification Tools, and Early Diagnosis

To date, the biological basis of autism spectrum disorders (ASDs) remains unknown. Thus, identification and diagnosis are reliant on behavioral presentation and developmental history. There have been significant advances in our knowledge of the early signs of ASD through the use of retrospective videotape analysis, parental report, screening studies, and more recently, studies on high-risk infant siblings. Despite behavioral markers being identified within the first year of life, the current average age of diagnosis for ASD remains at approximately 3 years or older. Consequently, these children are not receiving intervention in their early years, which is increasingly recognized as an important time to begin intervention. There remains little research on the prospective identification of these children in a community-based sample before 18 months. It is recommended that future prospective studies monitor behavior repeatedly over time, thereby increasing the opportunity to identify early manifestations of ASD and facilitating the charting of subtle behavioral changes that occur in the development of infants and toddlers with ASD.

Early Identification of Autism: How Early Can We Go?

Identification of young children at risk for Autism Spectrum Disorders (ASD) depends on early behavioral symptomatology and yet conventional criteria provide little guidance for use with infants and toddlers. Recent research, however, has demonstrated that there are patterns of behavior below 2 years of age that distinguish children with autism from those who are developing typically or those with other developmental disabilities. Skill areas with particular promise for early identification include social communication, sensory regulation, and play. This article previews current innovative methodologies, presents a synthesis of recent research findings related to these three key areas, and provides clinicians with practical guidelines for early identification of infants and toddlers at risk for ASD and other disorders.

The broader autism phenotype in infancy: when does it emerge?

Early Identification of Autism Spectrum Disorder: Recommendations for Practice and Research

Early identification of autism spectrum disorder (ASD) is essential to ensure that children can access specialized evidence-based interventions that can help to optimize long-term outcomes. Early identification also helps shorten the stressful "diagnostic odyssey" that many families experience before diagnosis. There have been important advances in research into the early development of ASDs, incorporating prospective designs and new technologies aimed at more precisely delineating the early emergence of ASD. Thus, an updated review of the state of the science of early identification of ASD was needed to inform best practice. These issues were the focus of a multidisciplinary panel of clinical practitioners and researchers who completed a literature review and reached consensus on current evidence addressing the question "What are the earliest signs and symptoms of ASD in children aged #24 months that can be used for early identification?" Summary statements address current knowledge on early signs of ASD, potential contributions and limitations of prospective research with high-risk infants, and priorities for promoting the incorporation of this knowledge into clinical practice and future research.

The development of siblings of children with autism at 4 and 14 months: social engagement, communication, and cognition

Aims: To compare siblings of children with autism (SIBS-A) and siblings of children with typical development (SIBS-TD) at 4 and 14 months of age. Methods: At 4 months, mother-infant interactional synchrony during free play, infant gaze and affect during the still-face paradigm, and infant responsiveness to a name-calling paradigm were examined (n ¼ 21 in each group). At 14 months, verbal and nonverbal communication skills were examined as well as cognition (30 SIBS-A and 31 SIBS-TD). Results: Most SIBS-A were functioning as well as the SIBS-TD at 4 and 14 months of age. However, some differences in early social engagement and later communicative and cognitive skills emerged. Synchrony was weaker in the SIBS-A dyads, but only for infant-led interactions. Infant SIBS-A revealed more neutral affect during the still-face procedure and were less upset by it than was true for the SIBS-TD. A surprising result was that significantly more SIBS-A responded to their name being called by their mothers compared to SIBS-TD. At 14 months, SIBS-A made fewer nonverbal requesting gestures and achieved lower language scores on the Bayley Scale. Six SIBS-A revealed a language delay of 5 months and were responsible for some of the significant differences between SIBS-A and SIBS-TD. Furthermore, infant SIBS-A who showed more neutral affect to the still face and were less able to respond to their name being called by their mothers initiated fewer nonverbal joint attention and requesting behaviors at 14 months, respectively. Discussion: Focused on the genetic liability for the broad phenotype of autism as well as the possible influence of having a sibling with autism.

Early social and emotional communication in the infant siblings of children with autism spectrum disorders: An examination of the broad phenotype

Infants with older siblings with Autism Spectrum Disorders (ASD-sibs) are at risk for socioemotional difficulties. ASD-sibs were compared to infants with typically developing older siblings (TDsibs) using the face-to-face/still-face (FFSF) at 6 months and the Early Social Communication Scale (ESCS) at 8, 10, 12, 15, and/or 18 months. ASD-sibs smiled for a lower proportion of the FFSF than TDsibs and lacked emotional continuity between episodes. With respect to TD-sibs, ASD-sibs engaged in lower rates of initiating joint attention at 15 months, lower rates of higher-level behavioral requests at 12 months, and responded to fewer joint attention bids at 18 months. The results suggest subtle, inconsistent, but multi-faceted deficits in emotional expression and referential communication in infants at-risk for ASDs.

In the first year of life, infants who later go on to develop autistic spectrum disorders (ASD) may exhibit subtle disruptions in social interest and attention, communication, temperament, and head circumference growth that occur prior to the onset of clinical symptoms. These disruptions may reflect the early course of ASD development and may also contribute to the later development of clinical symptoms through alterations in the child's experience of his or her environment. By age 2, developmental precursors of autism symptoms can be used to diagnose children reliably, and by age 3, the diagnosis is thought to be relatively stable. The downward extension of the autism diagnosis poses important questions for therapists in designing interventions that are applicable for infants who demonstrate early risk factors. We review current knowledge of the early signs of ASD in the infancy period (0-12 months) and the manifestation of symptoms in toddlerhood (12-36 months), noting the importance of considering the variability in onset and trajectory of ASD. Finally, we consider the implications of this emerging research for those who work or interact with young children, including the importance of early monitoring and the development and evaluation of age-appropriate interventions.

