Organizations and investigators are alike in searching for ways to improve delivery and safety of patient care. Many have reported that the existence of a patient safety and risk management system will have an effective impact on the overall patients' outcomes. Aims: To study the effectiveness of a patient safety model on patient safety indicators when implemented in a university hospital. Methods: A task force constituted by various patient safety experts was established to design a practical concept of patient safety management based on a nine steps model and applied by all hospital departments. Patient safety indicators (780) were monitored over a four years period and the model's effectiveness was analyzed on 40 selected indicators. Results: A statistical significant improvement by 67.5% (27/40) of initially measured patient safety indicators was evidenced mainly in the domains of peri-operative mortalities, neonatal mortality, return to surgeries, healthcare associated infections, safety and medication use, blood transfusion reactions, cardio pulmonary resuscitations, patient adverse events, and occurrence variance reporting. However, 12.5% (5/40) of the indicators of hospital standardized mortality and specific mortality were not improved by the model's implementation while others, 20% (8/40) of the patients safety indicators were maintained as their initial baselines were satisfactory. Conclusions: The implementation of a patient safety management model was found to be effective in improving patient safety practices (PSP) as well as patient safety indicators (PSIs) and finally patient outcomes.

Relationship between staff satisfaction, productivity and patient satisfaction: a study in physical rehabilitation services
Patient and staff satisfaction are considered as important indicators to monitor quality in healthcare. In rehabilitation services the patient and therapist encounters are of significant value by the amount of time, the therapeutic approach but also the personal relations in the care process. The aim of the research was to evaluate the staff job satisfaction in physical rehabilitation services and the related variables as well as its correlations to patients’ satisfaction. A cross sectional survey approach in the rehabilitation services has measured the job satisfaction and the related elements of productivity. Simultaneously a survey of the patients’ satisfaction was performed. As the research in the physical rehabilitation services was conducted in ten different health care facilities of the Jeddah region, a common assessment tool was utilized based on the Effort Reward Imbalance (ERI) model and complementary information were collected using specifically developed survey questionnaires. Socio-demographic variables such as age, nationality, work specialty, educational levels, as well as the caseload and workload appeared also as significantly influencing job satisfaction. The findings confirmed a positive correlation between the Effort Reward Imbalance and the quantitative performance (productivity) of the rehabilitation staffs. It is supported by the level of job satisfaction of the therapists which is related to the number of patients per day, caseload, hours of work and high productivity measures. From the aspects of positive rewards, the role of the supervisor and doctors appear as important motivators. It was also found that therapists valued the fact of being given the opportunity to participate and discuss opinions in the patient management and quality improvement. While no evidence confirmed a positive correlation between…

Quality & Safety in Health Care, Feb 1, 2009

Europeans have always been free to travel to receive care abroad, but if they wished to benefit from their statutory social protection scheme, they were subject to their local or national legislation on social protection. This changed in 1991 with the European Court of Justice defining healthcare as a service, starting a debate on the right balance between different principles in European treaties: movement of persons, goods and services, versus the responsibility of member states to organise their healthcare systems. Simultaneously, crossborder cooperation has developed between member states. In this context, patient mobility has become a relevant issue on the EU's agenda. The EU funded a number of Scientific Support to Policies (SSP) activities within the Sixth Framework Programme, to provide the evidence needed by EU policy makers to deal with issues that European citizens face due to enhanced mobility in Europe. One SSP project ''Methods of Assessing Response to Quality Improvement Strategies'' (MARQuIS), focused on cross-border care. It aimed to assess the value of different quality strategies, and to provide information needed when: (1) countries contract care for patients moving across borders; and (2) individual hospitals review the design of their quality strategies. This article describes the European context related to healthcare, and its implications for cross-border healthcare in Europe. The background information demonstrates a need for further research and development in this area.

The objectives will be attained integrating these changes into the two first steps of the evaluation: the elaboration of norms and the measuring, including structure and process and mentioning the outcome. Proposals as how to act are presented; different actors have to play their own role: the nursing profession, the nursing unit, the nursing department, the executive board of hospitals, the legal and representative authorities.

They are reluctant to provide it as they dread promoting sexual activity and worse, an unbridled sort of sexuality. Sex education was introduced in schools less than five decades ago, for political and public health reasons. First limited as a whole to "no sex" or "safer sex" and often delivered in an excessively conventional way. More recently, other topics relevant have begun to be added, such as incest, gender identity, sexual orientation, consent relationship, interpersonal violence, and LGBTQIA+. At a time when social networks and online pornography have become young people's main information sources on sexuality, sexual education is more essential than ever. Clearly, it is high time we had one approach to sex education, serving emotions, the body, human relationships as well as erotic art and ethics. Programs using interactive learning and skill building are essential in engaging young people with the knowledge and tools required for healthy sexual decision-making. Modern implementation strategies of communication, including digital and gaming, are necessary to address young people in a positive way.

For that reason the authors set out to clarify the terms and concepts used in a nursing audit and to translate the notions of productivity, efficacy, efficiency and performance into nursing terms. They propose an assessment system which integrates the demands of management (performance concepts) with the professional nursing requirements for quality of care. The final chapter presents recommendations for norm development as formulated by the WHO's nursing department.

Definitions are given for basic terms such as reliability, validity and sensitivity of a method. The principal problems that have to be dealt with concern a problem of vocabulary, limits to nursing care (definition), unit of analysis, criteria used, standards to be attained, population for study, observer reliability and sources of information. A brief discussion of each of these is given, in all instances a rigorous adherence to protocols is required to ascertain outcome quality.

Topics covered have ranged from "no sex" to "safe sex", and there has been a tendency for pupils to drift into the "alternative" areas of pornographic media. This approach may have contributed to generations of women and men finding themselves, in adult life, living with unresolved sexual problems caused by a truncated and disorderly "education". The arrival of robots presents an opportunity to reconsider the way in which sexuality is taught and to introduce an innovative and ethical educational practice. This article proposes a conceptual framework where there can be an extension of medical simulation toward sex education based on advanced sex robots, offering a dynamic, effective and ethical teaching method. This article considers why, how and where as well as what is workable and technically feasible. The target is that, with the help of a new generation of robots, a fulfilling love life will be accessible to every man and woman. The engagement of both, the simulation community itself and the political will, is vital to ensure that a more enlightened and emotionally intelligent sex education can become a reality in the near future.
