Diagnoza ADHD by měla být zvažovana i v připadě, pokud nebyla diagnostikovana již v dětstvi, důležita je perzistence symptomů a jimi způsobene funkcni postiženi. Ve farmakoterapii ADHD v dospělosti maji nejlepe prokazanou ucinnost se stejnou silou důkazů stimulancia a atomoxetin. Atomoxetin je jediným preparatem se schvalenou indikaci k lecbě ADHD v dospělosti v CR. Tento text přinasi přehlednou informaci o diagnostice a psychofarmakoterapii ADHD v dospělosti.

Vortioxetine: a review of new antidepressant

Vortioxetin je nové antidepresivum s tzv. multimodálním mechanizmem účinku, působí jako antagonis... more Vortioxetin je nové antidepresivum s tzv. multimodálním mechanizmem účinku, působí jako antagonista 5-HT 3A , 5-HT 7 a 5-HT 1D receptorů, agonista 5-HT 1A , parciální agonista 5-HT 1B receptorů a inhibuje serotoninový transportér. Antidepresivní účinnost vortioxetinu v dávce 10-20 mg prokázalo celkem 13 krátkodobých a 1 dlouhodobá kontrolovaná studie. Ve 3 placebem kontrolovaných studiích vortioxetin také pozitivně ovlivnil kognitivní funkce, a to nezávisle na zlepšení depresivních příznaků. Vortioxetin je dobře tolerován, s nízkou incidencí sexuálních dysfunkcí. Nejčastějším nežádoucím účinkem je přechodná a na dávce závislá nauzea a zvracení. Vortioxetin je v ČR schválen v indikaci léčby depresivní poruchy u dospělých. Úvodní a doporučená terapeutická dávka vortioxetinu je 10 mg jednou denně, dávkovací rozmezí je dle odpovědi, snášenlivosti a věku 5-20 mg denně. Vortioxetine: a review of new antidepressant Vortioxetine is a novel antidepressant with multimodal mechanism of action, acting as 5-HT 3A , 5-HT 7 , and 5-HT 1D receptor antagonist, 5-HT 1A agonist, 5-HT 1B partial agonist, and serotonin transporter inhibitor. Antidepressant efficacy of vortioxetine 10-20 mg daily has been shown in the total of 13 acute short-term and 1 relapse-prevention controlled trials. Moreover, in 3 placebo-controlled trials vortioxetine improved cognitive functions, independent of antidepressive effects. Vortioxetine was well-tolerated, with low incidence of sexual dysfunction. Most frequently reported side effects were transient and dose-dependent nausea and vomiting. Initial and recommended therapeutic dose is 10 mg of vortioxetine once a day, the dosing range, according to response, tolerability, and age, is 5-20 mg daily.

Poporodní období je u žen spojeno se zvýšeným rizikem rozvoje akutních psychiatrických stavů, kte... more Poporodní období je u žen spojeno se zvýšeným rizikem rozvoje akutních psychiatrických stavů, které vyžadují intenzivní péči, vedou k dlouhodobé separaci matky od dítěte a narušují celý rodinný systém. Nejčastější příčinou relapsu schizofrenie i bipolární afektivní poruchy po porodu je vysazení medikace během těhotenství. Poporodní psychóza, která bývá prvním klinickým projevem bipolární afektivní poruchy, je onemocněním s nejasnou etiologií. Zdá se, že vztah laktace a poporodní psychózy není kauzální a preventivní farmakologická zástava laktace není indikovaná. Léčbu bipolární afektivní poruchy a schizofrenie se snažíme, pokud je to možné, upravit s přihlédnutím k plánovanému těhotenství a porodu. Důležitá je farmakologická profylaxe poporodní psychózy. Pokud to stav matky umožňuje, podporujeme vzhledem k benefitům pro matku i dítě její rozhodnutí kojit a volíme léky s relativně bezpečným profilem v laktaci. Dle aktuálních doporučení antipsychotiky slučitelnými s kojením jsou olanzapin a kvetiapin. Z antikonvulziv v indikaci thymoprofylaktik to jsou valproát, karbamazepin a lamotrigin. Lithium se dle současných doporučení v kojení nedoporučuje. Klíčová slova: poporodní psychóza, bipolární afektivní porucha, psychofarmaka, laktace, kojení. Part Two: psychotic disorders and bipolar disorder Postpartum period is accompanied by an increased risk of onset of severe mental disorders that require intensive psychiatric care and may result in long-term separation of mother and baby. This may disrupt the whole family system. The most frequent cause of postpartum relapse of schizophrenia or bipolar disorder is discontinuation of drug treatment during pregnancy. The etiology of postpartum psychosis, which is often a first manifestation of bipolar disorder, is still unknown. There is no evidence of a causal link between lactation and postpartum psychosis. Thus, pharmacological suppression or cessation of lactation is not indicated for postpartum psychosis prevention. If possible, pharmacotherapy of bipolar disorder or schizophrenia should be adjusted with respect to planned pregnancy and delivery. Prophylactic drug management of postpartum psychosis is essential. Breastfeeding is beneficial for both mother and child, therefore we encourage women to breastfeed their infant, depending on their health condition. Psychotropic drugs with safe profile in lactation should be chosen. According to the current guidelines, antipsychotics olanzapine and quetiapine, and mood stabilizing anticonvulsants, valproate, carbamazepine and lamotrigine, are relatively safe in lactation. Lithium is not recommended during breastfeeding.

