Abstract

The purpose of this chapter is to discuss design of acute care settings with a focus on the evolution of design practice over the last decade. In particular, ideas around healing environments describe how design is contributing to enhanced health and wellbeing and how recent developments are helping to create better designs. The chapter covers aspects of the theoretical hypothesis explaining the relationship between people, environment and healing with a focus on three elements: how people perceive the environment, social interaction as part of healing and the influence of people's socio-cultural background. The chapter looks at evidence-informed design in the context of acute healthcare settings whilst also covering trends, challenges and opportunities in healthcare design, and how this field is developing. A case study demonstrates elements of a real project on a before and after basis. The chapter concludes with the message that design of acute care settings involves much more than simply providing a space for care services to take place.

Holistic Healing Framework: Impact of the Physical Surrounding Design on Patient Healing and Wellbeing

With advances in healthcare design settings and technology, previously established frameworks warrant to be revisited. A new framework based on the principles of holistic healing that integrates design theories and incorporates significant interior design components that help designers create healing spaces, through better understanding of user perception and preference of interior design elements is discussed. The holistic healing framework proposed is aimed at achieving satisfaction for single-occupancy room users with their hospital rooms. This framework appears to be particularly valuable in studying and understanding the physical setting and individual interactions within the environmental behaviour and how people perceive their surroundings. The onus is on designers to introduce design and décor to meet hospital room user requirements and showcase designer skills. Addressing a hospital room user's satisfaction with healthcare facilities is only possible if these requirements are clearly documented and environmental behavior theories are fully understood.

Evidence-based design: The role of inpatient typology in creating healing environment, hospitals in Erbil city as a case study

This paper discusses the potential role of inpatient layout in creating a healing environment for its users, whether there is an existing relationship between inpatient typology and healing environment. The study employs an evidence-based design method adopting a qualitative-quantitative approach as a multiple research methodology. Quantitatively, an EBD questionnaire-ASPECT was distributed to the users to assess the healing environment within specific cases; syntactic maps were used to assess the configuration of inpatient layouts. Qualitatively, a modified EBD-AEDET tool was used as a checklist evaluation. Four different inpatient typologies (H-shape, T-shape, U-shape, L-shape) were chosen to represent four hospitals in Erbil city. Results showed that there is a clear effect of the inpatient typology on the healing environment of patients. Research outputs provide a useful database for healthcare designers in their future designs, especially for inpatient units due to their influential role in creating a healthy and safe healing environment for patients.

Normalcy in healthcare design: An extension of the natural and built environment

The healthcare industry is increasingly adopting integrated care systems in which building design plays a significant role in patient experiences, treatment and wellbeing. People-centred building design builds on several decades of research that demonstrate how certain environments stimulate behavioural, as well as physiological and neurological changes in people conducive to healing and general wellbeing. Importantly, these design considerations for health and wellbeing are typically well aligned with concurrent efforts to increase the sustainability of healthcare facilities. This paper explores the design of healing gardens in hospitals and how these environments can moderate stress and anxiety for patients and families during potentially traumatic periods. Healing Spaces: Elements of Environmental Design That Make an Impact on Health

The "ambiance" of a space has an effect on people using the space. In recent years, design for health care environments has begun to include esthetic enhancements in an attempt to reduce stress and anxiety, increase patient satisfaction, and promote health and healing. In this paper, the authors survey the existing research on those elements of the built and natural environment most often asserted by proponents as being inherently healing or promoting health. We postulate a hierarchy of effect of environmental elements ranging from simply nontoxic to safe (both physically and psychologically) to "providing a positive context" to being actively salutogenic. Most relevant research has been concentrated on a limited number of settings and is inadequate to inform the creation of design guidelines for the physical elements of an optimal healing environment. Opportunities exist to make meaningful contributions in this area that are likely to make a significant impact on health outcomes of human beings.

Hospital design has an influence on performance of the hospital. With globalization and medical tourism healthcare sector has changed enormously in every aspect to cater to the expectations and need of people. Today hospitals are largely working in corporate style for its customer satisfaction, good working environment and patient safety which required architects and designers to change their outmoded hospital design process. Hospital design is about creating physical environment which has its direct impact on the patient recovery and safety. Recent researches advocated that certain design elements should be considered in the design of a hospital building to promote the healing environment which is critical to the patient recovery rate and safety. This proposes the vital role of Hospital Architects and designers to be played in coordination with the hospital administration and staff at the early stage of project conceptualization so that all the required steps can be well preplanned and implemented for a successful design and functional hospital building with a healing environment.

