During the past ten years in the UK, education, housing, community regeneration and healthcare in... more During the past ten years in the UK, education, housing, community regeneration and healthcare in particular have witnessed a massive programme of capital investment by the government through Public Private Partnerships (PPP). There is now a need to asses and review the degree to which the original benefits have been identified and what have been the unanticipated benefits / impact. This will also ensure that future planning and policy setting can be adequately informed by evidence and a fuller appreciation of potential outcomes and impacts. This paper investigates the developments so far in Benefits Realisation Management. The paper focuses on the requirements to manage change and benefits in a joint approach to deliver the necessary outputs on time, to quality and cost without failing to realise the benefits of the change.

ealthcare buildings are designed to achieve diverse objectives. These range from providing approp... more ealthcare buildings are designed to achieve diverse objectives. These range from providing appropriate environments where care can be delivered to communities to increasing operational efficiency and improving patient flows and the patient experience. Intuitively, improvements in operating efficiency should result from state-of-the-art buildings, more appropriate layouts, departmental adjacencies, efficient clinical and business processes and enhanced information systems. However, complexities around requirements and stakeholders management may prevent the achievement of such objectives. The aim of this research is to identify and understand how healthcare services (re)design and building design can be integrated to facilitate increased performance both in terms of service delivery and future changes/flexibility. The paper presents results from a state of the art review, highlighting gaps in the linkage between service and building design. Findings demonstrate that current approaches and innovation are restricted due to functional barriers to design. Also, healthcare operations tend to be very much focused around the transformation view of production. Therefore, there is a need to support the development of operations driven design through time (e.g. flexible and durable) that satisfies diverse needs.

BeReal benefits realisation model integrated approach: The built environment lifecycle and organisational views

The overall performance measurement of a healthcare system should be related to benefits realisat... more The overall performance measurement of a healthcare system should be related to benefits realisation optimisation, looking for equilibrium between resource utilization (cost and time) and services provided (access and quality). The built environment should be seen, not only as context, but also as a resource that enables and potentially impacts on healthcare operations efficiency, influencing care and service efficacy. Based on an extensive literature review, focus group discussion, and case studies, this paper gives a perspective of the BeReal (benefits realisation) model structure and, describes a selected group of benefits which are used as preliminarily performance high-level targets (strategic benefits and sub benefits), organising the basis for further characterisation of detailed (end) benefits. In line with that, the scope of the BeReal model is discussed and characterised through a selected group of criteria (i.e. through the built environment lifecycle view, organisational view and other related dimensions). For some of these criteria a roadmap is proposed, emphasising on further research that could lead to fundamental improvements in the BeReal model, more precisely in benefits specification and segmentation, within the context of the UK healthcare sector.

Lean Knowledge Management: The Problem of Value

Lean knowledge management is defined here as: getting the right information, in the right form, t... more Lean knowledge management is defined here as: getting the right information, in the right form, to the right people at the right time. This definition highlights series of practical problems for knowledge management in the built environment which, in turn, have implications for lean theory. In the terms of TFV theory, the problems that arise from getting information to the right people at the right time are essentially flow (F) issues, but those that are concerned with defining the right information and the form in which it is to be delivered are more concerned with value (V). Here, we focus primarily on the problem of defining right information. A distinction is made between sociological 'values' and economic 'value', showing how both relate to production theory. In the course of benefits capture and realisation, both values and value are negotiated between project participants and other stakeholders. It is argued that these processes are best conceived as conversations and that this is implied in the basic formulation of V theory. The notion of objectivity and its significance for these values/value negotiations is examined. The problem of benefits realisation is considered and a set of hypotheses are generated regarding the nature of an effective benefits realization management process.

