Topics covered include diagnostic validity, prevalence, developmental issues, comorbidity, risk and protective factors, and treatment. Novel indicated prevention and early intervention programs for borderline personality disorder in adolescence are given special priority. To conclude, directions for future research are provided.

Borderline personality disorder (BPD) has been shown to be a valid and reliable diagnosis in adol... more Borderline personality disorder (BPD) has been shown to be a valid and reliable diagnosis in adolescents and associated with a decrease in both general and social functioning. With evidence linking BPD in adolescents to poor prognosis, it is important to develop a better understanding of factors and mechanisms contributing to the development of BPD. This could potentially enhance our knowledge and facilitate the design of novel treatment programs and interventions for this group. In this paper, we outline a theoretical model of BPD in adolescents linking the original mentalization-based theory of BPD, with recent extensions of the theory that focuses on hypermentalizing and epistemic trust. We then provide clinical case vignettes to illustrate this extended theoretical model of BPD. Furthermore, we suggest a treatment approach to BPD in adolescents that focuses on the reduction of hypermentalizing and epistemic mistrust. Formålet med denne præsentation er at undersøge sammenhængen og relevansen af personlighed og per... more Formålet med denne præsentation er at undersøge sammenhængen og relevansen af personlighed og personlighedsforstyrrelser vis-à-vis såkaldte neuropsykiatriske lidelser. Med udgangspunkt i et rids af psykometrisk forskning, bamser, babyer og obskure kulturelle artefakter, evalueres diagnostisk terminologi og forskning i forhold til personlighed(-sforstyrrelser) og neuropsykiatriske lidelser. Der argumenteres for at personlighed og psykopatologi er placeret på eet og samme (vulnerabilitets-)spektrum. Kliniske implikationer heraf vil blive diskuteret, med fokus på den centrale rolle børne- ungdomspsykiatrien spiller med hensyn til forebyggelse og tidlig indsats, samt betydningen af relationer og den dialogiske basis for faget.

Background: Callous-unemotional (CU) traits have been found to index an important subgroup of ant... more Background: Callous-unemotional (CU) traits have been found to index an important subgroup of antisocial youth who are at high risk for developing psychopathic personality pathology, and for becoming severe and persistent offenders. On the basis of such research findings, the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, have included a “with limited prosocial emotions” specifier in the diagnostic criteria for conduct disorder to designate a subtype with high levels of CU traits. This creates the need for psychometrically sound measures for the assessment of these traits. The self-report questionnaire Inventory of Callous-Unemotional Traits (ICU) was designed to provide an efficient, reliable, and valid measure of CU traits among youth populations. Convincing evidence demonstrates that psychopathy is associated with premeditated aggression. How... more Convincing evidence demonstrates that psychopathy is associated with premeditated aggression. However, studies have failed to explain why this association exists and whether socio-cognitive functions, such as mentalizing, could explain the relation. This cross-sectional study investigates, in 108 patients with schizophrenia, the association of psychopathy and mentalizing abilities with premeditated and impulsive aggression and probes the nature of their influence on these specific aggression patterns. Patients' engagement in premeditated aggression was associated with diminishing mentalizing and increasing psychopathic tendencies. Moreover, mediation analyses reveal that the ability to attribute mental states to others mediates the relation between psychopathy and type of aggression. This mediation is facilitated by a specific mentalizing profile characterized by the presence of intact cognitive and deficient emotional mentalizing capacities. This study is the first to report a mediating effect of mentalizing on the relationship between psychopathy and type of aggression in schizophrenia. Implications of these results are discussed.

Background: Most interventions for depression have shown small or no effects. 'Third wave' cognit... more Background: Most interventions for depression have shown small or no effects. 'Third wave' cognitive therapy and mentalization-based therapy have both gained some ground as treatments of psychological problems. No randomised trial has compared the effects of these two interventions for patients with major depression. Methods/ design: We plan a randomised, parallel group, assessor-blinded superiority clinical trial. During two years we will include 84 consecutive adult participants diagnosed with major depressive disorder. The participants will be randomised to either 'third wave' cognitive therapy versus mentalization-based therapy. The primary outcome will be the Hamilton Rating Scale for Depression at cessation of treatment at 18 weeks. Secondary outcomes will be the proportion of patients with remission, Symptom Checklist 90 Revised, Beck's Depression Inventory, and The World Health Organisation-Five Well-being Index 1999.

In this commentary on the 6 articles comprising this In Session issue on metacognition and mental... more In this commentary on the 6 articles comprising this In Session issue on metacognition and mentalizing in the psychotherapeutic treatment of severe mental disorders, we strive to contextualize and bring together salient issues reflected in these articles. In the foreground of our discussion is the point that the commonalities of these and related social cognitive treatments far outweigh their differences. We attempt to pinpoint some of the more specific tailored treatment elements described by the authors and relate these to empirical findings and theoretical and practical problems. Among the key issues addressed in this commentary are conceptual fallacies, therapist transparency, personality disorder and self-harm in adolescence, therapeutic alliance, and a metacognitive-informed group psychotherapy practice for patients with avoidant personality disorder or alexithymia.
