Abstract

Despite extraordinary advances in biomedicine and associated gains in human health and well-being, a growing number of health and well-being related challenges have remained or emerged in recent years. These challenges are often 'more than biomedical' in complexion, being social, cultural and environmental in terms of their key drivers and determinants, and underline the necessity of a concerted policy focus on generating healthy societies. Despite the apparent agreement on this diagnosis, the means to produce change are seldom clear, even when the turn to health and well-being requires sizable shifts in our understandings of public health and research practices. This paper sets out a platform from which research approaches, methods and translational pathways for enabling health and well-being can be built. Defining health by addressing individual, social and environmental determinants: New opportunities for health care and public health

Abstract The Millennium Development Goals (MDGs) mobilized global commitments to promote health, socioeconomic, and sustainable development. Trends indicate that the health MDGs may not be achieved by 2015, in part because of insufficient coordination across related health, socioeconomic, and environmental initiatives. Explicitly acknowledging the need for such collaboration, the Meikirch Model of Health posits that: Health is a state of wellbeing emergent from conducive interactions between individuals’ potentials, life’s demands, and social and environmental determinants. Health results throughout the life course when individuals’ potentials – and social and environmental determinants – suffice to respond satisfactorily to the demands of life. Life’s demands can be physiological, psychosocial, or environmental, and vary across contexts, but in every case unsatisfactory responses lead to disease. This conceptualization of the integrative nature of health could contribute to ongoing efforts to strengthen cooperation across actors and sectors to improve individual and population health – leading up to 2015 and beyond.

Examining Culture and Lifestyle in Public Health

Fundamental to the implication and understanding of the concept of culture and lifestyle in relation to public health is the fact that the context in which people live is of great importance for their health status and quality of life. Public health emphasizes the science and art of disease prevention, prolonging of life as well as promoting the health and well-being of the people. It targets to ensure conditions that promote the health and well-being of the people. However, the realization of the above objectives which is directed at the maintenance and improvement of health standard of the people is not only a function of the advancement and application of health science but as well a function of the people’s conscientious and determined efforts to maintain positive health practices and lifestyle. Construed from the foregoing perspective, it is easy to realize the fact that the concept of culture and lifestyle with regards to public health has sociological implication. Hence, though there are other variables that can also determine the health condition of a people, or even an individual, the focus of this chapter will centre on the sociological milieu or context. It is this sociological platform which actually defines people’s relationship within the structure, that also conditions their predisposition to healthy conditions of life or otherwise. From this sociological focal lens, emphasis will be on gender issues with regards to aspects of gender-based practices that exist as product of cultural construct as well as peer group influence as also generating from the lifestyle people choose to live, to critically examine the extent to which these determine or condition people’s health status and quality of life.

Toward Social Health for a Global Community

Mustakova-Possardt and Woodall explore what we know about the relationship between the social health of communities and the mental health of individuals. They advocate a dynamic systems perspective on social health and suggest that the mental health of an individual has to be understood in the context of the whole social group.Thus a healthy social organization is one that entails well-regulated harmonious inter-participation with each person and group. The health of the social organization entails maintenance of the dignity of every participant in that organization. Failure to meet this requirement creates prime ground for the emergence of all kinds of social ill-health. This chapter integrates insights that reach from the macro level of community life to the micro processes that animate the inner workings of persons. It has been recognized as a major conceptual contribution to the field.