Psychosociální změny u matky a otce po narození dítěte, spojené s adaptací na novou rodičovskou r... more Psychosociální změny u matky a otce po narození dítěte, spojené s adaptací na novou rodičovskou roli, jsou rizikovým faktorem pro exacerbaci či nový vznik duševních onemocnění. Raná vztahová vazba rodič-dítě je důležitá pro nenarušený neurobehaviorální vývoj dítěte. Důležitou roli v těchto procesech hraje kojení. Přítomnost dekompenzovaného duševního onemocnění u rodiče proces budování citové vazby narušuje. Proto je zásadní včasná a účinná terapeutická intervence. Při léčbě matky vybíráme farmakum s ohledem na jeho bezpečnostní profil v laktaci tak, aby terapie nemusela být přerušena. Údaje o bezpečnosti psychofarmak pocházejí z malých observačních studií či jejich meta-analýz. U většiny léků není jejich bezpečnostní profil znám a jen jej odhadujeme. Aktuální doporučení uvádějí jako relativně bezpečná antidepresiva v laktaci sertralin, paroxetin a nortiptylin; jejich užívání je slučitelné s kojením. Zdá se, že benzodiazepiny a Z-hypnotika přecházejí do mateřského mléka minimálně a jako nejbezpečnější se jeví oxazepam, alprazolam a zolpidem. Metylfenidát užívaný v terapii ADHD je rovněž považován za slučitelný s kojením.

In recent years, there has been an increase in the number of studies in patients with schizophrenia and their relatives searching for endophenotypes of the disease. The aim of our study was to investigate cognitive performance and cognitive subtypes in the siblings of the patients. Methods: Four groups of subjects were included: patients with a first episode of psychotic illness, the siblings of these patients, and two control groups. All the study subjects (N = 84) had a battery of neuropsychological tests that measured basic cognitive domainsmemory, executive functions, attention, visual-spatial skills, language skills and psychomotor speedadministered to them. The data were assessed with pairwise t-tests for group comparisons. The siblings were distributed into three groups according to their cognitive performance: non-deficit, partial deficit, and global deficit. Subsequently, the patients were assigned into three groups corresponding to their siblings' performance. Results: Our results revealed attenuation of abstract thinking in the siblings compared to the controls. As expected, the patients showed impairment across all cognitive domains. The patients and siblings demonstrated similar profiles in each subtype, in the severity of their impairment, and in their patterns of cognitive performance. Conclusions: Our results suggest that the cognitive profile can be considered as an endophenotype of psychotic disorders.

Většina psychofarmak přechází přes placentu do oběhu plodu, a tak mohou ovlivňovat jeho vývoj. Nicméně i neléčené duševní onemocnění v těhotenství představuje rizikový faktor pro narušení průběhu gravidity, vývoje plodu a horší poporodní adaptaci novorozence. V přehledu shrnujeme aktuální publikované údaje o efektu neléčené bipolární afektivní poruchy a užívání stabilizátorů nálady i neléčených úzkostných poruch a užívání anxiolytik na průběh těhotenství, vývoj plodu a časnou poporodní adaptaci novorozence. Dle dostupných studií mají některé stabilizátory nálady jasně prokázaný teratogenní efekt a neměly by být v těhotenství předepisovány. Avšak například u lithia byl názor na jeho negativní efekt na plod revidován. Rizika pro plod, která s sebou nese neléčená bipolární afektivní porucha v graviditě, se zdají být vysoká. Ošetřující psychiatr by měl po dobu těhotenství pacientku léčit pomocí lithia či některých nových (atypických) antipsychotik. Anxiolytika jako skupina nemají jasně prokázaný teratogenní efekt a jejich užívání v těhotenství je relativně bezpečné, ale rizika pro plod se zvyšují s rostoucí užívanou dávkou. Užívání benzodiazepinů v třetím trimestru gravidity s sebou nese zvýšené riziko pro komplikace časné poporodní adaptace dítěte. Farmakoterapie je namístě u těžkých případů, kde rizika neléčeného onemocnění pro narušení vývoje plodu a hladkého průběhu těhotenství převyšují rizika spojená s expozicí lékům.

