The efficacy of acidophilus milk and the effect of consuming unfermented milk that had been inoculated with yogurt bacteria have not been examined in children. We compared scores for breath H2 excretion and symptoms of 20 lactose-maldigesting children following ingestion of 250 ml of uninoculated milk with two identical milks inoculated with 10 10 cells of Lactobacillus acidophi/us or with a commercial yogurt starter culture containing lOS cells of Lactobacillus lactis and 10 10 cells of Streptococcus thermophilus. Nine of 10 subjects who were symptomatic following ingestion of uninoculated milk experienced a reduction in symptoms following ingestion of milk inoculated with L acidophilus, without a decline in H2 excretion. Five of 6 subjects who were symptomatic following uninoculated milk had decreased symptoms and a significant reduction in H2 excretion following milk inoculated with the yogurt culture. This study assessed growth of infants fed a partially hydrolyzed whey formula with L reuteri. Healthy term infants were randomized to 1 of 2 formulas (partially hydrolyzed whey formula with (PRO) or without (CON) L reuteri from 14 to 112 days of age. Anthropometric measures were assessed at 14, 28, 56, 84, and 112 days of age. Tolerance records were completed 2 days prior to each visit. A total of 122 subjects completed study per protocol (60 PRO, 62 CON). No differences were seen in daily weight gain, length, or head circumference. Overall, between groups, there were no significant differences in formula intake, stool frequency, color, consistency, flatulence, frequency of spit-up/vomiting, mood, sleep, or incidence of adverse events. Infants fed probiotic formula had similar growth to infants fed control formula. Both formulas were well tolerated.

Weight and length were evaluated longitudinally utilizing generalized estimating equations. HIV-infected children were compared with uninfected children born to HIV-infected and uninfected mothers, and to United States National Center for Health Statistics (NCHS) growth standards. Median weight and length-for-age of seronegative infants born to either seronegative or seropositive mothers approximated the NCHS median from birth to approximately 4 months of age. Median weight and length of HIV-infected infants deviated from the NCHS median at an earlier age, and the deviation was more pronounced than was observed for uninfected infants. Increased hygienic measures and pasteurization perpetuate this decreased microbe-host interaction. These changes in environmental and gut microbiota are associated with an altered and inadequate development of immune response, with related health implications. This study's objective was to determine whether a new amino acid-based formula (AAF) meets the AAP hypoallergenicity criteria. Methods. Children with CMA were randomized to double-blind placebo-controlled food challenges (DBPCFC) with a new AAF and a commercial AAF in crossover fashion followed by an at-home open challenge with the new AAF. Allergic reactions were assessed using a scoring system. Results. Thirty-three subjects completed DBPCFCs with both formulas without acute allergic reactions. The lower bound 95% confidence interval for hypoallergenicity was 91.3%. No unusual stool patterns, allergic symptoms, or signs of intolerance were reported during the open challenge. . The use of high-fat, low-carbohydrate diets for advancement of enteral feedings in children with short bowel syndrome

We report using low-carbohydrate (CHO), high-fat diets with medium chain triglycerides (MCT) to transition SBS patients from parenteral to enteral nutrition. Thirteen SBS patients (7 months to 7 years old, 7% to 60% residual bowel length) initially received protein hydrolysate drip feedings. When severe CHO malabsorption precluded feeding advancement, caloric density was increased with MCT to provide 56.6 +/- 7.3% of calories as fat and 32 +/- 4.8% as CHO. Patients reached 50% of requirements enterally by 16 +/- 14 months. Seven patients reached full requirements by 27 +/- 20 months. In this population, high-fat diets successfully accelerated the transition to complete enteral nutrition. (C) 1995 Aspen Publishers, Inc.

Since 3D models are becoming more popular, the need for effective methods capable of retrieving 3... more Since 3D models are becoming more popular, the need for effective methods capable of retrieving 3D models are becoming crucial. Current methods require an example 3D model as query. However, in many cases, such a query is not easy to get. An alternative is using a hand-draw sketch as query. We present a structure-based local approach (STELA) for retrieving 3D models using a rough sketch as query. It consists of four steps: get an abstract image, detect keyshapes, compute a local descriptor, and match local descriptors. We represent a 3D model by means of suggestive contours. Our proposal includes an additional step aiming at reducing the number of models that will be compared by our local approach. The proposed method is invariant to position, scale, and rotation changes as well. We evaluate our method using the first-tier precision and compare it with a current global approach (HELO). Our results show an increasing in precision for many classes of 3D models.

