The aim of this study was to evaluate functionality and clinical application of a novel immediate... more The aim of this study was to evaluate functionality and clinical application of a novel immediate device in the treatment of temporomandibular disorders (TMDs). To address the research purpose, authors developed and implemented a randomized control clinical trial. PATIENTS AND METHODS: Eighty patients were enrolled in this study and were randomly divided into two subgroups based on the treatment applied: patient group (PG) and control group (CG). The CG was not subjected to any kind of treatment, even placebo, in order to be able to assess the spontaneous development of the pathology over time. The PG was treated applying the novel device for a maximum of three months. The following parameters were evaluated at baseline (T0) and at the end of therapy (T1): presence/absence of articular noises, painful symptomatology (articular pains, muscle pains, headache, cervicalgia), parafunctional habits and duration of symptoms. The x 2-index of association was performed, with a p-value < 0.05 considered as statistically significant. RESULTS: No patient in the PG worsened its symptomatology. Thirteen patients (33%) declared themselves cured from their symptoms and were included in a monitoring protocol. Twenty-seven patients (67%) improved their symptoms and were treated with other conservative conventional methods to complete the therapeutic cycle. Therefore, 100% of PG obtained benefits from the application of the new therapeutic approach. In contrast, among patients of CG, eighteen subjects (45%) worsened their symptoms, while eighteen (45%) were defined as stationaries compared to T0 and only four (10%) were defined as improved. CONCLUSIONS: The device presented the following advantages: immediacy of use, reduction of waiting times for its application, good tolerability and comfort and specificity in the execution of tongue rehabilitation exercises.

The regulatory factors controlling the low-threshold mechanisms of consciousness within the stoma... more The regulatory factors controlling the low-threshold mechanisms of consciousness within the stomatognathic system at the central nervous level are not yet fully understood, as they are difficult to evaluate. Neuroplasticity, neuro signatures, engrams, and praxis patterns-which manage complex regulatory pathways potentially involved in occlusal hypervigilance, dysesthesia, and possibly even in mandibular clenching and bruxism-remain only partially known. As a result, these phenomena are often overlooked or mismanaged. Clinicians tend to focus more easily on visible malocclusions and muscular tension, often choosing invasive occlusal treatments that address peripheral symptoms. This article examines the low-threshold mechanisms of consciousness in the stomatognathic system that can affect the tone of masticatory muscles and change the interocclusal rest space to improve clinical understanding. Special emphasis is given to occlusal hypervigilance and occlusal dysesthesia, helping to achieve more accurate diagnoses and suitable therapeutic plans to avoid unnecessary or invasive treatments. A therapeutic protocol backed by previously published clinical trials is suggested. This protocol is minimally invasive, conservative, and designed to be cost-effective, in accordance with current international guidelines.

The purpose of this study was to evaluate the influence of an orthopedic oral device on the perfo... more The purpose of this study was to evaluate the influence of an orthopedic oral device on the performance of competitive athletes. Seven athletes from different sports have been examined during athletic tests with and without an oral device. The athletes were examined by two calibrated gnathologic operators and with specific radiography. All of the subjects were submitted to a postural exam on an electronic platform with and without the oral device applied. All subjects underwent objective tests, performed by federal trainers in specialized centers, to evaluate the athletic performance with and without the occlusal splint. The results showed an improvement of all the tests carried out. Our findings lead us to believe that the use of a cos-tumed oral device, it is able to optimize neuro-muscular coordination and to improve the competitive performance of athletes, while the muscular strength not respond significantly.