Background: Neuroactive steroids (NAS) affect neurotransmitter systems and cognition; thus, they ... more Background: Neuroactive steroids (NAS) affect neurotransmitter systems and cognition; thus, they play role in etiopathogenesis of psychiatric disorders. Aims: The primary aim was to examine cognition and effects of NAS on cognitive functioning in first-episode psychosis patients and in their healthy siblings. The secondary aims were to verify whether cognitive deficit is an endophenotype of psychosis and whether higher NAS levels represent a high-risk factor for psychosis. Methods: Studied participants were 1) patients with first episode of psychosis, 2) healthy siblings of the patients, and 3) matching healthy controls. Study procedures included administration of a battery of neuropsychological tests assessing six cognitive domains and examination of NAS plasma levels [cortisol (CORT), 11-deoxycorticosterone (DOC), testosterone (TEST), dehydroepiandrostendione (DHEA), dihydrotestosterone (DHT), and progesterone (PROG)]. Results: A total of 67 subjects were analyzed (16 patients, 22 siblings, and 29 controls). Significant group differences were found in most of the cognitive domains; the patients had the lowest scores. The Kruskal-Wallis test revealed significant group differences in CORT levels (p < 0.01), TEST (p < 0.01), and DHT (p < 0.001); no difference was found in PROG, DHEA, and DOC. All cognitive domains, except for attention, were affected by the NAS levels. CORT levels of patients correlated with speed of processing (r = 0.55) and working memory (r = 0.52), while PROG levels correlated with abstraction (r = −0.63). In siblings, there was a negative correlation between TEST levels and verbal memory (r = −0.51) and PROG with attention (r = −0.47). Conclusions: Our results verified that individual domains of cognitive deficit (abstraction and verbal memory) can be considered as an endophenotype of psychosis. Higher levels of cortisol and testosterone in siblings are consistent with high-risk states for psychosis. Multiple interactions between NAS and cognitive functioning, particularly memory functions, were observed. Study limitations (small sample size and administration of antipsychotic medication) did not allow us to establish unequivocally NAS as an endophenotype.

The first part of schizophrenia study examined the needs and gaps in the patients' care pathway. Descriptive analysis was based on an inventory of needs and treatment opportunities, using focus group sessions, expert interviews, users' input, and literature review. Three patient pathways were selected: indicated prevention, duration of untreated psychosis, and relapse prevention. The analysis identified several critical barriers to optimal treatment. Available health care services often miss or delay detection of symptoms and diagnosis in at-risk individuals. The aim of this study, conducted within the European Brain Council project "The Value of Treatment", was to estimate costs and the potential cost- savings associated with adopting these services within the context of the Czech mental health care reform. Czech epidemiological data, probabilities derived from meta-analyses, and data on costs of mental health services in the Czech Republic were used to populate a decision analytical model. From the health care and societal perspectives, costs associated with health care services and productivity lost were taken into account. Although several studies have reported altered circulating steroids accompanying various mental disturbances, knowledge about alterations in the peripheral steroid pattern in such pathologies is incomplete. Therefore, we attempted to add to this knowledge using the simultaneous quantification of circulating steroids by gas chromatography mass spectrometry (GC-MS) in groups of premenopausal women in the follicular phase of the menstrual cycle (22 women with depression, 17 with anxiety disorders, 17 healthy controls). In addition to age-adjusted analysis of covariance (ANCOVA) followed by multiple comparisons, we developed models to successfully discriminate these groups from each other on the basis of steroid levels. Risperdal Consta - dlouhodobě působící injekce v léčbě schizofrenie a schizoafektivní poruchy: předběžné 12měsíční výsledky projektu e-STAR v České a Slovenské republice

E-STAR je mezinárodní dlouhodobá, observační studie zaměřená na pacienty, kteří jsou pro schizofr... more E-STAR je mezinárodní dlouhodobá, observační studie zaměřená na pacienty, kteří jsou pro schizofrenii nebo schizoafektivní poruchu převedeni na léčbu Risperidonem Consta dlouhodobě působícími injekcemi (Consta). Data jsou získávána retrospektivně za období jednoho roku a prospektivně v tříměsíčních intervalech po dobu dvou let. Tato předběžná analýza reprezentuje výsledky skupiny 280 nemocných, kteří dokončili jednoroční prospektivní sledování léčby Constou v České a Slovenské republice. Jednoroční výsledky ukazují, že Consta redukovala počet relapsů a potřebu hospitalizace a zvýšila podíl nemocných v remisi. Léčba Constou byla spojena se zlepšením denních životních aktivit a zlepšeno bylo celkové klinické hodnocení nemocných, navíc vedla k významné redukci suicidálních tendencí a násilného jednání. Podíl nemocných léčených kombinacemi antipsychotik byl při léčbě Constou menší než v retrospektivním jednoročním období. Risperidon ve formě dlouhodobě působících injekcí zlepšuje adherenci nemocných schizofrenií a schizoafektivní poruchou k antipsychotické léčbě. Projekt e-STAR umožňuje hodnotit farmakologickou léčbu schizofrenie a schizoafektivní poruchy v prostředí běžné klinické praxe.