Foods currently available for weaning infants are generally low in fibre content and lack these potential benefits. Data documenting tolerance of FOS in weaning foods are greatly lacking. Our present objective was to evaluate the tolerance and gastrointestinal effects of FOS-supplemented infant cereal used as a daily addition to the diet of healthy infants. Healthy infants were randomly assigned to receive either 0·75 g FOS per serving of cereal or placebo for 28 d. The primary outcome of interest was gastrointestinal tolerance, which was assessed by daily parental reporting of functional variables for 28 d, including stool patterns and signs and symptoms of gastrointestinal tolerance. The prevalence of AD has increased in developing countries and regions, including the Middle East. Despite similarities in the presentation of the disease, there is a lack of consistent management and treatment guidelines for AD. The objective of this survey was to develop further insight into the management patterns of AD from dermatologists, pediatricians, and primary care/family medicine physicians in the Middle Eastern nations of Egypt, Lebanon, the United Arab Emirates, and Saudi Arabia. Methods: The survey was composed of 47 closedended, multiple-choice questions. These questions assessed physician and patient characteristics and treatment familiarity and approach. Results: A total of 400 physicians, including 200 dermatologists, 100 pediatricians, and 100 primary care physicians, participated in the survey. The findings provide insight into the management of AD by physician specialty within the region. A diverse array of management approaches was observed for both referral patterns and treatments for AD in the Middle East. Conclusion: The diversity of management tactics highlights the lack of a standard approach for the management of AD throughout the Middle East.

Background: Although IgA endomysial antibodies (EMA) and tissue transglutaminase (TG) are sensiti... more Background: Although IgA endomysial antibodies (EMA) and tissue transglutaminase (TG) are sensitive and specific serologic tests for the diagnosis of celiac disease, there is limited information on the association of the magnitude of antibody level with the severity of the histological abnormalities of the intestine. Purpose: To determine if EMA and TG titers correlate with the severity of histological changes in patients with celiac disease. Methods: We identified 148 children from our laboratory database that had EMA, TG and intestinal biopsies performed. IgA EMA was determined by indirect immunofluorescence with results expressed as a dilutional titer with positivity determined at 1:5. IgA TG was determined by an enzyme linked human immunosorbent ELISA assay with results expressed in standardized units. A modified Marsh histological grading system was used to describe the duodenal biopsies: Type 0 normal, I increased intraepithelial lymphocytes (IEL), II hyperplastic crypts, IIIa partial villus atrophy, IIIb subtotal villus atrophy, IIIc total villous atrophy. Results: Mean values for EMA (Table ) and TG (Table ) progressively increased with increasing Marsh score. There was considerable variability in EMA and TG levels for each Marsh grade, so that an individual level could not be utilized to predict histological severity. The data show that as a group, increasing severity of the histological lesion in celiac disease was associated with increased levels of both IgA EMA and TG antibodies.

Early acquisition and maintenance of a healthy intestinal microbiota is critical to the longterm ... more Early acquisition and maintenance of a healthy intestinal microbiota is critical to the longterm health of the host; and the mechanisms through which this occurs are beginning to be elucidated. Factors that lead to alterations in microbial composition during infancy are associated with inadequate and inappropriate host immune responses, which lead to the development of disease. A greater understanding of these factors and of these mechanisms will unlock potential strategies to modulate the acquisition and maintenance of a human microbiota that maintains and promotes health. These strategies will be critical in battling the epidemic of noncommunicable immunerelated diseases encountered in the modern world.

These include higher than generally recommended energy, protein, and saturated fat intakes. The majority of infants are bottle fed at some point in their first year of life, and their weaning diet often includes low intakes of fruits and vegetables, with high starchy, rather than green or yellow, vegetables. Early introduction of solids, use of cow's milk prior to 1 year of age, and high juice intake in the first 2 years - all less desirable diet practices - are improving, but are still prevalent. More preschoolers are likely to get sweets or sweetened beverages than a serving of fruit or a vegetable on a given day.
