The Society of Radiologists in Ultrasound convened a panel of specialists from gynecology, radiol... more The Society of Radiologists in Ultrasound convened a panel of specialists from gynecology, radiology, and pathology to arrive at a consensus regarding the management of ovarian and other adnexal cysts imaged sonographically in asymptomatic women. The panel met in Chicago, Ill, on October 27-28, 2009, and drafted this consensus statement. The recommendations in this statement are based on analysis of current literature and common practice strategies, and are thought to represent a reasonable approach to asymptomatic ovarian and other adnexal cysts imaged at ultrasonography.

Informing Resource-Poor Populations and the Delivery of Entitled Health and Social Services in Rural India

Context A lack of awareness about entitled health and social services may contribute to poor deli... more Context A lack of awareness about entitled health and social services may contribute to poor delivery of such services in developing countries, especially among individuals of low socioeconomic status. Objective To determine the impact of informing resource-poor rural populations about entitled services. Community-based, cluster randomized controlled trial conducted from May 2004 to May 2005 in 105 randomly selected village clusters in Uttar Pradesh state in India. Households (548 intervention and 497 control) were selected by a systematic sampling design, including both low-caste and midto high-caste households. Intervention Four to 6 public meetings were held in each intervention village cluster to disseminate information on entitled health services, entitled education services, and village governance requirements. No intervention took place in control village clusters. Main Outcome Measures Visits by nurse midwife; prenatal examinations, tetanus vaccinations, and prenatal supplements received by pregnant women; vaccinations received by infants; excess school fees charged; occurrence of village council meetings; and development work in villages. At baseline, there were no significant differences in self-reported delivery of health and social services. After 1 year, intervention villagers reported better delivery of several services compared with control villagers: in a multivariate analysis, 30% more prenatal examinations (95% confidence interval [CI], 17%-43%; PϽ.001), 27% more tetanus vaccinations (95% CI, 12%-41%; PϽ.001), 24% more prenatal supplements (95% CI, 8%-39%; P=.003), 25% more infant vaccinations (95% CI, 8%-42%; P=.004), and decreased excess school fees of 8 rupees (95% CI, 4-13 rupees; PϽ.001). In a difference-in-differences analysis, 21% more village council meetings were reported (95% CI, 5%-36%; P=.01). There were no improvements in visits by a nurse midwife or in development work in the villages. Both low-caste and mid-to high-caste intervention households reported significant improvements in service delivery. Conclusions Informing resource-poor rural populations in India about entitled services enhanced the delivery of health and social services among both low-and midto high-caste households. Interventions that emphasize educating resource-poor populations about entitled services may improve the delivery of such services.

Most patients with Barrett&amp;amp;amp;amp;#39;s esophagus do not progress to cancer, but tho... more Most patients with Barrett&amp;amp;amp;amp;#39;s esophagus do not progress to cancer, but those who do seem to have markedly increased survival when cancers are detected at an early stage. Most surveillance programs are based on histological assessment of dysplasia, but dysplasia is subject to observer variation and transient diagnoses of dysplasia increase the cost of medical care. We have previously validated flow cytometric increased 4N fractions and aneuploidy as predictors of progression to cancer in Barrett&amp;amp;amp;amp;#39;s esophagus. However, multiple somatic genetic lesions develop during neoplastic progression in Barrett&amp;amp;amp;amp;#39;s esophagus, and it is likely that a panel of objective biomarkers will be required to manage the cancer risk optimally. We prospectively evaluated endoscopic biopsies from 325 patients with Barrett&amp;amp;amp;amp;#39;s esophagus, 269 of whom had one or more follow-up endoscopies, by a robust platform for loss of heterozygosity (LOH) analysis, using baseline 17p (p53) LOH as a predictor and increased 4N, aneuploidy, high-grade dysplasia, and esophageal adenocarcinoma as outcomes. The prevalence of 17p (p53) LOH at baseline increased from 6% in negative for dysplasia to 57% in high-grade dysplasia (p &amp;amp;amp;amp;lt; 0.001). Patients with 17p (p53) LOH had increased rates of progression to cancer (relative risk [RR] = 16, p &amp;amp;amp;amp;lt; 0.001), high-grade dysplasia (RR = 3.6, p = 0.02), increased 4N (RR = 6.1, p &amp;amp;amp;amp;lt; 0.001), and aneuploidy (RR = 7.5, p &amp;amp;amp;amp;lt; 0.001). Patients with 17p (p53) LOH are at increased risk for progression to esophageal adenocarcinoma as well as high-grade dysplasia, increased 4N, and aneuploidy. 17p (p53) LOH is a predictor of progression in Barrett&amp;amp;amp;amp;#39;s esophagus that can be combined with a panel of other validated biomarkers for risk assessment as well as intermediate endpoints in prevention trials.

