Among all the congenital heart disease, bicuspid aortic valves (BAVs) are cardiac valvular anomal... more Among all the congenital heart disease, bicuspid aortic valves (BAVs) are cardiac valvular anomaly, occurring in 12% of the general population. It is twice as common in males as in females. In many cases, a bicuspid aortic valve will cause no problems. However, BAV may become calcified later in life, which may lead to varying degrees of severity of aortic stenosis that will manifest as murmurs. If the leaflets do not close correctly, aortic regurgitation can occur. These mixed clinical finding sometimes confuses physician and culminating to considering other differentials, needs further more subsequent investigation and imaging, thus prolonging evaluation time and delaying decision making for treatment plan. It occurs mainly in patients with established erosive nodular RA. It is usually sudden and permanent. It has several mechanisms, but the most common cause is infiltration in or near the AV node or bundle of His. If CHB develop the best option of treatment is the insertion of a permanent pacemaker. Chronic nature of rheumatic mitral regurgitation causes gradual dilatation of the left atrium with little increase in pressure and therefore relatively fewer symptoms. However, occasionally the dilatation of LA is so severe, it is termed giant LA. Giant left atrium is a rare condition, with a reported incidence of 0.3%, and occurs following mainly rheumatic mitral valve disease 2 . Atrial fibrillation is almost always present 3 predisposing to thromboembolic complications. A subset of patients may develop severe dysphagia from esophageal compression or hoarseness from laryngeal nerve impingement 4 . Here we are presenting a case of isolated mitral regurgitation with giant left atrium in sinus rhythm. A 30-year-old woman normotensive, non-diabetic presented with the complain of SOB and palpitation for 2 ½ months and leg swelling for 10 days. 2 ½ months back, after 10 days of delivery of her 3 rd child, she developed SOB which was initially present on exertion (NYHA-II) and relieved by rest, but since last 10 days, it was present on minimal exertion or rest (NYHA III-IV) with orthopnea but no PND. She also had dry cough but no hemoptysis or diurnal and seasonal variation. She also had palpitation which occurred with SOB during exertion and was relieved by rest. It was regular and not associated with dizziness or syncope. Since last 10 days, she also developed swelling of legs which was gradually increasing, associated with decreased urine output but no puffiness of face or abdominal swelling. The present study was conducted to evaluate the adherence to standard treatment regimen in Bangab... more The present study was conducted to evaluate the adherence to standard treatment regimen in Bangabandhu Sheikh Mujib Medical University (BSMMU) for the treatment of corona virus disease 19 (COVID-19) patients. The study assessed the awareness of prescribers, adherence to treatment guidelines and reasons for non-adherence. A questionnaire survey was conducted followed by a retrospective and prospective medical review and in-depth interview. The study revealed that 79.9% of key prescribers are aware of treatment guidelines. Adherence was 28.4% in moderate cases and 53.3% in severe cases and difference was highly statistically significant (p<0.001). Adherence in moderate cases was highest in patients with hypertension (43.3%) and lowest in bronchial asthma (16.6%). Adherence in severe cases was highest with diabetes (80%) and lowest in bronchial asthma (20 %). It may be either acquired or congenital. e right coronary sinus is most frequently a ected, with the most common complication being rupture of the sinus. Cardiac failure and sudden auscultation changes suggest the possibility of SVA rupture. Echocardiography is su cient to diagnose SVA, its complications, repercussions and to determine surgical options. SVA, even if asymptomatic, has potential risks of expansion, rupture, cardiac failure, endocarditis, embolism and sudden death. is justi es surgical correction which has a low mortality rate in both the short and long-term.
2013 to June 2013.Adult patients of spontaneous subarachnoid haemorrhage (SAH), diagnosed clinica... more 2013 to June 2013.Adult patients of spontaneous subarachnoid haemorrhage (SAH), diagnosed clinically and con rmed by CT scan of the head were included in the study. However, patients who are not capable nancially of undergoing Digital Subtraction Angiography (DSA), traumatic subarachnoid haemorrhage, intracerebral haemorrhage and patients taking antiplatelet and anticoagulant drugs and with comorbidities were excluded. A total of 30 subjects meeting the above eligibility criteria were selected consecutively from the study population. e present study demonstrated that 80% of the patients were 50 or younger than 50 years old (mean age 45.0 ± 9.4 years) with a male preponderance (60%). Sudden headache accompanied by vomiting was invariably complained by the patients at onset of Athe disease. On admission two-thirds (66.7%) of the patients were unconscious. 4 out of 30(16.65%) patients exhibited neurological de cit. Of the risk factors, hypertension and smoking demonstrated their signi cant presence (around 45%) among the patients studied. Based on Glasgow Coma Scale, 7(23.3%) patients out of 30 in the present study were in grade-v. Our data showed that the common site of aneurysm was anterior communication artery (36.7%) followed by middle cerebral artery (26.7%) and posterior communicating artery (23.3%). Saccular aneurysms formed the main bulk (93%) of the cases irrespective of anatomical distribution of aneurysm. In aneurysmal subarachnoid haemorrhage, aneurysms are mainly located in anterior communicating and middle cerebral arteries and of medium-sized. Majority are saccular type and narrow-necked.
