Abstract

On average, language and communication characteristics of individuals with Down syndrome (the most common genetic cause of intellectual disability) follow a consistent profile. Despite considerable individual variability, receptive language is typically stronger than expressive language, with particular challenges in phonology and syntax. We review the literature on language and literacy skills of individuals with Down syndrome, with emphasis on the areas of phonology, vocabulary, syntax, and pragmatics. We begin by describing the hearing, oral-motor, cognitive, social, and prelinguistic and early nonverbal communication characteristics of individuals with Down syndrome. We conclude with a discussion of clinical implications and research directions.

Conflict of interest: non-existent may occur changes in language, as expressive as receptive, with the reception more preserved, when compared to verbal expression 3. A difficult of verbal expression's presented by DS individuals is to narrate facts and happening with precision 4. It had been observed that these children, in narrative task, present common and stuttering non-fluencies. It exists an average prevalence of stuttering in 3% of intellectual disability children contrasting to 1% in general population. DS individuals present more non-fluencies than intellectual disability individuals of other origins 5. Do not exist a consensus in literature to the causes of non-fluency in DS 6. Some writers associate the non-fluency to motor loss, others, to language impairment 7. Besides, the articulation's changes and language disturb can co-exist, or even overlap to others disorders 8. Based on the above, this study has as aim review the literature, between 2002 and 2012, about non-fluency and the narrative task development in Down syndrome.

The Association between Difficulties with Speech Fluency and Language Skills in a National Age Cohort of Children with Down Syndrome

The present study (a) addressed difficulties in speech fluency in children with Down syndrome and typically developing children at a similar non-verbal level and (b) examined the association between difficulties with speech fluency and language skills in children with Down syndrome. Data from a cross-sectional parent survey that included questions about children’s difficulties with speech fluency, as well as clinical tests from a national age cohort of 43 six-year-olds with Down syndrome and 57 young typically developing children, were collected. Fisher’s exact test, Student’s t-test, linear regression, and density ellipse scatter plots were used for analysis. There was a significantly higher occurrence of parent-reported difficulties with speech fluency in the children with Down syndrome. Vocabulary and Syntax Reception in Children with Down Syndrome

Introduction: Intellectual disability and specific cognition in down syndrome (DS) affect language development and processing. We assessed vocabulary and syntax reception among children with DS with typically developed (TD) Persian-speaking Iranian children. We also investigated the association between vocabulary and syntax reception in children with DS. Materials and Methods: Using the Stanford–Binet intelligence scales test, 18 TD children (4-6 years old) and 18 DS children were matched based on non-verbal mental age. Vocabulary reception was assessed using the receptive picture vocabulary test (RPVT) and syntax reception by the Persian syntax comprehension test. Results: Mean total vocabulary reception was lower in children with DS (P<0.001). Syntax reception was also impaired in children with DS compared to the normal values of the test. Among children with DS, vocabulary reception was positively correlated with syntax reception. Every unit increase in total vocabulary reception score was associated with 0.08 (95% confidence interval, CI: 0.04-0.12) improvement in syntax score in children with DS. Conclusion: Vocabulary and syntax reception in DS children, especially complex syntax structures, was impaired. The observed association between vocabulary and syntax reception shows that vocabulary reception improvement might result in progressing syntax reception in children with DS.

SIGNIFICANCE OF PRAGMATIC LANGUAGE ASSESSMENT IN DOWN SYNDROME: A COMPARATIVE STUDY BETWEEN DS, ASD AND ITS CO-MORBIDITIES

Down syndrome exhibits cognitive and linguistic impairment, but few of them shows pragmatic deficit as Autism Spectrum disorder (ASD). We tried to explore communication and pragmatic difficulties in Down syndrome with and without ASD. CCC was administered on 97 children (6 and 20.11 years) with Malayalam as their mother tongue. Statistical analysis revealed a significant difference in pragmatic composite score between the four groups. Pure DS scored highest pragmatic score, whereas ASD scored least. DS+ASD scored poorer than DS plus. The study could highlight the usefulness of CCC. We come across with different subcategory of Down syndrome (DS), ie., pure DS, DS plus & DS+ASD; as it is a spectrum of disorder like ASD. We conclude that pragmatic language assessment is important in Down syndrome, in order to plan proper management strategies and good prognosis.

