The incidence of CHD in developed and developing countries is varied, between 6 and 10 cases per 1000 live birth. Some factors contribute to the nutritional status of CHD patients, such as nutrient inputs, energy requirements, and dietary components. Irrespective of the nature of the cardiac defect and the presence or absence of cyanosis, malnutrition is a common finding in children with congenital heart anomalies. Recent studies have tried to investigate malnutrition development based on the type or category of CHD. Febrile seizures are common neurological disorders in pediatric patients and usually affect child... more Febrile seizures are common neurological disorders in pediatric patients and usually affect children between 6 months and five years, peaking in 14 to 18 months. Mothers have a vital role in ensuring their family’s well-being. However, they often cannot contribute significantly to febrile seizure prevention and treatment at home due to the lack of knowledge and awareness. Therefore, we need to provide a promotive and preventive program for febrile seizures to the mothers at Medan Barat District, Medan, Sumatera Utara. This activity aims to reduce the incidence of febrile seizures in Indonesia. We delivered the program in several stages, from basic data collection, lectures, febrile seizure prevention, early management at-home training, evaluation, and guidebook handover. Twenty-nine people attended this program, twenty women (72.4%) and nine men (27.6%). No significant change was found in the level of knowledge and behavior before and after the activity (Z-score: 0,001; p= 1,000).

Background: Sepsis is a systemic and syndromic response to infection and is a common final pathwa... more Background: Sepsis is a systemic and syndromic response to infection and is a common final pathway to death from many infectious diseases. Matrix metalloproteinase 9 (MMP-9) and Tissue inhibitors of matrix metalloproteinase 1 (TIMP-1) are potential sepsis biomarkers. Thiamine administration has been suggested to affect MMP-9 and TIMP-1 in several critical conditions, such as brain and lung tumors. This study investigated thiamine effects on MMP-9, TIMP-1, and their balance in sepsis patients. Method: We conducted a randomized experimental study to investigate the effects of thiamine administration on MMP-9, TIMP-1, and TIMP-1/MMP-9 ratio in sepsis patients. The study was conducted at Haji Adam Malik General Hospital and network hospitals in Medan, Sumatera Utara. The investigation lasted for six months, from April to September 2020. Results: We found that the MMP-9 values in the thiamine group and the control group were significantly different (p=0.042). The TIMP-1/MMP-9 ratio was also lower in the thiamine group after the administration, and the difference was significant (p = 0.033). Conclusion: Thiamine administration reduced the TIMP-1/MMP-9 ratio, which could be related to better survival. Thiamine could be considered as adjuvant therapy in patients with sepsis.

Despite there is no national data of Indonesia available currently, around 350,000 people in the United States and Canada experienced an arrest every year, and half of them were dead. The community knowledge and awareness about basic life support, particularly among mothers, is still low. Therefore, they cannot contribute effectively in providing first-aid to reduce the mortality. To improve this situation, we need an education and training program about basic life support for the PKK mothers and Posyandu cadres in West Medan to reduce the mortality rate of cardiac arrest in Indonesia. However, we still lack measures to reduce this high mortality rate. Fluid balance is known as a marker for mortality in ICU. If the balance of fluid becomes more positive, the mortality rate consequently becomes higher. Positive fluid balance elevates central venous pressure (CVP), while this elevation increases the risk of renal failure and mortality. Mean perfusion pressure (MPP) is the difference between mean arterial pressure and CVP. AIM: We propose that the MPP value can be used as an alternative indicator to monitor excessive fluid balance since its measurement is faster and more accurate than the manual 24 h record of fluid balance. Recently, RDW has emerged as a potential independent predictor of clinical outcomes in adults with sepsis. However, RDW as a mortality predictor in pediatric populations has not been well established.Objective To determine the relationship between RDW value and mortality outcomes in pediatric sepsis patients.Methods We performed a cross-sectional study of 40 consecutive pediatric patients with sepsis admitted to the PICU from December 2013 to March 2014. All patients’ RDW were collected within 24 hours of sepsis diagnosis. We determined the association between RDW and hemoglobin (Hb) using Spearman’s correlation.
