Caveats in the neuropsychological assessment of African Americans

Abstract

This preliminary investigation examined the predictive accuracy of six neuropsychological tests in a population of non-brain-injured African Americans. False positives were unacceptably high on five of the neuropsychological tests administered. These pilot data raise important questions about the utility of neuropsychological test norms with groups dissimilar in sociocultural background to the normative population. These findings are examined in terms of the relative merits of the race-homogenous and race-comparative paradigms and underscore the importance of conducting normative studies that involve ethnic minority populations.

Demographic, psychosocial, clinical, and neurocognitive baseline characteristics of Black Americans in the RAISE-ETP study

This study compared baseline characteristics of Black Americans and Caucasians with first-episode psychosis in the Recovery After an Initial Schizophrenia Episode Early Treatment Program (RAISE-ETP). Black American (N=152) and Caucasian (N=218) participants were compared on demographic, psychosocial, clinical, and neurocognitive measures. Results indicated several notable racial differences in baseline characteristics: a greater proportion of Black Americans than Caucasians were female, and Black Americans reported less personal and parental education than Caucasians. Black Americans were also less likely to have private insurance, more likely to be homeless or transient, had significantly poorer quality of life, more severe disorganized symptoms, worse neurocognition, and were less likely to abuse alcohol than Caucasians. Race-specific norms: Using the model of hypertension to understand issues of race, culture, and education in neuropsychology

Development of appropriate and clear guidelines for proper use of neuropsychological tests among ethnic minorities is a current and significant challenge. Although development of race/ethnicity-specific norms is popular, it is also controversial. Some have argued that these norms will improve the sensitivity and specificity of neuropsychological measures in detecting cognitive impairment. However, two major arguments have surfaced that: (1) race-specific norms ignore underlying cultural and educational factors for which race serves as a proxy, and (2) setting "more lenient" cutoffs for impairment among ethnic minorities denies these groups needed services. In this paper, we argue that recent research on hypertension reveals a number of crucial lessons for neuropsychologists who are struggling with this issue. The model of hypertension is helpful in understanding issues of construct validity in neuropsychological testing, and is also helpful in revealing possible underlying causes of poor cognitive test performance for which race serves as a proxy.

Brain Behavior Relationships Among African Americans, Whites, and Hispanics

There is increasing racial and ethnic diversity within the elderly population of the United States. While increased diversity offers unique opportunities to study novel influences on aging and dementia, some aspects of racial and ethnic research have been hampered by the lack of culturally and linguistically consistent testing protocols. Structural brain imaging is commonly used to study the biology of normal aging and cognitive impairment and may therefore serve to explore potential biological differences of cognitive impairment amongst racially and ethnically diverse individuals. To test this hypothesis we recruited a cohort of approximately 400 African American, Caucasian and Hispanic subjects with various degrees of cognitive ability. Each subject was carefully evaluated using standardized diagnostic protocols that included clinical review of brain MRI to arrive at a clinical diagnosis of normal cognition, mild cognitive impairment (MCI) or dementia. Each MRI was then independently quantified for measures of brain, WMH and hippocampal volumes by a technician blind to subject age, gender, ethnicity, race and diagnostic category. The appearance of infarction on MRI was also rated by examining neurologists. Regression analyses were used to assess associations with various MRI measures across clinical diagnostic categories in relation to racial and ethnic differences. Hispanic subjects were, on average, significantly younger and had less years of education than African Americans or Caucasians. Caucasians with dementia were significantly older than both African American and Hispanic dementia patients. Highly significant differences in MRI measures were associated with clinical diagnoses for the group as a whole after adjusting for the effects of age, gender, education, race and ethnicity. Subsequent independent analyses by racial and ethnic status revealed consistent relationships between diagnostic category and MRI measures. Clinical diagnoses were associated with consistent differences in brain structure amongst a group of racially and ethnically diverse individuals. We believe these results help to validate current diagnostic assessment of individuals across a broad range of racial, ethnic, linguistic and educational backgrounds. Moreover, interesting and potentially biologically relevant differences were found that might stimulate further research related to the understanding of dementia etiology within an increasingly racially and ethnically diverse population.

Psychological Assessment Do Neuropsychological Test Norms From African Americans in the United States Generalize to a Zambian Population