Are there early features of autism in infants and preschool children?

A DSM-IV 1 or ICD-10 2 diagnosis of autism requires impairment in three areas of development: (i) social interaction; (ii) communication; and (iii) restricted, repetitive and stereotyped patterns of behaviour, interests and activities, and onset must be before the age of 3 years. Taking into account variations in definition and methodology, the best available prevalence estimate for autism in children is 5 per 10 000. 3 The male to female ratio is within the range of 3-4 males to 1 female. 4 The rate of intellectual disability in autism is 75-80%, 4,5 with the majority in the moderate to severe range. 1,6 Although the precise aetiology remains unknown, autism is recognized as a neurobiological condition involving central nervous system dysfunction 7,8 most likely with a genetic basis involving multiple, interacting genes. 9-11 AGE OF RECOGNITION AND DIAGNOSIS The onset of autism is within the first 30 months of life; 12-14 however, the majority of parents suspect problems with their child's development before 24 months of age (Table 1). Despite this, the age at which children are first diagnosed with autism is usually at least 2 years later (Table 1). The optimal age for the commencement of early intervention is less than 4 years of age, 15,16 therefore the delay in diagnosis is preventing some children from receiving the benefits of early intervention. Language delay and speech problems are the symptoms that initially cause parents the most concern. 17-22 Abnormalities in social development such as being indifferent to or upset by social approaches, rituals, stereotypies, sleeping problems, feeding problems, motor delay, emotional problems, medical problems and a lack of imaginative play are also reported as early concerns. 18,22 A number of these concerns may not be specific to autism but relate to general developmental delay. 17,23 Deficits in social relating and responsiveness are usually not detected by developmental screening until 18 months of age. 23 Potential reasons for diagnostic delay The diagnostic criteria for autism relating to social and communicative development require time to emerge and may therefore be difficult to assess in preschool children. 24,25 For example, a follow-up study found that a number of behaviours which differentiated children with autism from children with specific speech and language delays without autism, were more prevalent in children with autism at 3 years of age than at 2 years of age. 26 These included abnormalities in understanding gesture, sharing enjoyment, greeting, social reciprocity and directing attention, all of which showed higher rates in those children with autism. Similarly, in young children autism symptoms may change, develop or present only intermittently. 27 For example, a number of behaviours have been shown to decrease in prevalence in children with autism from age 2 to age 3, including abnormalities in the use of another's body as a tool, interest in children and unusual sensory behaviours. 26 The differentiation of children with autism with a mental age of less than 18 months from non-verbal children with

An experimental investigation of social-cognitive abilities in infants with autism: Clinical implications

Competing theoretical accounts of psychopathological development in individuals with autism emphasize the role of different infant social, cognitive and affective factors, including affective responsivity, pretend play, joint attention, and imitation. However, due to the fact that autism is rarely diagnosed before the age of 3, until now these abilities have only been studied with school-age children, adolescents, or young adults with autism. Taking advantage of a new prospective screening instrument for autism in infancy (Baron-Cohen et al., 1996), the present study compared the performance of 20-month-old infants with autism and pervasive developmental disorder to that of children with developmental delay without autism on experimental tasks of empathic response, pretend and functional play, joint attention and requesting behaviors, and imitation. The 20-month-old infants with autism failed to use social gaze declaratively in the joint attention task, they showed poor emphatic response, fewer imitated modelled actions on objects, and none produced spontaneous pretend play. Surprisingly, the infants with pervasive developmental disorder did not perform significantly differently from the infants with developmental delay without autism on any of the measures. The identification of autism-specific impairments in early social cognitive abilities may have important clinical implications, for the early diagnosis of the disorder and for the setting of goals and monitoring of progress in early intervention programs.

    FMRI was performed with the dynamic facial expressions fear and happiness. This was done to detect differences in valence processing between 25 subjects with autism spectrum disorders (ASDs) and 27 typically developing controls. Valence scaling was abnormal in ASDs. Positive valence induces lower deactivation and abnormally strong activity in ASD in multiple regions. Negative valence increased deactivation in visual areas in subjects with ASDs. The most marked differences between valences focus on fronto-insular and temporal regions. This supports the idea that subjects with ASDs may have difficulty in passive processing of the salience and mirroring of expressions. When the valence scaling of brain activity fails, in contrast to controls, these areas activate and/or deactivate inappropriately during facial stimuli presented dynamically.

    Intrinsic and extrinsic motivators of attachment under active inference

    This paper addresses the formation of infant attachment types within the context of active inference: a holistic account of action, perception and learning in the brain. We show how the organised forms of attachment (secure, avoidant and ambivalent) might arise in (Bayesian) infants. Specifically, we show that these distinct forms of attachment emerge from a minimisation of free energy-over interoceptive states relating to internal stress levels-when seeking proximity to caregivers who have a varying impact on these interoceptive states.     Joint attention, or the shared focus of attention between objects or events and a social partner, is a crucial milestone in the development of social communication and a notable area of deficit in children with autism spectrum disorder. While valid parent-report screening measures of social communication are available, the majority of these measures are designed to assess a wide range of behaviors. Targeted assessment of joint attention and related skills is primarily limited to semi-structured, examiner-led interactions, which are time-consuming and laborious to score.