Healing architecture: A study on the physical aspects of healing environment in hospital design

The complexity of designing healthcare facilities functionally and psychologically is apparent. Apart from building services, various requirements of the Ministry of Health (MoH) are expected to be followed by healthcare designers. This paper outlines the role and importance of physical built environment in the design of hospitals as a means to create a healing environment. Lighting (day and artificial lighting) will be one of the focus areas to be examined in the study of healing architecture. At the outset, the existing conditions in 100 to 150 bedded hospital buildings in Malaysia will be reviewed and further investigation will be carried out with particular attention to the design and implementation of lighting and its relationship to other environmental factors. The paper concludes with the discussion on the influence of lighting in hospital design as one of the physical aspects in creating a healing environment. Conference theme: Daylighting and lighting

Healthcare spaces: The intersection of design, health and well being

Architectural design is decisive in creating spaces that positively affect users’ psychological and physical health. This article delves into a few key environmental factors identified in existing research that are associated with their effects on human behaviour and overall well-being within healthcare spaces. Significant design strategies, such as exposure to natural light, acoustic control, incorporation of biophilic elements, and the use of flexible layouts, are highlighted as approaches to mitigate commonly observed adverse outcomes associated with hospitalization. These outcomes encompass issues such as social isolation, stress, depression, sleep disturbances, and medical errors. Drawing insights from multiple studies, this article compiles supportive evidence that can inform the creation of improved healthcare spaces by transforming the environmental factors that impact users into potential benefits. This will enhance patient outcomes while also elevating the experiences of a broader range of stakeholders, including healthcare staff and the families of patients. This article does not propose a completely new theory or model but aims to advance the conversation around the transformative power of design in healthcare settings.

    Effective antimicrobial resistance communication: the role of information design

    Getting the message across about the dangers of antimicrobial resistance (AMR) and how to prevent it is a global priority. This article discusses the role of information design in the effective communication of information about AMR, and suggests that the design process-user input, iteration and consideration of circumstances of use-is key to successful communication. To illustrate this, using material from the Otto and Marie Neurath Isotype Collection at the University of Reading, this article considers the work of Otto and Marie Neurath who developed the Isotype system for visual education in the 1920s. They collaborated with scientists, writers and illustrators to ensure that messages were both accurate and relevant for their intended audiences, so that people could make their own decisions based on factual explanation. Their use of pictograms and schematic illustrations, consistent use of colours to represent meaning, and carefully considered relationships between text and image provided a distinctive visual 'look and feel'. The charts designed by the Neuraths to educate people about health included a series made in the 1930s for the US National Tuberculosis Association to explain the dangers of, and how to prevent TB. This article reviews how these charts were designed, including the designated role of the 'transformer' who worked to ensure that scientific facts were presented in an understandable form. The way of working and the verbal and graphic characteristics of the charts deserve serious consideration for effective communication of information about AMR today. Particularly relevant are the use of schematic images using scale and simplification to attract attention, the use of story-telling to engage people, not overwhelming readers with too much information, and using straightforward language. The article asserts that cross-disciplinary working is key to successful explanation about the dangers of AMR. It illustrates this with reference to a research project, influenced by the Isotype approach to public health communication, that brings together academics and practitioners in information design, architecture, ergonomics and human factors, and pharmacy in communicating information about AMR to people using community pharmacies.

    Explaining drug-resistant infection in community pharmacies through effective information design

    This paper describes a research project in which information design, human factors, architecture and pharmacy academics worked with pharmacy professionals and pharmacy users to consider how to present information about antimicrobial resistance (AMR) in a community pharmacy setting. Project outcomesas a result of an innovative design competitionincluded five different design solutions that explain aspects of AMR within the context of a community pharmacy. The project raised awareness in pharmacy professionals of how design can be used to challenge ideas and encourage new ways of thinking to communicate public health messages. Two winning prototype solutions were installed in a Day Lewis pharmacy in Reading and evaluated by pharmacists and pharmacy users. We make preliminary recommendations for effective health communication in community pharmacies.

    The application of sustainable materials and their integration into newly constructed, valuable, and cultural architecture is a topic currently often mentioned in connection with the new initiative called the New European Bauhaus. The aim of this paper is to highlight the impact of wood material in the interiors of preschool institutions, its positive influence on the development of children and its inclusivity in education. The use of wooden furniture and wooden structural elements in kindergarten interiors opens a new area of research and interest in the context of promoting diversity and access for every child, regardless of their abilities or limitations.