Prompted by the confirmation of KRAS mutation as a predictive biomarker of response to epidermal growth factor receptor (EGFR)–targeted therapies, limited clinical testing for RAS pathway mutations has recently been adopted. We performed a multiplatform genomic analysis to characterize, in a nonbiased manner, the biological, biochemical, and prognostic significance of Ras pathway alterations in colorectal tumors and other solid tumor malignancies. Mutations in exon 4 of KRAS were found to occur commonly and to predict for a more favorable clinical outcome in patients with colorectal cancer. Exon 4 KRAS mutations, all of which were identified at amino acid residues K117 and A146, were associated with lower levels of GTP-bound RAS in isogenic models. To replicate these findings, we did a case-control association analysis on 6q22.33 (rs2180341) in an additional 487 Ashkenazi Jewish breast cancer cases and in an independent non-Jewish, predominantly European American, population of 1,466 breast cancer cases and 1,467 controls. We confirmed the 6q22.33 association with breast cancer risk in the replication cohorts [per-allele odds ratio (OR), 1.18; 95% confidence interval (95% CI), 1.04-1.33; P = 0.0083], with the strongest effect in the aggregate meta-analysis of 3,039 breast cancer cases and 2,616 Ashkenazi Jewish and non-Jewish controls (per-allele OR, 1.24; 95% CI, 1.13-1.36; P = 3.85 × 10-7). Purpose With the availability of ultra-sensitive PSA assays, early biochemical relapse (eBCR) of ... more Purpose With the availability of ultra-sensitive PSA assays, early biochemical relapse (eBCR) of prostate cancer is increasingly being detected at values much lower than the conventional threshold of 0.2 ng/ml. Accurate localisation of disease in this setting may allow treatment modification and improved outcomes, especially in patients with pelvis-confined or extra-pelvic oligometastasis (defined as up to three pelvic nodal or distant sites). We aimed to measure the detection rate of [68]Ga-PSMA-HBNED-CC (PSMA)-PET/CT and its influence on patient management in eBCR of prostate cancer following radical prostatectomy (RP). Methods We retrospectively identified 28 patients who underwent PSMA-PET/CT for post-RP eBCR (PSA < 0.5 ng/ml) at our tertiary care cancer centre. Two nuclear medicine physicians independently recorded the sites of PSMA-PET/CT positivity. Multidisciplinary meeting records were accessed to determine changes in management decisions following PSMA-PET/CT scans. Results The mean age of patients was 65.6 years (range: 50-76.2 years); median PSA was 0.22 ng/ml (interquartile range: 0.15 ng/ml to 0.34 ng/ml). Thirteen patients (46.4%) had received radiotherapy in the past. PSMA-PET/CT was positive in 17 patients (60.7%). Only one patient had polymetastasis (> 3 sites); the remainder either had prostatectomy bed recurrence (n = 2), pelvic oligometastasis (n = 10), or extra-pelvic oligometastasis (n = 4). PSMA-PET/CT resulted in management change in 12 patients (42.8%), involving stereotactic body radiotherapy (n = 6), salvage radiotherapy (n = 4), and systemic treatment (n = 2). Conclusions Our findings show that PSMA-PET/CT has a high detection rate in the eBCR setting following RP, with a large proportion of patients found to have fewer than three lesions. PSMA-PET/CT may be of value in patients with early PSA failure, and impact on the choice of potentially curative salvage treatments.

Frequency and cause of disagreements in imaging diagnosis in children with ventriculomegaly diagnosed prenatally

Purpose-To assess the frequency and etiology of variability in diagnoses on cranial ultrasound (U... more Purpose-To assess the frequency and etiology of variability in diagnoses on cranial ultrasound (US) and magnetic resonance (MR) imaging for children referred for prenatally diagnosed ventriculomegaly (VM). Materials and Methods-Between 9/19/03-3/16/07, 119 children with US and MR studies performed within 13 months (median 6 days) after birth, after prenatal referral for VM, were studied as part of a prospective IRB-approved HIPAA-compliant study with written parental consent. 3 sonologists and 3 pediatric neuroradiologists interpreted the US and MR examinations, blinded to prenatal diagnosis. Final diagnosis was obtained by consensus (97 US, 53 MR and 31 US/MR comparisons). Ventricular size, types of disagreements, and reasons for disagreements were recorded. Disagreements on a per patient basis were categorized as major when they crossed diagnostic categories and had potential to change patient counseling. Results-There was prospective agreement on 42/97 (43%) US and on 9/53 (17%) MR readings. Prospective consensus was more likely when the number of CNS anomalies was lower (P<.001 and =.002 for US and MR, respectively). In 24/55 (44%) of US and 11/44 (25%) MR with disagreements, one of the disagreements concerned the presence of VM. In 22/97 (23%) US studies and 22/53 (42%) MR studies the disagreements were potentially important. Reasons for discrepancies in reporting of major findings included errors of observation as well as modality differences in depiction of abnormalities. In comparing prenatal to postnatal diagnoses, there were 11/97 (11%) US and 27/53 (51%) MR examinations with newly-detected major findings, the most common being migrational abnormalities, callosal dysgenesis/destruction and interval development of hemorrhage. diagnosis is common after a prenatal diagnosis of VM.
