Objective To explore the relationship among the high senstive C-reactive protein(hs-CRP),carotid ... more Objective To explore the relationship among the high senstive C-reactive protein(hs-CRP),carotid artery atherosclersis and the uigur aged patients with unstable angina pectoris(UP).Methods 56 patiens with unstable angina pectoris were UP group,the other 53 without coronary heart diaease were control gruup.The levels of hsCRP was measured by means of Enzyme-Linked-Immonosorbent-Assay(ELISA).Compared with the cartid intima-medio thinkness(IMT) and carotid plague score in two groups patiens by ultrasound.Their results were analyzed by correlate analysis.Results The levels of the hsCRP were significantly higher in UP group than in contral group(4.72±2.55mg/l vs 1.85±0.79mg/l).There were significant difference in intimin-media thickness(1.82±0.66mm vs 1.01±0.40mm) and the prevelence rate of atherosclerotic(85.7%vs35.8%) between the two groups.The thickness of IMT in the subjects with hs-CRP3mg/L(n=43)were 1.80±0.63mm,the prevelence rate of atherosclerotic were 83.7%,the prevelence rate o...

In this study, silver nanoparticles (AgNPs) were functionalized by various molecules, including s... more In this study, silver nanoparticles (AgNPs) were functionalized by various molecules, including sodium borohydride (NaBH4), polyhexamethylene biguanide hydrochloride (PHMB), and Tween 80 to investigate the long-term stabilization of AgNPs in an aqueous dispersion. PHMB-functionalized silver nanoparticles (AgNPs/PHMB) exhibited better stability than others and could be stored at ambient temperature for at least 180 days. In addition to creating stabilization based on the electrostatic repulsion, the use of PHMB helped to increase the degree of stability of the colloidal AgNPs for a long time owing to strong interactions between Ag atoms on AgNPs with nitrogen (N) positions in PHMB molecules. The formed bond led to improving maintenance ability of the electrostatic repulsion layer among independent nanoparticles. Ils comportent les oedèmes hémodynamiques (transfusion-associated circulatory overload [TACO]) et les oedèmes lésionnels (transfusion-related acute lung injury [TRALI]). Les TACO représentent actuellement une cause majeure de décès lors d'une transfusion en France. Les données d'hémovigilance sur le TRALI montrent qu'ils sont de plus en plus notifiés. Dans la série française, aucun TRALI n'est survenu après transfusion de plasma poolé SD. Un logigramme permettra de classer de manière plus reproductible les oedèmes pulmonaires dans la base e-fit.

Patients’ expectations of acute low back pain management: implications for evidence uptake

Background: In many countries, general practitioner (GP) care of acute low back pain often does n... more Background: In many countries, general practitioner (GP) care of acute low back pain often does not adhere to evidence-based clinical guidelines. There has been little exploration of this deviation from evidence-based care from the patients' perspective, particularly in relation to patients' care expectations. The aim of this study was to explore the care expectations in patients who present to their GP with acute low back pain, influences on expectation development, and congruence of these expectations with clinical guideline recommendations. Methods: Qualitative study in an inner urban general practice in Brisbane, Australia. Semi-structured interviews were conducted with 11 patients who presented to their GP with acute low back pain. Results: Patients had a biomechanical understanding of back pain, how it should be tested and treated, and a poor understanding of its natural history. Most expected x-rays, believing they were necessary to identify the "cause of the pain" without belief of any downsides to x-rays. Patients' expectations were primarily influenced by the experiences of family and friends, their own previous experiences of low back pain care, and comments from other health professionals they were consulting. The GP-patient relationship was important in influencing patient satisfaction of care provided. Most patient expectations, and some of the care that they reported receiving, were incongruent with guideline recommendations. Conclusions: A biomechanical approach to management rather than an awareness of empirical evidence was evident in patients' expectations. Communication and education by the GP that includes specifically enquiring about patients' expectations, provides an opportunity to correct misperceptions, explain acute low back pain natural history, and the rationale for test and treatment recommendations.

Un incident transfusionnel de grade zéro se définit comme la transfusion inappropriée d'un produi... more Un incident transfusionnel de grade zéro se définit comme la transfusion inappropriée d'un produit sanguin labile consécutive à un ou plusieurs dysfonctionnements, sans conséquence immédiate clinique ou biologique chez le receveur. Deux ans après la mise en place de la déclaration obligatoire de ces incidents, une première analyse descriptive nationale des données disponibles à l'unité hémovigilance de l'Afssaps a été réalisée sur les années 2003 et 2004. Cette analyse a été effectuée à partir d'une part, de la base de données nationale informatique e-fit et d'autre part, des documents d'analyse et compléments d'enquête rédigés par les acteurs du réseau. D'un point de vue quantitatif, cette étude montre des différences de notification et d'analyse de ces incidents d'une région à une autre. Au plan qualitatif, 45 % des incidents de grade zéro correspondent à des erreurs d'attribution. Le lieu d'origine des grades zéro est pour près de 73 % des cas l'établissement de santé, le service clinique ou le dépôt de sang, et pour près de 23 % des cas l'établissement de transfusion sanguine. L'analyse détaillée a notamment montré que les dysfonctionnements survenus dès la prescription de produits sanguins labiles étaient retrouvés dans 9 % des cas. Cette analyse descriptive, qui a permis notamment d'identifier les dysfonctionnements récurrents et les points critiques à l'origine de transfusions inappropriées, devrait constituer le point de départ d'une réflexion visant à optimiser et à standardiser les méthodes d'analyse des grades zéro et à élaborer des propositions pour mieux maîtriser les points critiques.