Policy makers, development practitioners and ordinary citizens can reflect on well-being from many linked perspectives and find common ground that can lead to new sustainable solutions. The book presents research across a spectrum of issues on health, wellness and well-being, including perspectives on well-being and happiness; new paradigms in measuring well-being; well-being across the lifespan and wellness in the workplace. Additionally, the section on 'wellness in practice' offers readers practical tips to improve or sustain their own health and well-being. The authors commence the introductory chapter, The Global Framework for Wellness: Concepts, Theories, Measurements, Interventions & New Directions by articulating the global agenda for wellness and explaining key concepts related to health and well-being. All of this links people and the planet, to health and well-being, while embodying the guiding principles of equity, inclusivity, resilience, peace and partnerships, as well as climate action, which are integral to the United Nation's Sustainable Development Goals (SDGs), the World Health Organization's (WHO) Geneva Charter for Well-being and the WHO's One Health Initiative. This chapter further explores the subjects of measuring well-being; regional health disparities; well-being policies and programmes across the lifespan, inclusive of support mechanisms such as digital health. The chapter concludes by acknowledging the relevance of redefining the dominant narratives around wellness to incorporate issues of culture and lived experiences within the global village. 2. Planetary well-being and the global frameworks for health Planet Earth and its inhabitants face an existential crisis that threatens the wellbeing and survival of both humans and the environment [1]. Despite global, regional and national commitments to human rights and sustainable development, there is abundant evidence of worsening social, economic, political and environmental hazards that threaten well-being for all. Planetary well-being as defined by Kortetmäk Well-Being across the Globe-New Perspectives, Concepts, Correlates and Geography 2 et al. [2] is a concept that explains the moral need to give equal value and consideration to both human well-being and non-human well-being. This concept promotes transdisciplinary, cross-cultural discourse to not only address current social and ecological crises but to also promote social and cultural transformation of underlying systems that impact well-being. Kortetmäk et al. [2] state that: 'Planetary well-being is a state where the integrity of Earth's system and ecosystem processes remains unimpaired to a degree that species and populations can persist to the future and organisms have the opportunity to achieve well-being' ([1], p. 1). Planetary well-being and the sustainable development goals (SDGs) address similar planetary challenges and; promote sustainable policies, programmes and partnerships. Both reflect a symbiotic relationship between humans and the environment, and the sustainable use of resources to meet present and future needs. The WHO's [3] One Health Initiative also promotes 'an integrated, unifying approach to balance and optimize the health of people, animals and the environment' ([3], p. 1). It seeks to 'prevent, predict, detect, and respond to global health threats such as the COVID-19 pandemic' ([3], p. 1). It also promotes cooperation, coordination and collaboration between various sectors, communities and disciplines. The goal of the initiative is to identify and address the root causes of health-related problems and create long-term solutions. Programme priorities to promote wellbeing address issues such as: food, nutrition and water safety; the control of diseases that can spread between animals and humans; pollution management, and action to combat the emergence of microbes that are resistant to antibiotic therapy, as well as chronic non-communicable diseases (CNCDs) [3]. The WHO's (2021) Geneva Charter for Well-Being [4], also promotes global health and well-being. It charts a new approach to respond to the global social environment and health challenges. It uses multidisciplinary approaches that address inequities in both human behavior and the environment. The Charter's Principles promote holistic approaches to health that integrate physical, mental, spiritual and social well-being, as well as peace with nature and the environment. Like the SDGs [5], it is human rights-based and promotes social and environmental justice, peace, solidarity, as well as gender and inter-generational equity. Indicators of success in health and wellbeing, go beyond gross domestic product (GDP). They consider human and planetary well-being which contributes new perspectives and priorities for public spending and focus on empowerment, inclusivity, equity and meaningful participation. Global well-being is also closely linked to climate change which the United Nation's (UN's) Intergovernmental Panel on Climate Change 2008 (IPCC) [6] defines as 'a change in the state of the climate that can be identified … by changes in the mean and / or the variability of its properties, and that persists for an extended period, typically decades or longer' [6]. Boehm and Schumer [7] highlight 10 major findings of the IPCC 2023 report that highlight major risks to global well-being from climate change. Amidst the closing window to address climate change crises, the IPCC highlights possible actions to avert these risks, offering some hope for sustainable planetary well-being. 3. A global framework for integrating public health into wellbeing: why a public wellbeing system is needed

There is a growing focus on public health initiatives that prioritize wellbeing. The main question of our study is whether this, in its current form, can really represent a new response to the challenges of previous strategies, or whether there is a greater chance that it will essentially reproduce the problems associated with the paradoxical situation of public health. Based on a review, analysis and evaluation of the literature on wellbeing in public health, we outlined the foundations of a new meta-theory of wellbeing and a possibility for its social application. In our view, wellbeing is seen as a social representation of a combination of positive and negative freedom of choice concerning the quality of everyday life, used in a positioning process involving both individual and collective aspects. Health is a particular aspect of the social representation and positioning of wellbeing, which encompasses aspects of the physical, psychological, social and spiritual functioning of individuals. The wellbeing meta-theory also opens up the possibility for more e ective solutions to the social challenges related to wellbeing and salutogenetic health. It underscores the importance of the need for a dedicated social subsystem where the goals and organizational culture of the organizations involved are focused on wellbeing and health promotion. In our study, we consider this to be the Public Wellbeing System (PWS). Our conclusion is that the development and operation of a new set of institutions-the Public Wellbeing System (PWS)-based on the co-production of services that meet the needs and demands of society, and dedicated to the promotion of wellbeing, may provide an opportunity to overcome the public health paradox.

