Objective: Eating disorders (EDs) are intractable diseases that frequently occur in young girls. The priority in their treatment is to improve nutrition. Autism spectrum disorder (ASD) is associated with EDs in 10 to 20% of cases, and their prognosis is often poor. In treatment, it is necessary to focus on the characteristics of ASD. We report on the implementation of nutrition counseling focusing on the characteristics of ASD as part of team medical care for two patients with ASD who were hospitalized for EDs. Methods: Two 15-year-old girls with ASD were admitted to a psychiatric hospital for treatment of EDs. Case 1 was an aesthetic athlete who was hospitalized due to low body weight caused by excessive exercise and dietary restrictions. Case 2 presented an anorexic reaction to stress, who was hospitalized for rapid weight loss due to anorexia. The registered dietitian focused on the characteristics of ASD and provided nutritional counseling based on 1) understanding and praise, 2) use of visual information, and 3) repetition of specific explanations. Results: In Case 1, the patient's goal weight was achieved by decreasing her activity level and consuming all her provided food, which increased every week. In case 2, anorexia subsided, and the target weight was achieved. Conclusion: In two cases with EDs and ASD, the usefulness of nutritional guidance based on 1) understanding and praise, 2) use of visual information, and 3) repetition of specific explanations was suggested.

Summar y Daily dietary exposure level of lead was estimated by market basket survey and low-contamination lead analysis. We collected 151 food samples of 17 food categories at supermarkets in Shizuoka city, Japan, and prepared 17 composites by mixing the food items in each of the categories after pretreatment and cooking. The composite was digested with ultrapure acid in a clean room and lead concentration was measured by ICP mass spectrometry. Lead was detected in all of the composites at the concentration of 0.21 to 124 ng/g fresh weight. Daily dietary exposure level of lead in Tokai district was calculated by multiplying lead concentration with daily intake of each of the 17 food composites, and by summing them all to find 4.69 μg/day. The obtained value is considered reliable because lead was detected in all of the composites: inclusion of "undetectable" composites generally results in uncertainty in daily exposure estimation based on market basket survey. "Algae" composite contributed most to the total daily exposure (28.8 %), which was followed by "Beverage" (13.8 %) and "Vegetable" (12.2 %) in this order. The present result will be valuable in the assessment and management of risk due to lead exposure in Japan.

Workfare programs, which have become prevalent worldwide. In Japan, governments are providing employment assistance for welfare recipients who are considered capable of working. The recipients of these services may have complex health concerns, all of which could be barriers to finding employment. However, there is a lack of research on this matter. The aim of this study is to clarify the structure of health characteristics that affect the willingness of welfare recipients to work via the workfare program. We conducted interviews with 31 welfare recipients in the workfare program and extracted [persistent agony, which is hard for others to understand], [vulnerability to stress], [difficulty of social adaptation], and [health management via their own style]as health characteristics. Additionally, we extracted [expecting to work], [worrying and mental conflict about ending welfare assistance], and [perception of social exclusion]as factors that affect their health and willingness to work. These categories relate to each other and cause «fracturing of willing of work due to work with complicated health problems versus the benefit of being a welfare recipient». The welfare recipients in this study who had physical, mental, and social health problems tended to be isolated; they have anyone to advise them regarding health problems. Health professional's support is needed to alleviate recipients' problems and provide more effective encouragement to strengthen their willingness to work.

We examined the relationships between physical activity performed during the perioperative period by patients undergoing gastrectomy for gastric cancer and such factors as life condition, psychological condition, and health-related quality of life (QOL). Patients undergoing gastrectomy for gastric cancer were assessed, and data on such factors as physical activity (exercise intensity (METs)× duration (h) =Ex), gastrointestinal symptoms, and dietary intake were collected continuously from before hospital admission until 2 months after surgery. Results of the Hospital Anxiety and Depression Scale (HADS) and the SF-8 Health Survey (a QOL scale), body weight, and hematological data were collected at the time of admission and discharge, 1 month after surgery, and 2 months after surgery. The subjects were 14 patients (9 men and 5 women) with a median age of 65.5 years.