This is mainly due to the nature of regulatory requirements and the uniqueness of the design process. Past research identified that subjectivity in regulations is a key issue for automated code and rule checking. The aim of this paper is to discuss how requirements subjectivity could be addressed within building models through semantic enrichment, within the context of automated rule and code compliance checking. The paper presents preliminary findings of a research that follows the Design Science Research approach, framed within the UK healthcare design context. Findings suggest that part of the requirements subjectivity exists due to the implicit relationships between the elements of the healthcare built environment, which also include healthcare services. Fuzzy front-end of design in the NHS Manchester, Salford and Trafford (MaST) LIFT primary healthcare projects

This report sets out the findings of research into the development of MaST LIFT (Manchester Salfo... more This report sets out the findings of research into the development of MaST LIFT (Manchester Salford and Trafford) primary healthcare facilities, the aim of which was to understand the process and identify how the front-end of design was conducted, the skills and competencies used and where potential lessons could be learnt for in future developments. ? Data has been gathered through a series of 22 semi-structured interviews with key personnel involved in MaST, namely the public sector client, the LIFT team and the private sector partner. ? Particular focus was given to 4 schemes that had achieved financial close by 1st of July 2004. The interviews focused on eliciting the interviewees' personal background in terms of their educational and career history, their involvement in the front-end of the design process and their perspectives of the issues encountered in defining requirements and developing designs. ? The analysis in this report represents a snapshot in time, from the establishment of MaST LIFT until September 2004. ? The findings describe the organisational structure of MaST LIFT, a process map describing key activities on a timeline, a description of the main issues identified and possible improvements, and a description of the skills and competences used throughout the process. ? The main issues identified were classified as follows: (a) the healthcare delivery system redesign; ? There were some concerns that the service models were not sufficiently defined when design development started (b) the organisational structure; ? A complex organisational structure and multitude of stakeholders were put in place to deliver MaST LIFT, the expected economies of scale have not yet been achieved. There have also been constant changes of the stakeholders involved with the schemes, leading to continuous redesign. In addition, some tensions between stakeholders has occurred as a consequences of the complex MaST organisational structure. (c) the definition of processes; ? PCTs are considered to be novice construction clients. However, they were involved in the development of 13 schemes without appropriate technical support and without knowledge on the requirements capture and design issues. There is some indication that the schemes were being developed at their initial stages without appropriate project management structures to link the LIFT vision to its operational level. Design management is considered by the interviewees to have been generally poor. There were also some difficulties with regards to affordability and, in general, erratic management of change. All these issues are related to poor definition of processes. d) the definition of roles and responsibilities; ? The poor definition of processes has also led to a degree of confusion with regards to roles and responsibilities. There was lack of specific expertise that should have been available throughout the process (e.g. development), some roles and responsibilities were not clear for all involved in the process (e.g. MaST core team after the bidding process), and this has led to difficulties in terms of drilling down directives from the strategic leadership. ? Fourteen broad level skills, qualifications and experience were identified amongst the team. ? It was found that the complexity of the organisational structure coupled with the lack of common understanding of the process, roles and responsibilities exerted pressures on the 'as is' skills profile of the stakeholders involved. ? The report makes recommendations with regard to the use of skills audits and the skill needs for managing such initiatives.

There are several sources of evidence demonstrating that the physical environment impacts directl... more There are several sources of evidence demonstrating that the physical environment impacts directly and indirectly in our lives. Both positive and negative impacts of the physical environment have been observed in our health and wellbeing. Thus, to understand this relationship is crucial to prevent negative impacts and accelerate positive ones. In this regard, this paper presents a summary of the research findings of two research projects looking into the links between environment and wellbeing. The purpose of this paper is to present evidence about the impacts of the physical environment on wellbeing as well as trends related to the physical environment so that future scenarios can be explored. The research method used was an extensive literature review. Additionally, a workshop was organised in order to better understand the major trends and related drivers in the physical environment that could impact wellbeing over the next 20 years. Research findings demonstrate that there is a considerable amount of evidence linking the physical environment to wellbeing despite the lack of clarity in relation to cause-effect relationships. Moreover, 10 trends and drivers related to the physical environment were identified and considered as impacting on wellbeing.

A total of 1,163 abstracts were assessed, leading to 92 papers being reviewed.FindingsThere is a considerable amount of evidence linking healthcare environments to patients' health outcomes, despite the lack of clarity in relation to cause‐effect relationships.Originality/valueThe paper proposes a theoretical framework linking different built environment characteristics to health outcomes. This framework provides a structure to group causal effects according to their relation with design features, materials and ambient properties, art and aesthetic aspects and use of the built environment.