Healthy Zambian adults (N324) were evaluated to determine to what degree a Western neuropsychological (NP) test battery, with African American norms adjusted for age, gender, and education could be used in healthy Zambians, including 157 men (48.46%) and 167 women (51.54%) with an average age of 38.48 (SD  12.80) years and an average education level of 11.02 (SD  2.58) years. The NP battery included tests of attention/working memory, executive function, verbal fluency, processing speed, verbal and visual episodic memory, and fine motor skills. The Zambian Achievement Test (ZAT) and the U.S. Wide Range Achievement Test-4 (WRAT-4) reading subtest also were administered to assess literacy and quality of education. Similar to findings in Western countries, the Zambian results show substantial age and education effects on most tests and smaller, less consistent effects of gender. Beyond the basic demographic effects, urban/rural background had small effects on some cognitive variables, and the ZAT (but not WRAT-4) reading level was a robust predictor of performance on many NP tests, even when other background characteristics were controlled. Women in the United States tend to outperform men on tests of processing speed and episodic memory. However, Zambian women showed modest but statistically significant disadvantages versus their male counterparts. The results show that tests developed in the United States may be used in Zambia. Nevertheless, development and use of local cultural norms remains very important and is a must. New demographically corrected norms were developed for the cohort that was examined. Increasing Culturally Competent Neuropsychological Services for Ethnic Minority Populations: A Call to Action

US demographic and sociopolitical shifts have resulted in a rapidly growing need for culturally competent neuropsychological services. However, clinical neuropsychology as a field has not kept pace with the needs of ethnic minority clients. In this discussion we review: historical precedents and the limits of universalism in neuropsychology; ethical/professional guidelines pertinent to neuropsychological practice with ethnic minority clients; critical cultural considerations in neuropsychology; current disparities germane to practice; and challenges to the provision of services to racial/ethnic minority clients. We provide a call to action for neuropsychologists and related organizations to advance multiculturalism and diversity within the field by increasing multicultural awareness and knowledge, multicultural education and training, multicultural neuropsychological research, and the provision of culturally competent neuropsychological services to racial/ethnic minority clients. Lastly, we discuss strategies for increasing the provision of culturally competent neuropsychological services, and offer several resources to meet these goals.

Ethnic differences in functional and neuropsychological test performance in older adults

Numerous studies outline discrepancies in neuropsychological test profiles in African American and European American samples, despite similarities on major background factors. In our clinical sample of convenience, African American and European American older adults did not diverge on age, years of formal education, or global cognitive impairment. We predicted that ethnic groups would differ on a financial index, reading, and naming measures, but not on form discrimination and health problem-solving tasks. Results showed the expected patterns with the exception of form discrimination, which was lower in the African American group. Ethnicity no longer predicted standing on financial and nonverbal discrimination measures once we adjusted scores to equate for word-reading ability. In contrast, ethnicity status continued to explain significant variance in confrontation naming following adjustment for reading. When ethnic groups were subdivided by sex, European American men tended to have the highest scores of all four subgroups despite having equivalent levels of education. Therefore, future studies should examine the impact of cognitive activity throughout adulthood on real-world function and neuropsychological test performance among groups defined by both ethnicity and sex. In conclusion, in African American and European American elders seen for clinical assessment, literacy appeared to be associated with discrepant scores on a measure of real-world financial knowledge and skill, as well as with differences in verbal and nonverbal neuropsychological measures.

Race-Norming of Neuropsychological Tests

Recent studies in the United States indicate that some neurologically intact minority groupings perform well below White Americans on neuropsychological tests. This has sparked the production of race-norms, especially for African Americans, that seek to reduce false positive rates (i.e., neurologically intact individuals misdiagnosed with cognitive impairment) in neuropsychological assessments. There are problems with this enterprise including: possible justification for inferior/superior treatment of different racial groupings; unknown effects on false negative rates (i.e., cognitive deficit misdiagnosed as normal); the overlooking of factors possibly responsible for group racial differences (e.g., acculturation); non-scientific and non-operational definitions of race/ethnic groupings; and an impossibly large number of potential race/ ethnic groupings for which to generate race-norms. An alternative approach is to use a single set of combined race/ ethnic norms and estimate preexisting neuropsychological skill levels by using individual comparison standards. This alternative has been poorly researched, a situation that needs correcting.

The Cross-Cultural Neuropsychological Test Battery (CCNB): Effects of age, education, ethnicity, and cognitive status on performance