Down syndrome phonology: Developmental patterns and intervention strategies

This paper describes phonological development in children with Down syndrome paying particular attention to underlying defi cits and intervention strategies. The fi rst section provides an overview of factors believed to infl uence phonological development in this population. The second section describes four aspects of Down syndrome phonology: (1) the prelinguistic stage; (2) the transition to speech; (3) the phonology of the single words; and (4) phonological characteristics of conversational speech with a focus on intelligibility. Intervention strategies associated with each aspect are also presented. Children with Down syndrome are slow to acquire the phonological system of their mother tongue. In spite of normal or nearly normal prelinguistic development, these children are delayed in the use of meaningful speech and slow to acquire a productive vocabulary. In some cases their speech remains unintelligible throughout childhood and adolescence, making it diffi cult to communicate with those around them. The purpose of this paper is to summarize research on phonological development of children with Down syndrome with attention to underlying defi cits and to the speech characteristics of prelinguistic vocalisations as well as words and conversation. Current views on intervention are also described.

Phonological Disorders of Children with Down Syndrome Based on the Level of Intelligence: A Case Study

Many of speech therapists have given therapy to the children with Down Syndrome (DS) without thinking whether the children belong to mild mental retardation (IQ 50-69) or moderate mental retardation DS children (IQ 35-49). Therefore, a study was conducted to see the differences and the findings showed that the moderate mental retardation DS child dominated the three types of phonological disorders, i.e. phoneme substitution, simplification, and addition. In this research, the last type of phonological disorder was not found for both DS children. In addition, the mild mental retardation DS child responded the stimulus, yet the moderate mental retardation DS child could not answer about 1/5 stimulus given. It was also found that the moderate mental retardation DS child was so sensitive with touching. So, the therapy for the DS should not be the same. For the mild mental retardation DS children (IQ 5-069), the stimulus given to them could be by using flash cards, but for the moderate mental retardation down syndrome children (IQ 35-49), the stimulus should be something real, concrete, or something that could be touched so that they can respond the stimulus well and their language can be better in the future.

This article provides an overview of the literature on phonological development in infants and children with Down syndrome. The review is divided into four major sections: the first summarizes studies of factors that may impede normal phonological development in this population: hearing loss and abnormalities of the middle ear; anatomical and physiological features of the speech mechanism; characteristics of input; and perceptual and encoding abilities. The second section examines prelinguistic vocal development and the links between babble and speech. The third section reviews literature on phonological acquisition after the onset of meaningful speech and briefly summarizes research on speech intelligibility in Down syndrome individuals. The fourth section discusses recommendations for intervention programs aimed at improving articulation and phonology in children with Down syndrome. 1997 Wiley-Liss, Inc.

Noteworthy is the positive correlation between higher intelligence and enhanced phonological performance in both groups. The study suggests the importance of early interventions for children with Down syndrome and advocates for personalized educational programs.

Down syndrome (DS) as a chromosomal abnormality has different symptoms including cognitive problems and language delay. Studies showed a heterogeneous profile of language skills in this group of children. This study aimed to provide further information about the most vulnerable area of language-grammatical skills-regarding the unanalyzable utterances, clauses, phrases, grammatical morphemes, and mean length of utterances (MLU) in children with DS and compare them with those of non-verbal age-matched controls. Materials and Methods: The grammatical structures of 12 children with DS (non-verbal age=39 months) were compared to those of 50 non-verbal mental age-matched controls (non-verbal age=41 months). Clause, phrase, and grammatical morphemes were investigated through spontaneous language sample analysis according to Persian-language assessment remediation and screening procedure (P-LARSP). Results: Children with DS had higher percentages of unanalysable text units compared with typically matched peers (P≤0.001). The MLU, number of phrases and clauses structures, and inflectional morphemes were significantly lower in children with DS compared with typical children (P<0.001). Conclusion: Children with DS showed a noticeable gap in grammatical structures compared with typical children. The possibility of specific language problems in children with DS should be considered by future studies.

