The Generation suffering related items: a process integrating consensus experts and Sorensen’s similarity analysis

Health psychology and public health aspire to find ways for evaluating the suffering of patients. Quantitative assessment of pain-related suffering exists, but methodological frameworks implying qualitative approach based on a direct communication with participants are missing. Our aim was to analyse the procedure generating pain and suffering-related items and explore the validity of their content.
Volunteers were all right-handed and of European origin. After received information concerning the aims of the study, they signed a consent to accept the experimental pain stimulation protocols and to receive financial compensation. Descriptors of pain-related suffering were gathered through 106 semi-structured interviews of 31 particpants (16 men ;15 women), age 18-33 years. A thematic categorical content analysis on the transcriptions was conducted. Verbatim were extracted and classified under categories. Cluster analysis on word of verbatim using the index of Sorensen was performed with N’Vivo12.
The content of each category was validated through consensus by multidisciplinary experts (some have lived the experimental pain stimulation). Then they formulated 70 items to capture the overall pain-related suffering experience. In accordance to theories and with the help of the Sorensen’s similarity analysis, a conceptual framework with different dimensions (physical, mental, etc.) has been discussed and elaborated.
The list obtained constitutes a first classification for laboratory use and a basis for the development of a routine clinical suffering assessment tool. This generation process should guarantee that the items are acceptable, comprehensive and relevant (reflecting the lived experience of the participants) ensuring a content validity of the future questionnaire.

Objectif : Construire et valider une échelle des attitudes professionnelles des médecins générali... more Objectif : Construire et valider une échelle des attitudes professionnelles des médecins généralistes (APMG) et une échelle des attitudes professionnelles des pharmaciens d'officine (APPO). Méthodes : Étape 1. Construction et validation qualitative d'une échelle. Nous avons repéré, dans le discours de 40 usagers lorrains d'un centre de médecine préventive, de 21 médecins généralistes (MG), et de 22 pharmaciens d'officine (PO), les items candidats définissant le construit théorique des attitudes professionnelles intervenant dans la mise en place et le suivi des traitements des patients. Ensuite nous avons élaboré, avec un groupe d'experts, les items génériques du questionnaire permettant leur validation et identifié les fonctions que remplissent ces attitudes. Étape 2. Validation statistique de l'échelle des APMG et de l'échelle des APPO. Nous avons étudié, à l'aide d'un questionnaire auto-administré auprès de 393 usagers lorrains, les propriétés métriques de ces échelles. Résultats : Les attitudes relationnelles sont au nombre de 15 pour les MG et de neuf pour les PO. Elles sont associées aux fonctions d'information, de communication, d'éducation. Les échelles des APMG et des APPO forment deux échelles unidimensionnelles. Les analyses montrent une cohérence interne élevée (coefficient alpha de Cronbach respectivement 0,91 et 0,87) ainsi qu'une reproductibilité correcte (coefficient de corrélation intra-classe respectivement 0,74 et 0,72). Conclusion : Les compétences mises à jour représentent les critères de qualité de la relation interpersonnelle médecinusager et pharmacien-usager. Les propriétés métriques des échelles des APMG (15 items) et des APPO (neuf items) sont satisfaisantes. Ceci légitime leur utilisation comme outil d'aide : à la formation à la relation professionnel-usager ; à la décision ; à l'évaluation des compétences relationnelles.

Background Till present there are practically no information on the prevalence and risk factors o... more Background Till present there are practically no information on the prevalence and risk factors of osteoarthritis in populations residing in severe climatic zones of Russia and subject to the influence of a number of unfavourable environmental factors. Objectives To study the epidemiology of osteoarthritis among native population of rural areas of Siberian Northin Yakutia. Methods Cross-sectional study of residents of four Yakutian settlements (North/North-Eastern part of Siberia) was performed. At the moment of the study 1282 persons aged 18 and more resided lived there. Among them 1216 persons (94.8%) were examined. 1002 persons were Yakutians (Saha) and 214 -Evenks. Results Osteoarthritis was diagnosed in 218 settlement residents. Correspondingly the prevalence of OA in the population under study was 18.00.8%. Among women the OA prevalence was 22.51.4% and among men 12.61.4% (p < 0.05). There was no reliable difference in prevalence between Yakuts and Evenks (correspondingly 17.3% and 21.0%, p < 0.05). We found linear correlation (r = 0.96) between frequency of OA and age. Conclusion Thus, epidemiological study of osteoarthritis among rural population of Russian North demonstrated that the prevalence of the disease is 18.0% and women prevail and there is direct age relation between age and frequency of the disease.

Introduction: Five years after coronary angiography, life satisfaction (LS) among patients may be... more Introduction: Five years after coronary angiography, life satisfaction (LS) among patients may be related to incidents of cardiovascular diseases, risk factors and unhealthy behaviours and socioeconomic conditions but their respective influence remains unclear. Our aim is to analyze LS and its relationships with those factors. Methods: Among the 4,391 patients initially contacted, 547 deaths were reported and 209 had an invalid address. 3,635 patients who underwent coronary 23/11/2015 ... abstainers' who had no intention of changing and were insufficiently concerned to do so. Patients who stopped smoking and perceived it as unpleasant also had the lowest LS. 'Declined actors' were those patients who had to adjust their lifestyles, but were ambivalent about their intentions and the behaviour, which they continued. Health promotion programs would benefit from targeting factors that moderate the unfavourable intention-behaviour relationship and can help enhance LS.