The Cross-Cultural Neuropsychological Test Battery (CCNB) was developed in response to the growing need for a culturally fair method of assessing cognitive functioning in minority populations. The CCNB includes 11 tests, takes approximately 90 minutes to administer, and has been given to 336 healthy older adults and 90 demented patients from five ethnic groups (African-American, Caucasian, Chinese, Hispanic, and Vietnamese). The participants' age ranged from 54 to 99 years (M = 74.5, SD = 8.4) and they had from 0 to 22 years (M = 10.2, SD = 4.6) of education. While education contributed significantly to performance on most of the tests, age affected scores on measures of recent memory and psychomotor speed. Ethnicity and language affected scores on measures of attention, category fluency, and visualspatial functioning. As a whole, the demented patients scored significantly worse than their healthy peers on the entire CCNB. Tests of mental status and recent memory proved particularly useful in discriminating the two groups. Overall, the results demonstrated the effectiveness of the CCNB in identifying cognitive impairment in minority individuals and highlight the importance of considering education, age, and language when interpreting neuropsychological test findings. Cross-Cultural Neuropsychological Test Battery 3 The Cross-Cultural Neuropsychological Test Battery (CCNB): Effects of Age, Education, Ethnicity, and Cognitive Status on Performance The field of neuropsychology is being challenged by the increasing ethnic diversity and aging of the United States population. Currently, 12% of all Americans are over age 65 and 1 in 10 of these individuals is of minority background. By the year 2020, the 65 plus age group is expected to comprise 22% of the entire United States population, with a third of these seniors coming from diverse minority groups (U.S. Bureau of the Census & NIA, 1993). Despite the anticipated increase in the number of minority elders, relatively little work has been directed towards understanding the effects of age, culture, language, and education on neuropsychological test performance (Ardila, 1995; Gurland et al., 1992; Loewenstein et al., 1994). The lack of research on this issue is surprising as over a decade ago the National Institutes of Health, Consensus Development Conference, Statement on Diagnosing Dementias specifically noted the need for an "evaluation of results obtained with current neuropsychological instruments in populations that differ in age, education, ethnic composition, and social and cultural backgrounds" (NIH, 1987). As the number of older adults continues to grow, the incidence of chronic conditions associated with aging, such as Alzheimer's disease (AD), can be expected to increase in minority as well as nonminority populations. The prevalence of AD has been estimated as ranging from 1-3% in the 65-74 age group, from 7-19% in those 75-84, and from 25-47.2% in those 85 and older (Evans et al., 1989; U.S. Congress Office of Technology Assessment, 1987). Although the number of neuropsychological studies of AD has increased substantially over the past decade, most of these investigations have included primarily English-speaking Caucasian participants. There are relatively few published studies examining neuropsychological functioning in either cognitively intact or demented minority populations. Apart from a handful of studies (Ganguli et al., 1991; Glosser et al., 1993), most of the existing research contrasts the performance of English-speaking Caucasians on specific neuropsychological tests with either African

Disparities in Cognitive Functioning by Race/Ethnicity in the Baltimore Memory Study

The Baltimore Memory Study is a cohort study of the multilevel determinants of cognitive decline in 50-70-year-old randomly selected residents of specific city neighborhoods. Prior studies have demonstrated that cognitive function differs by race/ethnicity, with lower scores in minorities than in whites, but the underlying basis for these differences is not understood. Studies have differed in the rigor with which they evaluated and controlled for such important confounding variables as socioeconomic status (SES), health-related behaviors, comorbid illnesses, and factors in the physical environment. The goal of this study was to describe differences in neurobehavioral test scores by race/ethnicity, before and after control for a four-dimensional measure of SES and health-related behaviors and health conditions, in a cross-sectional analysis of first visit data. Random samples of households in the study area were selected until enrollment goals were reached. Among the 2,351 persons on whom eligibility was determined, 60.8% were scheduled for an enrollment visit; of these, 1,140 (81.3%) were enrolled and tested. These study participants were 34.3% male and 65.7% female and were from 65 Baltimore, Maryland, neighborhoods. After adjustment for age, sex, and testing technician, there were large and statistically significant differences in neurobehavioral test scores by race/ethnicity, with African-American scores lower than those for whites, for both men and women. After adjustment for individual SES (educational status, household income, household assets, and occupational status), the average difference declined by 25.8%. After additional adjustment for SES, health-related behaviors and health conditions, and blood lead, the average difference declined another 10%, but large differences persisted; African Americans had test scores that averaged 0.43 standard deviation lower than those for whites across all neurobehavioral tests. These differences were present in all cognitive domains, including tests that would not be characterized as susceptible to differential item functioning by race/ethnicity, suggesting that the results are not due to race/ethnicity-associated measurement error.

Neuropsychological assessment is recommended when we suspect an organic de fi cit or insult (either congenital or acquired, as in the case of stroke, traumatic brain injury, dementia, fetal alcohol syndrome, pervasive developmental disorder, or toxin exposure to name a few) has impacted expected functional performance for an individual, or when we wish to rule out such de fi cits or insults. These assessments take on a holistic approach in that we attempt to identify the region of the brain that is impacted, identify the extent of the insult, ascertain the spared functioning, and predict prognosis through a multimodal/multidomain assessment.

As management of ''representation'' became unwieldy, and to avoid potential imbalances or slights, efforts toward ''representation'' were largely abandoned in favor of invitation based on expertise. In the end, panelists were chosen to reflect key perspectives that needed discussion or to contribute as a function of their demonstrated expertise in an area critical to the Summit. Finally, each of these panelists was allowed to invite one other participant, in the hopes of populating the Summit with trainees or early career professionals with a strong commitment to diversity issues. The overarching goal of the Summit was to develop a plan for the future of cross-cultural neuropsychology. Specific targets for discussion included the delineation of the proper use of ethnic norms, allocation of resources for research, scientific approaches to the study of multicultural neuropsychology, education and training, and the development and dissemination of products from the Summit. It was intended that the Summit would be an inspirational springboard from which many efforts would follow.