Parental Reports of Spoken Language Skills in Children With Down Syndrome

Spoken language in children with Down syndrome and in children in a normative group was compared. Growth trends, individual variation, sex differences, and performance on vocabulary, pragmatic, and grammar scales as well as MaxLU (maximum length of utterance) were explored. Subjects were 330 children withDown syndrome (age range: 1–5 years) and 336 children in a normative group (1;4–2;4 years;months). The Swedish Early Communicative Development Inventory-words and sentences (SECDI-w&s) was employed. Performance of children with Down syndrome at ages 3;0 and 4;0 was comparable with that ofchildren in the normative group at ages 1;4 and 1;8 respectively. In comparison with children in the normative group of similar vocabulary size, children with Down syndrome lagged slightly on pragmatic and grammar scales. The early development proceeded in most cases with exponential or logistic growth. This stresses the great potential of early intervention.

Successful communication depends on language content, language form, and language use (pragmatics). Children with Down syndrome (DS) experience communication difficulties, however little is known about their pragmatic profile, particularly during early school years. The purpose of the present study was to explore the nature of pragmatic communication in children with DS. Twenty-nine six-year-old children with DS were assessed, in the areas of 1) initiation, 2) scripted language, 3) understanding context and 4) nonverbal communication, as reported by children&#39;s parents via the Children&#39;s Communication Checklist-2 (Bishop, 2003).     Narrative skill in boys with fragile X syndrome with and without autism spectrum disorder

    We examined recalled narratives of boys with fragile X syndrome with autism spectrum disorder (FXS-ASD; N=28) and without ASD (FXS-O; N=29), and compared them to those of boys with Down syndrome (DS; N=33) and typically developing boys (TD; N=39). Narratives were scored for mentions of macrostructural Story Grammar elements (Introduction, Relationship, Initiating Events, Internal Response, Attempts/Actions, and Ending). We found that narrative recall is predicted by short-term memory and nonverbal mental age levels in almost all groups (except TD), but not by expressive syntax or caregiver education. After adjusting for these covariates, there were no differences between the three groups with intellectual disability (ID). The FXS-ASD group, however, had significantly poorer performance than the TD group on the overall Story Grammar score, and both the FXS-O and FXS-ASD groups had lower Attempts/Actions scores than the TD group. We conclude that some form of narrative impairment may be associated with FXS, that this impairment may be shared by other forms of ID, and that the presence of ASD has a significantly detrimental effect on narrative recall.

    Prosody is a fundamental speech element responsible for communicative functions such as intonation, accent and phrasing, and prosodic impairments of individuals with intellectual disabilities reduce their communication skills. Yet, technological resources have paid little attention to prosody. This study aims to develop an automatic classifier to predict the prosodic quality of utterances produced by individuals with Down syndrome, and to analyse how inter-individual heterogeneity affects assessment results. A therapist and an expert in prosody judged the prosodic appropriateness of a corpus of Down syndrome’ utterances collected through a video game. The judgments of the expert were used to train an automatic classifier that predicts prosodic quality by using a set of fundamental frequency, duration and intensity features. It is generally agreed that by adding something fun to the learning process students become more engaged and, consequently, retain more knowledge.     The Locus Preservation Hypothesis: Shared Linguistic Profiles across Developmental Disorders and the Resilient Part of the Human Language Faculty

    Grammatical markers are not uniformly impaired across speakers of different languages, even when speakers share a diagnosis and the marker in question is grammaticalized in a similar way in these languages. The aim of this work is to demarcate, from a cross-linguistic perspective, the linguistic phenotype of three genetically heterogeneous developmental disorders: specific language impairment, Down syndrome, and autism spectrum disorder. After a systematic review of linguistic profiles targeting mainly English-, Greek-, Catalan-, and Spanish-speaking populations with developmental disorders (n = 880), shared loci of impairment are identified and certain domains of grammar are shown to be more vulnerable than others.