This study assessed its association with gender, age, education, living alone, socioeconomic position, income, poor health, obesity, smoking, alcohol abuse, fatigue/sleep disorders, and physical, sensorial and cognitive disabilities. This cross-sectional study was conducted in 6,198 people, aged 15 years or over, randomly selected in north-eastern France. They completed a post-mailed questionnaire that covered: • Socio-economic characteristics: gender, age, education, living alone, socioeconomic position, income, • lifestyle: obesity, smoking, alcohol abuse, • health status, fatigue and sleep disorders during the previous 8 days • physical disability for daily living activities (a 20-item scale), sensorial (seeing and hearing) disability, and cognitive disabilities (problems with concentration, attention, orientation, problem-solving, and memory). Any traffic accidents during the previous 2 years Data were analyzed using adjusted odds ratios computed with logistic regression models. Traffic accident is associated with younger age, living alone, poor heath and physical disability. Preventive measures to reduce traffic accidents should aim at improving health and lifestyle and increase awareness of risk, particularly among people who smoke, are on a low income, or suffer from fatigue, sleep disorders, or physical or cognitive disabilities. Our main goal was the study of causes of the psychological and pharmacological of anorgasmia in affected women in Ardabil Methods: The present study is a descriptive-cross sectional one. a total of 500 married women were selected and they completed the demographic and female sexual function index (FSFI) questionnarie.orgasm was assessed according to the relevant questions in the FSFI questionnaire .122 women were anorgasmic and they completed the SCL-90 questionnaire .the data were analyzed using the statistical software SPSS and the descriptive statistics (frequency-percent) and the deductive statistics (Correlation-t test). P value < 0.05 was assumed meaningful. Results: the prevalence of anorgasmia among women in Ardabil was 24.4% and the average age of th anorgasmic women was 31.1% .The relation between the obsession -compulsion disorder and the anorgasmia with P=0.000 was meaningful. The relation of depression with P=0.91 and using the SSRI drugs with P=0.28 and using the three cyclic anti-depression drugs with P=0.09 and using the ACE-I drugs with P=0.63 with the anorgasmia wasn't meaningful. The prevalence of anorgasmia in Ardabil women is at its high level and psychological factors influence this disorder so the diagnosis of this disorder and control of effective factors will help reducing of family fade and its impacts on society.

However, for poorly understood reasons, it is often lacking, especially among immigrants with lower family and socioeconomic resources. This study assessed health-related QOL of European and non-European immigrant adolescents and the contributions of socioeconomic difficulties, unhealthy behaviors, and violence. It included 1,559 middle-school adolescents from north-eastern France (mean age 13.5, SD 1.3; 1,451 French adolescents, 54 European immigrants and 54 non-European immigrants), who completed a self-administered questionnaire including sex, age, socioeconomic characteristics (family structure, parents' education, occupation, and income), unhealthy behaviors (uses of tobacco/alcohol/cannabis/hard drugs, obesity, and involvement in violence), having sustained violence, sexual abuse, and the four QOL domains measured with the World Health Organization's WHOQOL-BREF

Les systèmes de santé européens ont besoin de se renouveler pour faire face aux défis sanitaires ... more Les systèmes de santé européens ont besoin de se renouveler pour faire face aux défis sanitaires du XXIème siècle, notamment celui de l'évolution du profil sanitaire de la population et celui du rôle que doivent tenir à l'avenir les différents acteurs de la santé. Au cours de ces dernières années, des expériences internationales ont mis en lumière l'intérêt d'accorder une place active au patient et au citoyen dans les négociations autour des questions de santé. Motivés par le renforcement de la démocratie sanitaire ou par la recherche d'efficience des systèmes de santé, ces mouvements se caractérisent par l'attribution à la personne (patient ou citoyen) d'un statut de partenaire en santé. L'expérience que vit la personne avec sa santé, sa maladie, et son utilisation du système de santé l'amène à construire des savoirs profanes complémentaires aux savoirs scientifiques. Ces savoirs « patients » permettent de mieux encadrer et implémenter les interventions en santé. Cette approche s'applique à diverses activités dans lesquelles cet engagement peut avoir lieu : les pratiques cliniques et la coordination des soins (niveau micro), l'organisation des services et la gouvernance dans les établissements de santé (niveau méso), l'élaboration des politiques de santé (niveau macro), la recherche et la formation des professionnels de santé. Même s'il reste un certain chemin à parcourir, les premiers travaux publiés dans littérature montrent l'intérêt de s'inscrire dans cette approche en santé de par son impact sur la satisfaction des patients, la qualité de leur santé et les économies potentielles qu'elle peut générer. Dans plusieurs pays, des efforts sont entrepris pour impliquer les patients de manière active dans le système de santé. Toutefois, des questions demeurent sur l'implémentation et l'évaluation de ce nouveau modèle que constitue le partenariat et sur son impact sur le système de santé dans le contexte spécifique de chaque territoire. C'est dans ce contexte que le consortium interdisciplinaire APPS, composé de 9 équipes, soutenu par un financement européen et sous le contrôle des autorités régionales partenaires, a développé un programme de recherche sur les questions liées au développement d'une « Approche Patient Partenaire de Soins » (APPS) à l'échelle de la Grande Région (GR). L'objectif général du projet est d'explorer la transférabilité et l'applicabilité de l'approche patient partenaire au sein de la GR en tenant compte de ses caractéristiques socioculturelles, organisationnelles et légales. Sa finalité s'inscrit dans une véritable démarche concertée en matière de prévention en santé en GR. Le Livre Blanc est « une des pièces maitresses » de ce projet. Il propose des pistes de réflexion argumentées pour soutenir le développement du partenariat dans la GR.
