Unfortunately, the infant died five days after admission. After the patient’s death, the diagnosis was made based on pathohistological changes and a pharyngeal aspirate culture.

The disease began with coughing, diarrhoea, vomiting and weakness. Investigation revealed; electrolytes showin hyponatremia (110 mmol/L) and hypokalemic (2.6 mmol/L) and hypochloremic (63 mmol/L) metabolic alkalosis (HCO3=43 mmol/L). • Three patients had precocious puberty, one male with beta subunit secreting microadenoma, and 2... more • Three patients had precocious puberty, one male with beta subunit secreting microadenoma, and 2 females with non functioning pituitary micro adenomas. • Among two adolescents with normal stature and NFMA one have delayed puberty. Adolescent with prolonged gynecomastia was diagnosed, due to dynamic tests, as primary hypothyroid and with hyperprolactinaemia. • Also interesting results were found in 3 obese adolescent girls with secondary amenorrhea. • Although with normal basal hormonal status, they underwent meticulous dynamic investigation, because of presence of pituitary lesions, and we discovered primary hypothyroidism and hyperprolactinaemia in all of them. • Appropriate treatment led to shrinkage or complete radiological resolution of MAs.

Background: Transmission of tuberculosis (TB) is still occurring, especially from adult to childr... more Background: Transmission of tuberculosis (TB) is still occurring, especially from adult to children. Tuberculin survey in the province of West Sumatera in 2006 had established the prevalence of infection and Annual Rate of Tuberculosis Infection (ARTI) values. Within two years after that, TB might develop in children. Methods: A cross-sectional study was conducted in the districts of West Sumatera province, Indonesia. Subjects were all children with positive tuberculin test found two years previously. Data was collected on age, sex, BCG scar, and history of contact with TB sufferers, economic status, and nutritional status. The diagnosis of having TB disease was based on TB scoring table, prepared by the Pediatrics Pulmonologist Working Group, Indonesian Pediatrician Association. Result: Proportion of TB occurrence on elementary schoolchildren with positive tuberculin test was 27.4%. Among them, the proportion were higher at age eight years old (40.0%) and boys (28.0). In children without history of BCG immunization, proportion of TB was higher (35.4%) compared to those without BCG (26.7%). TB was more prevalent in children with poor nutrition (41.3% vs. 21.6%) and in children from poor families (30.8% vs. 24.5%). Risk of catching TB increased with the identification of contact with TB sufferer (OR 3.3), fever lasting more than two weeks (OR 7.328), cough for three weeks or more (OR 3.15), regional lymph node enlargement (OR 3.104) and with chest X-ray suggestive of TB (OR 7.328). Conclusion: Occurrence of TB disease in children two years after positive result of tuberculin test indicates the transmission of TB is still occurring in all age group of children of the population.

Functional diversity of prolactin action is responsible for different initial clinical expressions of hyperprolactinemia. Patients and Methods: We investigated causes of hyperprolactinemia in 11 children and adolescents (6 females and 5 males), aged from 1.5 to 17.5 years. Children with primary hypothyroidism, iatrogenic hyperprolactinemia and adolescents with polycystic ovaries were excluded. Results: Four patients had short stature or growth deceleration, the same number were clinically obese, 2 adolescent girls had secondary amenorrhea, 1 girl had premature thelarche and gynecomastia, and hypogonadism was the indication for the endocrinologic examination of two adolescent boys. Delayed pubertal development was present in both sexes. Hyperprolactinemia was also found in the youngest girl with multiple ovarian cysts. A diagnosis of acute tuberculous pleuropneumonia was made based on clinical and radiographical findings, andMycobacterium tuberculosiswas identified by Ziehl-Neelson acid-fast stain and culture on Löwenstein-Jensen medium. Twenty months before pneumonia onset, the patient had been treated with immunosupressive therapy (ALL IC-BFM 2002 protocol).

Immunotherapy is the only treatment for allergic diseases with the ability to change the natural course of the disease, thus stopping its further progression as well as the development of new allergic diseases and new sensibilizations. The objective of this chapter is to give insight into the latest data on immunotherapy in treating children with allergic diseases. Methods: The study involved a search for relevant articles on the MEDLINE and PubMed up to 2017. Results: Numerous studies have shown that the sublingual application of allergen-specific immunotherapy is adequate, safe, and efficient in the therapy of immunoglobulin E (IgE)-mediated allergic diseases of the respiratory tract in children, but there are still some questions to be solved concerning the usage of SLIT in children younger than 5 years old, SLIT for polysensitized patients, duration of SLIT, long-lasting effects of SLIT. Conclusions: In order to improve the clinical efficacy of SLIT, we are looking for new routes of administration, new allergens, new protocols as well as combination of SLIT with other immune modulatory treatments.

The majority of asthmatic patients are atopic - that is to say, they exhibit IgE-mediated sensiti... more The majority of asthmatic patients are atopic - that is to say, they exhibit IgE-mediated sensitivity to common inhalant allergens. Exposure and sensitisation to allergens from the housedust mite has been established as an important risk factor in asthma in most parts of the world. The aim of our study was to assess total IgE and specific IgE on airborne allergens (Dermatophagoides pteronyssinus - DP, Dermatophagoides farinae - DF, mould, and grass pollen) in relation to asthma severity. The study involved 64 children with atopic asthma. Statistical significance was present between groups of intermittent and moderately persistent asthma for IgE and specific IgE on DP and moulds. In mild and moderate asthma, statistically significant differences were present for all parameters except for DF.