From public health to public good: Toward universal wellbeing

We aim to consolidate recent trends in public health into a reconceptualization of the field as public good. We build on several strands of theory, research and action to formulate a more impactful future for the field. Our argument comprises three main parts. In the first part, we describe the central components of the proposed public good: conditions of justice, experiences of mattering, and outcomes of subjective and objective wellbeing. In the second section, we identify continua of practices that paint a trajectory from traditional public health to ecological and participatory public health, to universal wellbeing framed as a public good. The continua are defined in terms of assumptions, practices and roles. Among others, these continua pertain to capabilities, scope of the field, ecological focus, timing of intervention, role of citizen, role of professional, role of settings and role of government. Finally, the third section introduces a series of strategies and recommendations to make the narrative of universal wellbeing as public good a reality.

For debate: a new wave in public health improvement

The rising burden of chronic disease poses a challenge for all public health systems and requires innovative approaches to eff ectively improve population health. Persisting inequalities in health are of particular concern. Disadvantage because of education, income, or social position is associated with a larger burden of disease and, in particular, multimorbidity. Although much has been achieved to enhance population health, challenges remain, and approaches need to be revisited. In this paper, we join the debate about how a new wave of public health improvement might look. We start from the premise that population health improvement is conditional on a health-promoting societal context. It is characterised by a culture in which healthy behaviours are the norm, and in which the institutional, social, and physical environment support this mindset. Achievement of this ambition will require a positive, holistic, eclectic, and collaborative eff ort, involving a broad range of stakeholders. We emphasise three mechanisms: maximisation of the value of health and incentives for healthy behaviour; promotion of healthy choices as default; and minimisation of factors that create a culture and environment which promote unhealthy behaviour. We give examples of how these mechanisms might be achieved.

Fostering a healthy public for men and HIV: a case study of the Movement for Change and Social Justice (MCSJ)

This paper examines the recently formed Movement for Change and Social Justice (MCSJ) in the township of Gugulethu, in Cape Town, South Africa. MCSJ is a health and social justice movement that has an unusual trajectory-guided by the political principles and strategies of South Africa AIDS activism, inspired but also frustrated by the contemporary progressive public health activism, and catalysed by a university research partnership at the University of Cape Town (UCT) that focused on innovative ways of using health information to spur community and health system action to respond to issues of men, masculinity and HIV. This paper synthesises findings from participant observation, qualitative process evaluations of MCSJ's work, and reflections from the UCT-based research team to develop a number of lessons about the challenges and opportunities in efforts to foster a healthy public for men and HIV. MCSJ's approach builds on current modes and models of public health thinking while also pushing for more responsive, more inclusive, more sustained, and more locally rooted forms of practice. The paper identifies a range of enabling conditions for MCSJ's work, describes the often delicate balancing act MCSJ has to manage in its work, and outlines some of the key strategies of this community-university partnerships that guided efforts to develop a healthy public for men and HIV. Lessons learned from this case study will be helpful for other efforts to promote and sustain engaged and impactful university-community collaborations to support the emergence of healthy publics.

The violence of narrative: embodying responsibility for poverty‐related stress

Narratives of self-responsibility are pervasive in neoliberally oriented contexts, and have been found to engender feelings of shame and failure amongst those affected by poverty. Here, we use findings from research in two low-income communities in southwest England to examine how these narratives become embodied within people's daily lives when they intersect with systems of welfare support and the current political drive to upscale treatment for common mental health conditions. Drawing on Bourdieu's notion of symbolic violence, we examine how narratives of self-responsibility and associated welfare reform strategies impact on the mental health of people living in economic hardship. The data show how such narratives inflict, sustain and exacerbate mental distress and suffering, and how they become naturalised and normalised by individuals themselves. We demonstrate how this situation pushes people to seek support from General Practitioners, and how clinical interactions can normalise, and in turn, medicalise, poverty-related distress. Whilst some people actively resist dominant narratives around self-responsibility, we argue that this is insufficient under broader sociocultural and political circumstances, to free themselves from the harms perpetuated by symbolic violence.

Women’s voices, emotion and empathy: engaging different publics with ‘everyday’ health histories

This article explores our experiences on a Wellcome Trust-funded project on women’s experiences of ‘everyday health’ in Britain between the 1960s and the 1990s. We explore issues around researching ‘everyday health’, including the generation and interpretation of source materials, and the role of empathy and emotion in interactions with different audiences as we share these materials in public engagement activities. We discuss three case studies of engagement activities to draw out potential uses of source materials and the responses of different audiences to these materials, and reflect on what we have learnt since embarking on these public engagement activities. Immigration/migration and healthy publics: the threat of food insecurity

Global climate change and the continued neoliberalization of food systems have exacerbated levels of food insecurity and hunger, producing an ever-expanding population of displaced persons who are also nutritionally vulnerable. Restrictive immigration policies in post-arrival and resettlement contexts compound with other cultural, social, political, and economic conditions to negatively affect the food security and health of displaced persons. This article engages a comparative ethnographic perspective for examining the migration-food security nexus.
