Desde que Sackett y Haynes en los años 70 identificaran que la mitad de los pacientes crónicos no... more Desde que Sackett y Haynes en los años 70 identificaran que la mitad de los pacientes crónicos no seguían su tratamiento 1 , la situación parece no haber cambiado demasiado y sugiere que los pacientes crónicos no se benefician de los avances farmacológicos y el desarrollo de los sistemas sanitarios 2. La OMS evidencia profundas carencias en el abordaje de los procesos crónicos, sobre todo en la promoción del autocuidado 3 y el problema adquiere cada vez mayor envergadura conforme aumenta la incidencia de las condiciones crónicas, como es el caso de España 4-6. La influencia de la cronicidad en la frecuentación de los servicios de urgencia hospitalarios (SUH) está bien documentada, así como su efecto deletéreo en la gestión de camas 7. De este modo, los SUH se convierten en unidades tampón que desarrollan una labor asistencial polivalente 8 , atendiendo no sólo a las necesidades de este segmento de pacientes, sino a las del resto del engranaje sanitario 9. Los pacientes crónicos son difíciles de manejar en cualquier escenario, máxime en los SUH, donde su complejidad y edad es superior a la de los que acuden al hospital por vía programada 10 y la falta de adherencia es una de las causas principales de reingreso 11 .

To evaluate the activation of the coagulation cascade through determinations of the thrombin-antithrombin complex in patients with atrial fibrillation and to correlate this data with the clinical and echocardiographic risk factors for systemic embolism. In 53 patients with atrial fibrillation plasma levels of the thrombin-antithrombin complex were determined on admission to a coronary care unit and 30 days later. Using a univariate and multiple regression analysis, the association basal thrombin-antithrombin with the duration of the arrhythmia, age over 70 years, previous use of antiplatelet agents, history of hypertension, mitral valve disease, diabetes, heart failure, previous systemic embolism, left atrial diameter and the presence of spontaneous contrast echo or thrombus in the left atrial appendage, was studied. An associative memory is a binary relationship between inputs and outputs, which is stored in an ... more An associative memory is a binary relationship between inputs and outputs, which is stored in an M matrix. In this paper, we propose a modification of the Steinbuch Lernmatrix model in order to process real-valued patterns, avoiding binarization processes and reducing computational burden. The proposed model is used in experiments with noisy environments, where the performance and efficiency of the memory is proven. A comparison between the proposed and the original model shows a good response and efficiency in the classification process of the new Lernmatrix.

Triggering AF by APC was related to wave break-up at the septopulmonary bundle near the RSPV ostium in 55% (lO/ZO), posterior roof of LA in 35% (7120). 2) Focally driven AF was shown in 22% (4/18): APCs not only initiated AF but also played a role in the maintenance of AF. During AF, these APCs were continuously generated from the arrhythmogenic foci and activated the LA. The wave propagation patterns of induced AF were different from those of spontaneous AF in 54.5% of patients. APC originated from single PV was in 45% (g/20), two PVs 15% (3/20) and more than three PVs 20% (4120). Non-PV foci were identified in 37.5% (6116) of the patients and mostly located at the posterior roof of the LA (12.5%, 2/16). Conclusion: A non-contact mapping system may facilitate identification of the mechanwns and the relationship with the underlying anatomvzzal structures responsible for the initiation and maintenance of AF.
