Its coexistence with pregnancy is exceptional and laparoscopic removal has rarely been reported. We describe the case of a 34-year-old woman with multiple endocrine neoplasia type 2a (MEN 2a) with adrenal pheochromocytoma diagnosed in the 6th week of pregnancy. After pretreatment with phenoxybenzamine, a successful transperitoneal laparoscopic adrenalectomy was performed in the twentieth week of gestation. The management of pheochromocytoma in pregnancy and the indications for laparoscopic surgery in pregnant patients are discussed.

ABSTRACT Gestational primary hyperparathyroidism presents with features which, from a physiologic... more ABSTRACT Gestational primary hyperparathyroidism presents with features which, from a physiological and prognostic viewpoint, entail great difficulty in diagnosis and a high risk of complications. These complications occur at rates of 67% and 80% in the mother and fetus, respectively, and can be reduced by up to four times by means of prompt application of effective therapeutic measures. We report a case involving a pregnant woman in the 16th week of gestation who presented with asymptomatic hypercalcemia secondary to a solitary parathyroid adenoma. When the patient did not improve after conservative therapeutic measures, it was decided to employ surgical treatment in the second trimester of pregnancy. The surgery was successful, and the follow-up period was without complications for the mother and neonate. We therefore agree with the growing evidence that surgery in the second trimester of pregnancy constitutes a safe and effective alternative to conservative treatment.

Our aim was to describe our cohort of T1D patients and identify potential risk factors susceptible to prevention strategies. Methods: Cross-sectional, observational study, including T1D patients treated at our center, from 1 March 2017 to 31 March 2020. Inclusion criteria: T1D, age > 14 years and signed informed consent. Exclusion criteria: diabetes other than T1D, age < 14 years and/or refusal to participate. Results: Study population n = 2181 (49.8% females, median age at enrollment 41 years, median HbA1c 7.7%; 38.24% had at least one comorbidity). Roughly 7.45% had severe hypoglycemia (SH) within the prior year. Macro/microvascular complications were present in 42.09% (5.83% and 41.14%, respectively). This paper presents a sensorless DSVM-DTC of an induction motor that propels an electrical vehicl... more This paper presents a sensorless DSVM-DTC of an induction motor that propels an electrical vehicle or a hybrid one. The drive uses an adaptive flux observer for speed estimation and a discrete space vector modulation direct torque control (DSVM-DTC) technique for torque and stator flux control. The adaptive flux observer uses a mechanical model to improve the behavior during speed transients. The estimated stator flux of the adaptive observer is used in the DSVM-DTC method to provide fast torque response combined with torque ripple free operation over the whole speed range. The sensorless drive system is capable of working from very low speed to high speed and exhibits good dynamic and steady-state performance. The combination of the adaptive observer and DSVM-DTC are quite effective in reducing the switching loss and torque ripples of the motor, as demonstrated in experimental results.

Erectile dysfunction (ED) is a sign of vascular disease in type 2 diabetic patients. The present ... more Erectile dysfunction (ED) is a sign of vascular disease in type 2 diabetic patients. The present subanalysis of the DIVA Registry, whose main objective was to estimate the prevalence of clinical vascular disorder and silent vascular disorder, as well as risk factors in type 2 diabetic patients treated in Spain, aims to analyze the relationship between those data and the prevalence of ED in these patients. A total of 2444 type 2 diabetic patients (56% male; mean age 65.2 years) attended by 387 cardiologists and endocrinologists at ambulatory care were included. Coronary heart disease was present in 37% of the patients, cerebrovascular disease in 12%, and peripheral arterial disease in 13%. Forty percent of male patients had ED (according to the IIEF criteria), although in this group, as compared to those patients without ED, the prevalence of cardiovascular disease and signs of subclinical vascular disorder (microalbuminuria and abnormal ankle/brachial index (ABI)) was higher. The only independent predictor of ED was left ventricular hypertrophy (OR 5.2; 95% CI: 1.1-24.1; P=.03), with the ABI &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt;0,9 being of borderline significance (OR 5.9; 95% CI: 0.9-39.9; P=.06). Forty percent of diabetic patients suffer from ED. The results of this study suggest that ED may be considered as an atherosclerosis marker and could be included in algorithms for risk stratification and subclinical vascular disorder detection.

30 pacientes (15 hembras y 15 varones) fueron intervenidos de adenoma hipofisario no funcionante ... more 30 pacientes (15 hembras y 15 varones) fueron intervenidos de adenoma hipofisario no funcionante (1 grado i, 3 grado ii, 15 grado iii, 11 grado iv) de los que ademas 19 fueron tratados con radioterapia postoperatoria. Los sintomas iniciales fueron debidos a efecto masa (cefaleas, trastosnos visuales, hipopituitarismo, etc.), mas frecuentes y severos entre los varones. Los deficits hormonales, frecuentes en el preoperatorio (27 de gh, 7 de acth, 2 de tsh, 14 de lh-fsh), aumentaron en el postoperatorio (28 de gh, 14 de acth, 12 de tsh, 15 de lh-fsh). Por contra, las alteraciones visuales mejoraron despues de la intervencion. La hiperprolactinemia, encontrada en 9 pacientes, se corrigio despues de la intervencion. 7 pacientes fueron tratados preoperatoriamente con bromocriptina depot (50 mgrs. Im.), normalizando dos de ellos su funcion visual, mejorando otros dos, mientras que los tres restantes no sufrieron cambios. En ninguno de ellos se objetivo disminucion tumoral. De 26 tumores es...

Introduction: Sodium-glucose cotransporter 2(SGLT2)-inhibitors are new antihyperglycemic agents t... more Introduction: Sodium-glucose cotransporter 2(SGLT2)-inhibitors are new antihyperglycemic agents that have shown a reduction in cardiovascular events in type 2 diabetes mellitus. Recent warnings have been developed about an increased risk of euglycemic and moderate hyperglycemic diabetic ketoacidosis with the use of SGLT2 inhibitors, but its real incidence is not available yet. Case Report: We present a case of DKA with moderate hyperglycemia in a patient treated with metformin and empagliflozin. Conclusion: DKA in patients treated with SGLT2 inhibitors can be presented as euglycemic and moderated hyperglycemia. This special presentation poses a physician’s challenge.