The authors developed a retrospective clinical study. A total of 2739 medical records were screened. One hundred and forty-one patients with chronic unilateral disc displacement without reduction and jaw limitation, treated with a multifunctional RA.DI.CA. splint, were enrolled. Temporomandibular pain, headache, familiar pain, neck pain, and emotional strain, maximum spontaneous mouth opening, and lateral excursions were evaluated at baseline (T0), after therapy (T1), and during the follow-up (T2). This study has been performed and compared before and after the application of a therapeutic protocol based on the use of an oral device working on the proper mandibular repositioning through the proprioceptive-based lingual re-education called “Lingual Ring”. Between January to December 2016, 32 TMD patients have been recruited out of a series of 321 individuals recruited at the Neurosensorial and Motorial Surgery Department, in Palermo (Italy). The neuromuscular system plays a primary role in the development of a myofascial syndrome of the ... more The neuromuscular system plays a primary role in the development of a myofascial syndrome of the temporomandibular joint. Patients suffering from this disease very often show changes in muscular functions. In biomechanical clinical pictures of temporomandibular myofascial syndrome the motor apparatus is involved only in the second stage. In order to determine the efficiency of the motor apparatus and to schedule a physiotherapeutic treatment program to improve condylar-meniscus coordination of the joint, an analysis of muscular function associated with postural examination has been performed, and this with a view to evidence possible skeletal changes, with particular regard to the spine. The study has been carried out in a group of healthy individuals in front of an other group of patients affected by the pathology mentioned.

This study analyzed the evolution of TMJ dysfunctions examined at the maxillo-facial department of the University of Rome "La Sapienza". The non therapeutic approach does not consider an addition al control-sample of the non treated being itself a control-sample of a parallel work whose aim was to analyze patients who had undergone a complete therapeutic cycle.

One the different interpretation of the pathological and clinical evolution of TMJ dysfunctions, ... more One the different interpretation of the pathological and clinical evolution of TMJ dysfunctions, the most-common of these pathologies is the condyle-disk uncoordination. This study analyzes the evolution of the treatment on the sample of patients affected by TMJ dysfunction and examined at the maxillo-facial department of the University of Rome "La Sapienza". After the TMJ dysfunction has been diagnosed the patients were informed of the individual therapeutic program which consisted of a temporary and definitive occlusal therapy. The aim of this scientific work is to control the efficiency within this therapeutic program in relation to the uncoordination of TMJ. In order to validate this approach the study was compared with another that considered the evolution of TMJ pathology in patients examined at the first diagnostic, visit but who had not undergone any treatment.

Temporo-mandibular disorders are classified into disorders involving TMJ components (articular di... more Temporo-mandibular disorders are classified into disorders involving TMJ components (articular disk displacement, deviation in form) and masticatory muscle disorders. In this study the authors describe their experience of the non-surgical treatment of these pathologies. They review a total of 312 patients suffering from TMJ disorders treated with occlusal splint therapy applied for any TMJ dysfunction. The sample was monitored before and after occlusal therapy with an average follow-up of 30 months. The results were evaluated by comparing the authors' method of assessment with the parameters put forward by Levitt in 1993. Both methods of evaluation gave comparable results with a percentage of long-term success in excess of 87%.

The aim of this work was to present data from a large sample of patients with Temporo-Mandibular ... more The aim of this work was to present data from a large sample of patients with Temporo-Mandibular Disorders (TMD) in order to clarify some aspects of the development of pathological conditions that affect large parts of the population. This preliminary work put in relation, through an epidemiological evaluation, anamnestic and clinical data collected from a sample of 2375 patients affected by TMD. Personal data were provided by questionnaire (age, sex, status, etc.), while clinical data were collected following a specific medical chart compiled in accordance with international criteria for TMD. An analysis of these data clearly showed that there were large quantities of variables involved in these disorders and which occur with a wide variety possible of clinical signs. This complexity, in accordance with the current knowledge that it is not able to clarify the etiology of these disorders, makes intricate both diagnostic then therapeutic aspects. You would find in front of a multifactorial systemic disease that, interfering with the individual bioavailability, exposes him to the possibility of perceiving noxious stimuli which otherwise would not able to reach the pain threshold. To support this hypothesis is the data founded in this report that showed how many patients suffered, at the same time, by muscle and spinal pain associated to headache, pain that occur with high frequency from the same side. The presence of these painful conditions tends to underestimate the dysfunctional problems even if they occur with greater clinical prevalence. Further research should be carried out to clarify these controversial issues.
